AB - A specific feature of dyslipidemia in pregnancy is increased high-density lipoprotein (HDL) cholesterol concentration, which is probably associated with maternal endothelium protection. However, preeclampsia is most often associated with low HDL cholesterol, and the mechanisms behind this change are scarcely explored. We aimed to investigate changes in HDL metabolism in risky pregnancies and those complicated by late-onset preeclampsia. We analyze cholesterol synthesis (cholesterol precursors: desmosterol, 7-dehydrocholesterol, and lathosterol) and absorption markers (phytosterols: campesterol and β-sitosterol) within HDL particles (NCSHDL), the activities of principal modulators of HDL cholesterol’s content, and major HDL functional proteins levels in mid and late pregnancy. On the basis of the pregnancy outcome, participants were classified into the risk group (RG) (70 women) and the preeclampsia group (PG) (20 women). HDL cholesterol was lower in PG in the second trimester compared to RG (p < 0.05) and followed by lower levels of cholesterol absorption markers (p < 0.001 for campesterolHDL and p < 0.05 for β-sitosterolHDL). Lowering of HDL cholesterol between trimesters in RG (p < 0.05) was accompanied by a decrease in HDL phytosterol content (p < 0.001), apolipoprotein A-I (apoA-I) concentration (p < 0.05), and paraoxonase 1 (PON1) (p < 0.001), lecithin–cholesterol acyltransferase (LCAT) (p < 0.05), and cholesterol ester transfer protein (CETP) activities (p < 0.05). These longitudinal changes were absent in PG. Development of late-onset preeclampsia is preceded by the appearance of lower HDL cholesterol and NCSHDL in the second trimester. We propose that reduced capacity for intestinal HDL synthesis, decreased LCAT activity, and impaired capacity for HDL-mediated cholesterol efflux could be the contributing mechanisms resulting in lower HDL cholesterol.

AB - The 1st trimester of pregnancy is accompanied with changes in different biochemical and
hematological parameters. Analyses scheduled to be performed in the 1st trimester are complete
blood count, blood group, Rh factor and the double test. Many experts also suggest the
determination of lipid status parameters as a routine analysis in the early pregnancy. Reliable data
about maternal and fetal health can be obtained by the assessment of the above-mentioned
parameters. They may be helpful in assessing the risk for pregnancy complication development
and/or perinatal adverse outcomes.
AB - Prvi trimestar trudnoće praćen je promenama različitih biohemijskih i hematoloških
parametara.
i tzv. „double“ test. Mnogi stručnjaci predlažu određivanje parametara lipidnog statusa kao
rutinsku analizu u ranoj trudnoći. Procenom gore navedenih parametara mogu se dobiti pouzdani
podaci o zdravlju majke i fetusa, a mogu da posluže i za procenu rizika za razvoj komplikacija u
trudnoći i/ili perinatalnih neželjenih ishoda.

AB - Background/Aim. One of the complications that can oc-
cur during pregnancy is the development of preeclampsia
(PE). The main characteristics of this condition are high
blood pressure and very often signs of kidney damage or
other organ damage. The condition affects 5–7% of all
pregnant women and is one of the main factors of maternal
and perinatal morbidity and mortality worldwide. The aim
of this study was to investigate the structural and functional
modifications of high-density lipoprotein (HDL) particles
during high-risk pregnancies (HRP) for PE development.
Methods. The longitudinal prospective study included a to-
tal of 91 pregnant women with a HRP for developing PE.
Out of this total number, 71 women did not develop PE
until delivery, and this group was designated as the group
without PE (WPE). The rest of the 20 HRP women devel-
oped PE before delivery and were designated as the PE
group. The blood was sampled toward the end of each tri-
mester and before the delivery. The distribution of HDL
particles was determined by the vertical 3–31% polyacryla-
mide gradient gel electrophoresis method. The antioxidative capacity of HDL particles was measured by the activity of
the HDL-associated enzyme – paraoxonase 1 (PON1).
PON1 activity was determined by the method of kinetic
spectrophotometry from serum samples. Results. The re-
sults have shown that the proportions of HDL 2b particles
significantly increased in the 2nd trimester (p ˂ 0.05) and
remained increased until the end of pregnancy in the WPE
group. PON1 activity was significantly higher in the 3 rd tri-
mester (p < 0.05) of the WPE group. In the PE group, we
found that the proportions of HDL 3a particles significantly
decreased in the 2 nd trimester (p < 0.05) and remained de-
creased until the end of pregnancy. PON1 activity has not
changed in the PE group during pregnancy. Conclusion.
Dyslipidemia in pregnancy could be associated with differ-
ent modifications of HDL particles. The adaptive pregnancy
mechanisms expressed as a functional modification of HDL
particles in pregnant women who develop PE seem inade-
quate and, therefore, lose their atheroprotective role.
AB - Uvod/Cilj. Jedna od komplikacija koja se može javiti
tokom trudnoće je razvoj preeklampsije (PE). Glavne
karakteristike ovog stanja su visok krvni pritisak i vrlo često
znaci oštećenja bubrega ili drugih organa. Ovo stanje
metodom vertikalne elektroforeze u 3–31% gradijentu
poliakrilamidnog gela. Antioksidativni kapacitet HDL
čestica je određivan na osnovu aktivnosti enzima
paraoksonaze 1 (PON1) vezanog za HDL. Aktivnost
PON1 u serumu je određivana metodom kinetičke
spektrofotometrije. Rezultati.
Udeo HDL 3a
čestica se značajno smanjio u drugom trimestru u PE grupi
trudnica (p < 0,05) i ostao je snižen do kraja trudno će.
Aktivnost PON1 enzima u PE gripi se nije menjala u toku
trudnoće. Zaključak. Dislipidemija u trudnoći može biti
posledica različitih modifikacija HDL čestica. Strukturne i
ulogu.

AB - Metabolic disorders in pregnancy, particularly gestational diabetes mellitus (GDM), are associated with an increased risk for adverse pregnancy outcome and long-term cardiometabolic health of mother and child. This study analyzed changes of serum cholesterol synthesis and absorption markers during the course of high-risk pregnancies, with respect to the development of GDM. Possible associations of maternal lipid biomarkers with neonatal characteristics were also investigated. The study included 63 women with high risk for development of pregnancy complications. Size and proportions of small low-density (LDL) and high-density lipoprotein (HDL) particles were assessed across trimesters (T1–T3), as well as concentrations of cholesterol synthesis (lathosterol, desmosterol) and absorption markers (campesterol, β-sitosterol). During the study, 15 women developed GDM, while 48 had no complications (non-GDM). As compared to the non-GDM group, women with GDM had significantly higher triglycerides in each trimester, while having a lower HDL-C level in T3. In addition, they had significantly lower levels of β-sitosterol in T3 (p < 0.05). Cholesterol synthesis markers increased across trimesters in both groups. A decrease in serum β-sitosterol levels during the course of pregnancies affected by GDM was observed. The prevalence of small-sized HDL decreased in non-GDM, while in the GDM group remained unchanged across trimesters. Newborn’s size in the non-GDM group was significantly higher (p < 0.01) and inversely associated with proportions of both small, dense LDL and HDL particles (p < 0.05) in maternal plasma in T1. In conclusion, high-risk pregnancies affected by GDM are characterized by altered cholesterol absorption and HDL maturation. Advanced lipid testing may indicate disturbed lipid homeostasis in GDM.

Advanced lipid testing may indicate disturbed lipid homeostasis in GDM.",

AB - Despite significant progress in improving pregnancy outcomes in recent decades,
predicting the risk and treatment of preeclampsia are still major challenges in clinical
practice (1). The aim of this study was to examine non-routine biomarkers in preeclampsia
risk assessment. The study involved 90 women with high-risk pregnancies, 20 of whom
developed preeclampsia by the end of pregnancy. Biochemical parameters were determined
between the 12th and 13 th weeks of gestation. The results of the study showed that women
who later developed preeclampsia had higher concentrations of lathosterol, cholesterol
synthesis marker (p <0.05), inflammatory proteins - monocyte chemoattractant protein-1
(MCP-1), and resistin (p <0.01, both), as well as paraoxonase-1 (PON1) activity (p <0.05).
Binary logistic regression analysis showed that higher concentrations of lathosterol, MCP-1,
resistin, and PON-1 were associated with preeclampsia development. To determine whether
the parameters significant in univariate analysis, are independent predictors of
preeclampsia, we applied multivariate regression analysis. Clinical markers commonly used
in risk assessment (maternal age and body mass index, mean arterial pressure, and uterine
blood flow), lathosterol, MCP-1, resistin, and PON-1 were included in the model. MCP-1 and
resistin stood out as significant independent predictors of preeclampsia. The diagnostic
accuracy of the investigated model was excellent (AUC=0.859). The study results indicated
the importance of a multi-marker approach in risk assessment for preeclampsia
development.
<0,05), inflamatornih proteina - monocitnog hemoatraktantnog proteina-1 (MCP-1) i
rezistina (p < 0,01, oba), kao i aktivnost enzima paraoksonaze-1 (PON1) (p <0,05). Binarna
primjenili smo multivarijantnu regresionu analizu.
uobičajeno koriste u procjeni rizika (starost i indeks tjelesne mase majke, srednji arterijski
pritisak i protok krvi kroz matericu), latosterol, MCP-1, rezistin i PON-1. MCP-1 i rezistin su
se istakli kao značajni nezavisni prediktori preeklampsije. Pokazana je odlična dijagnostička
tačnost ispitivanog modela (AUC=0,859). Rezultati ove studije su ukazali na značaj
multimarkerskog pristupa u procjeni rizika za razvoj preeklampsije.

mechanisms underlying endothelial dysfunction and atherosclerosis development in non-
pregnant state, but in pregnancy they are adaptive mechanisms responsible for fetal
development without health risks for mothers. A special feature of lipids profile in pregnancy
is increase in high density lipoprotein (HDL) cholesterol concentration. Protective role of
HDL particles resides on its complex composition that goes beyond its cholesterol
concentration and exhibits vasodilatory, antioxidant, anti-inflammatory and
antithrombogenic effects. Even the importance of HDL functionality in pathogenesis of
cardiovascular disease has been investigated and confirmed its role in persevering metabolic
health in pregnancy are almost unexplored. Project "HDL research to improve pregnancy
outcome" investigate the pattern of HDL particle distribution and functionality (HDL-ome)
with different pregnancy outcomes. With the sophisticated analytical methods we determine
HDL-ome in uncomplicated and high-risk pregnancies. We mark the HDL-ome component
with a different pattern of change during high-risk pregnancies and test their potential for
pregnancy complications risk assessment. In this this research we investigate molecular
mechanisms crucial for HDL remodelling during pregnancy and possible associations
between maternal metabolic health during pregnancy and maternal and child health in the
future. Such comprehensive approach will provide a better insight into HDL role in maternal
metabolic health during pregnancy and will open a door for further research in this area.

porast koncentracije holesterola u lipoproteinskim česticama visoke gustine (high density
lipoprotein, HDL).
antioksidativne, antiinflamatorne i antitrombogene efekte.

AB - Introduction The link between preeclampsia and dyslipidemia has been established. Even though lipid profile parameters have been intensively investigated in the pathology of preeclampsia, their accurate molecular mechanisms of action have not been fully decoded. Objectives We aimed to identify the specifics of cholesterol metabolism in women affected by late‑onset preeclampsia and single out potential biomarkers associated with late‑onset syndrome. Patient s and methods A total of 90 pregnant women with a priori risk for preeclampsia were monitored at 4 time points during gestation and, based on the outcome of pregnancy, they were classified into the high‑risk group (70 women) and the preeclampsia group (20 women). Cholesterol metabolic profiling was done using liquid chromatography‑tandem mass spectrometry. Result s The only significant change in the preeclampsia group was an increase in the lathosterol level (P = 0.001). The first‑trimester lathosterol level was higher in the preeclampsia group compared with the high‑risk group (P = 0.02). Further, in the preeclampsia group, positive correlations were found between desmosterol and β‑sitosterol (ρ = 0.474; P = 0.03) in the third trimester, desmosterol and campesterol changes between the second and the first (ρ = 0.546; P = 0.02), and the third and first trimesters (ρ = 0.754; P <0.001), as well as between the desmosterol and β‑sitosterol differences between the third and first trimesters (ρ = 0.568; P = 0.01). No similar correlations were found in the high‑risk group. Conclusions Late‑onset preeclampsia could be associated with an altered lipid profile. By studying the quantitative metabolic signatures of cholesterol, we might assume that both cholesterol synthesis and absorption are increased, that is, there is an imbalance in the cholesterol homeostasis regulation in women affected by the disease.

No similar correlations were found in the high‑risk group. Conclusions Late‑onset preeclampsia could be associated with an altered lipid profile. By studying the quantitative metabolic signatures of cholesterol, we might assume that both cholesterol synthesis and absorption are increased, that is, there is an imbalance in the cholesterol homeostasis regulation in women affected by the disease.",

AB - Objectives: The goal of this study was to investigate metabolic changes in lipids and oxidative stress parameters in the first trimester of pregnancy with the more specific aim of estimating the significance and strength of researched parameters in the prediction of preeclampsia. Design and Methods. The study included 87 high-risk pregnant (HRG) female subjects, 14 with developed preeclampsia (PEC) and 43 healthy pregnant female subjects matched for gestational age (CG). Thiobarbituric acid-reactive substances (TBARS) concentration, lipid hydroperoxides (LOOH), pro-oxidant antioxidant balance (PAB) and total oxidative status (TOS) were measured as oxidative stress markers, while total antioxidant capacity (TAC) was measured as an antioxidative defense parameter. The Atherogenic Index of Plasma (AIP) was calculated as the base 10 logarithm of the ratio of the plasma concentration of triglycerides (TG) to the plasma concentration of highdensity lipoprotein cholesterol (HDL-C), with each concentration expressed in mmol/L. Results: The results have shown that lipid indices, especially AIP, were significantly higher in the first trimester of HRG (p < 0.001) and PEC (p < 0.001). Oxidative stress parameters were significantlly higher, while TAC was significantly lower in HRG vs. CG [0.7 ± 0.15 vs 1.1 ± 0.16; (p < 0.001)] and in PEC [0.6 ± 0.12 vs 1.1 ± 0.16; (p < 0.001)] vs. CG. Also, in the HRG, results have shown an independent association of AIP with the preeclampsia development (p < 0.05), while placental growth factor did not show the expected level of significance (p = 0.648). Analysis of the Receiver Operating Characteristics (ROC) curves indicated that certain parameters included in the research model have very good diagnostic accuracy for preeclampsia (AUC = 0.856). Conclusions: AIP is associated with high-risk pregnancies. Furthermore, our results firmly underscored AIP as a potential marker for preeclampsia prediction.

AB - Background: Specific metabolic changes during physiological pregnancy are characterized by hyperlipidemia and increased oxidative stress. However, these specific changes raise the question of their pro-atherogenic potential during pregnancy and their influence on the risk of developing cardiovascular disease (CVD) in women later in life, as well as complications during pregnancy. The aim of this study was to investigate the changes in lipid indexes and parameters of lipid peroxidation in non-complicated pregnancy in order to estimate their course of change and potential relationship during non-complicated pregnancy. Methods: The study included 43 healthy pregnant women and 38 non-pregnant healthy women, in appropriate ages, as the control group. Lipid parameters and oxidative stress parameters were monitored in a longitudinal study in the first, second and third trimesters, and before delivery during non-complicated pregnancy. Results: Results have shown that lipid indexes rise during pregnancy. The values were significantly increased when compared to the first trimester in all lipid indices and in comparison with the control group. Thyobarbituric acid reactive substances (TBARS) and lipid hydroperoxides (LOOH) were not changed significantly during physiological pregnancy, but LOOH showed a significantly higher value in the first trimester compared with the control group. Prooxidative-antioxidative balance (PAB) significantly increases as pregnancy progresses. Conclusions: We observed the changes in lipids, lipid indexes and parameters that indicate oxidative modification of lipids in physiological pregnancy that may lead to an atherogenic, prooxidant state.

AB - Specifically altered lipid profile and physiological hyperlipidemia during pregnancy are considered essential for the normal course of pregnancy and fetal development. This specific alteration of the lipid profile raises the questions about potential proaterogenic effect of these altered lipid parameters during pregnancy and its influence on the development of cardiovascular disease in women later in life. Research topic was also the association of altered lipid profile during pregnancy with the development of complications in pregnancy, especially gestational diabetes, hypertension and preeclampsia. Through the mediation of cholesterol ester transfer protein (CETP), the activity of which grows in mid-gestation, there are exchanges of the triglycerides between VLDL and LDL or HDL particle, which leads to increased accumulation of triglycerides in these particles, causes them to become smaller and denser with much greater atherogenic potential. These changes in lipid profile point out that a large number of pregnancies increase risk of development of cardiovascular diseases later in life. In order to optimize the predictive capacity of the lipid profile during pregnancy, it is recommended to determine the indexes of lipid.

As a physiological condition closely linked with increased susceptibility to oxidative stress, pregnancy can be further compromised by cigarette smoking. Inadequate nutrition and reduced intake of antioxidants can also disrupt the prooxidant/antioxidant relationship and contribute to oxidative stress. Increased oxidative stress during pregnancy may be involved in several complications of pregnancy, such as preterm labor, fetal growth restriction, preeclampsia and Miscarriage. Objectives. The aim of this study was to investigate the influence of maternal smoking habits before pregnancy on the parameters of oxidative stress and the antioxidative defense system, lipid profile parameters and paraoxonase-1 (PON1) activity during the third trimester of uncomplicated pregnancies. Material and Methods. Healthy pregnant women (n = 86) were divided into non-smoking and smoking groups, and into groups taking vitamin supplements and not taking them. Oxidative damage was measured through the levels of thiobarbituric acid-reacting substances (TBARS) and plasma antioxidant status was evaluated by measuring total antioxidant capacity (TAC). Results. TBARS concetration was significantly higher (p lt 0.05) and PON1 activity was significantly lower (p lt 0.05) in the smokers' group. No significant differences were found in the investigated parameters in relation to vitamin supplement intake. Conclusions. Habitual smoking before pregnancy is associated with increased oxidative stress. Vitamin supplementation has no effect on the oxidative stress status of healthy pregnant women.

AB - Pregnancy is associated with alterations in low-density lipoprotein (LDL) and high-density lipoprotein (HDL) subclasses, but the exact pattern of these variations remains controversial. This study investigates longitudinal changes of plasma LDL and HDL particles distributions during the course of normal pregnancy, as well as associations of maternal LDL and HDL subclasses distributions before delivery with parameters of newborn size. Blood samples were collected from 41 healthy pregnant women throughout entire pregnancy, before delivery and 7 weeks postpartum. LDL and HDL subclasses were determined by gradient gel electrophoresis, while other biochemical parameters were measured by standard laboratory methods. During gestation LDL size significantly decreased (P lt 0.001), due to reduction in relative proportion of LDL I (P lt 0.01) and increase of LDL II (P lt 0.001) and IIIA (P lt 0.05) subclasses. In the same time, HDL size and proportions of HDL 2a particles significantly decreased (P lt 0.001), with concomitant increase of HDL 3b and 3c subclasses (P lt 0.05). Observed alterations were associated with changes in serum triglyceride levels. Rearrangement in LDL subclasses distribution during gestation was transient, while postpartum HDL subclasses distribution remained shifted toward smaller particles. Higher proportion of LDL IVB in maternal plasma before delivery was an independent predictor of smaller birth weights and lengths, while higher proportions of LDL IVB and HDL 2a subclasses were independent determinants of newborns' smaller head circumferences. Routine gestational and prenatal care in otherwise normal pregnancy could be complemented with evaluation of LDL and HDL particles distribution in order to ensure an adequate size of the newborn.

AB - The purpose of the present study was to determine changes in plasma paraoxonase-1 activity (an indicator of paraoxonase phenotype) throughout normal pregnancy and its relationship with maternal oxidative stress status. The frequencies of the paraoxonase-1 phenotype in the studied population were determined using a two-substrate (paraoxon/diazoxon) activity method. As a parameter of oxidative stress status we measured the redox balance. Paraoxonase-1 activity significantly decreased at gestational week 32. In addition, the lipid profile was more atherogenic. Redox balance was significantly increased across gestational weeks. There were independent direct associations between maternal smoking habits before pregnancy, glucose concentrations and redox balance with PON1 activity in the third trimester. This study shows that pregnancy is followed by a decrease in PON1 activity and increased risk for development of cardiovascular diseases. We conclude that changes in paraoxonase-1 status during pregnancy are associated with maternal oxidative stress status and smoking habits.

AB - Background: Pregnancy is a stressful condition linked with altered lipid profile, increased oxidative stress and increased inflammation processes. The purpose of the present study was to determine the associations between those alterations with increased weight gain during pregnancy. Methods: The atherogenic index of plasma (AIP) and oxidative stress status parameters were determinated in 50 healthy and 172 pregnant women with non-complicated pregnancy. Pregnant women were divided in four groups according to body mass index (BMI) values (BMI quartiles). Results: Oxidative stress parameters were significantly lower in the control group compared with all the pregnant women quartiles. Unexpectedly, differences in oxidative stress parameters between BMI quartiles groups were not significant. The antioxidant defence parameters remained quite similar in the different BMI quartiles. Weight gain and paraoxonase-1 (PON1) activities were independently associated with increased AIP while weight gain and triglyceride concentration were found to be significant predictors of PON1 activities. Conclusions: The results of our current study indicate the association of maternal weight gain during pregnancy and altered lipid profile, elevated oxidative stress and changed antioxidative capacity of PON1. Taken together all these facts indicate possible increased risk of cardiovascular disease (CVD) development in later life if the weight gain during pregnancy is excessive.
