AB  - Background: The pre-analytical (PA) phase is the most vulnerable phase of laboratory testing procedure, with critical procedures-collection, handling, sample transport, and time and temperature of sample storage. The aim of this study was to examine if different anticoagulants, storage conditions, and freeze-thaw cycles (FTCs) influence the concentrations of basic biochemical parameters. In parallel, the presence and the effect of sample microbiological contamination during routine laboratory work were examined.

Methods: Two plasma pools (EDTA, and sodium-fluoride/potassium oxalate plasma (NaF)) were stored at +4C˚/-20˚C. Total cholesterol (TC), glucose, triglycerides (TG), urea, total protein (TP), and albumin concentrations were measured using Ilab 300+. Sample microbiological contamination was determined by 16S rRNA sequence analysis. The experiment encompassed a 5 day-period: Day 1–fresh sample, Day 2–1st FTC, Day 3–2nd FTC, Day 4–3rd FTC, Day 5–4th FTC. The appearance of bacteria in two consecutive samples was the experiment's endpoint.

Results: During 4 FTCs there were no changes in plasma urea concentrations. Glucose was stable in EDTA+4˚C and NaF- 20˚C until the 3rd FTC (P=0.008, P=0.042, respectively). Changes in protein concentrations followed the zig-zag pattern. TG concentrations changed significantly in the EDTA-20˚C sample after 1st and 4th FTCs (P=0.022, P=0.010, respectively). In NaF samples no contamination was observed during 4 FTCs.

 

Conclusions: Urea and glucose concentrations were robust. Changes in lipid and protein concentrations after FTCs follow complex patterns. Bacterial growth was not observed in NaF plasma samples. This can promote NaF use in analytical procedures in which microbiological contamination affects the quality of analysis.

prikupljanje, rukovanje, transport i skladi{tenje uzoraka, i
predstavlja najzna~ajniji izvor laboratorijskih gre{aka. Cilj
ovog istra`ivanja je bio da se ispita stabilnost osnovnih
biohemijskih parametara u zavisnosti od uslova skladi{tenja
uzoraka i broja ciklusa zamrzavanja-odmrzavanja (FTC).
Pored toga, ispitivano je prisustvo bakterijske kontaminacije
uzoraka tokom rutinskog laboratorijskog rada.
Metode: Dva »pool«-a plazme (etilendiaminotetrasir}etna
kiselina (EDTA) i natrijum-fluorid/kalijum oksalat (NaF)) su
skladi{tena na +4 ˚C/-20 ˚C. Koncentracije ukupnog

300+ analizatoru. Bakterijska kontaminacija uzoraka je
potvr|ena 16S rRNA sekvencioniranjem. Eksperiment je
sproveden tokom 5 uzastopnih dana: 1. dan – sve` uzorak,
2. dan – 1. FTC, 3. dan –2. FTC, 4. dan – 3. FTC, 5. dan
– 4. FTC. Zavr{nu ta~ku eksperimenta predstavljala je
pojava bakterija u dva uzastopna uzorka.
Rezultati: Tokom 4 FTC koncentracije uree u plazmi se nisu
zna~ajno razlikovale. Koncentracija glukoze je bila stabilna
u EDTA +4 ˚C i NaF -20 ˚C do 3.FTC (P=0,008,
P=0,042, redom). Koncentracije TG su se zna~ajno pro-
menile u uzorku EDTA -20 ˚C nakon 1. i 4. FTC-a (P=0,022, P=0,010, redom). U uzorcima NaF plazme
nije do{lo do bakterijske kontaminacije tokom 4. FTC.
Zaklju~ak: Koncentracije uree i glukoze su bile stabilne
tokom trajanja eksperimenta. Promene u koncentracijama
lipida nakon FTC prate slo`ene obrasce. Rast bakterija nije
prime}en u uzorcima NaF plazme, te upotreba ovog anti-
koagulansa mo`e biti opravdana u analiti~kim proce -
durama podlo`nim uticaju mikrobiolo{ke kontaminacije.

Cholesterol Metabolic Profiling of HDL in Women with Late-Onset Preeclampsia

AB  - A specific feature of dyslipidemia in pregnancy is increased high-density lipoprotein (HDL) cholesterol concentration, which is probably associated with maternal endothelium protection. However, preeclampsia is most often associated with low HDL cholesterol, and the mechanisms behind this change are scarcely explored. We aimed to investigate changes in HDL metabolism in risky pregnancies and those complicated by late-onset preeclampsia. We analyze cholesterol synthesis (cholesterol precursors: desmosterol, 7-dehydrocholesterol, and lathosterol) and absorption markers (phytosterols: campesterol and β-sitosterol) within HDL particles (NCSHDL), the activities of principal modulators of HDL cholesterol’s content, and major HDL functional proteins levels in mid and late pregnancy. On the basis of the pregnancy outcome, participants were classified into the risk group (RG) (70 women) and the preeclampsia group (PG) (20 women). HDL cholesterol was lower in PG in the second trimester compared to RG (p < 0.05) and followed by lower levels of cholesterol absorption markers (p < 0.001 for campesterolHDL and p < 0.05 for β-sitosterolHDL). Lowering of HDL cholesterol between trimesters in RG (p < 0.05) was accompanied by a decrease in HDL phytosterol content (p < 0.001), apolipoprotein A-I (apoA-I) concentration (p < 0.05), and paraoxonase 1 (PON1) (p < 0.001), lecithin–cholesterol acyltransferase (LCAT) (p < 0.05), and cholesterol ester transfer protein (CETP) activities (p < 0.05). These longitudinal changes were absent in PG. Development of late-onset preeclampsia is preceded by the appearance of lower HDL cholesterol and NCSHDL in the second trimester. We propose that reduced capacity for intestinal HDL synthesis, decreased LCAT activity, and impaired capacity for HDL-mediated cholesterol efflux could be the contributing mechanisms resulting in lower HDL cholesterol.

AB  - The 1st trimester of pregnancy is accompanied with changes in different biochemical and
hematological parameters. Analyses scheduled to be performed in the 1st trimester are complete
blood count, blood group, Rh factor and the double test. Many experts also suggest the
determination of lipid status parameters as a routine analysis in the early pregnancy. Reliable data
about maternal and fetal health can be obtained by the assessment of the above-mentioned
parameters. They may be helpful in assessing the risk for pregnancy complication development
and/or perinatal adverse outcomes.
AB  - Prvi trimestar trudnoće praćen je promenama različitih biohemijskih i hematoloških
parametara. Analize koje se rade u 1. trimestru su kompletna krvna slika, krvna grupa, Rh faktor
i tzv. „double“ test. Mnogi stručnjaci predlažu određivanje parametara lipidnog statusa kao
rutinsku analizu u ranoj trudnoći. Procenom gore navedenih parametara mogu se dobiti pouzdani
podaci o zdravlju majke i fetusa, a mogu da posluže i za procenu rizika za razvoj komplikacija u
trudnoći i/ili perinatalnih neželjenih ishoda.

AB  - Current clinical data show that, despite constant efforts to develop novel therapies and clinical approaches, atherosclerotic cardiovascular diseases (ASCVD) are still one of the leading causes of death worldwide. Advanced and unstable atherosclerotic plaques most often trigger acute coronary events that can lead to fatal outcomes. However, despite the fact that different plaque phenotypes may require different treatments, current approaches to prognosis, diagnosis, and classification of acute coronary syndrome do not consider the diversity of plaque phenotypes. Long non-coding RNAs (lncRNAs) represent an important class of molecules that are implicated in epigenetic control of numerous cellular processes. Here we review the latest knowledge about lncRNAs’ influence on plaque development and stability through regulation of immune response, lipid metabolism, extracellular matrix remodelling, endothelial cell function, and vascular smooth muscle function, with special emphasis on pro-atherogenic and anti-atherogenic lncRNA functions. In addition, we present current challenges in the research of lncRNAs’ role in atherosclerosis and translation of the findings from animal models to humans. Finally, we present the directions for future lncRNA-oriented research, which may ultimately result in patient-oriented therapeutic strategies for ASCVD.

Izazovi primene hromatografskih tehnika u postavljanju dijagnoze SARS-CoV-2 infekcije

AB  - Preventivne mere, rano otkrivanje i potvrda novih slučajeva infekcije, predstavljaju osnov u
sprečavanju širenja i suzbijanju ininfektivne bolesti COVID-19. Primena visoko osetljive, spe-
cifične real-time RT-PCR metode predstavlja zlatni standard u detekciji SARS-CoV-2. Brza i
pouzdana dijanoza su neophodni za efikasno praćenje bolesti, ali veliki broj lažno negativnih
slučajeva omogućio je nekontrolisanu transmisiju infekcije.
Nove metode za detekciju SARS-CoV-2 iz nazofaringealnog brisa zasnovane na principu
tečne hromatografije sa masenom spektrometrijom (HPLC-MS/MS) isključivo se koriste u
istraživačke svrhe. Hromatografski testovi omogućavaju istovremenu detekciju više različitih,
specifičnih peptidnih markera za identifikaciju SARS-CoV-2. Na ovaj način, moguće mutacije
u genskoj sekvenci virusa, lako mogu biti prevaziđene. Upotreba gasne hromatografije sa
spektrometrijom pokretljivosti jona (GC-IMS) za detekciju odabranih molekula u izdahnutom
vazduhu pacijenata sa COVID-19 može omogućiti neinvazivnu, brzu i tačnu, „point of care”
potvrdu dijagnoze bolesti.
Uprkos superiornim analitičkim performansama hromatografskih tehnika, njihova primena
u rutinskoj laboratorijskoj praksi je retka. Pored opreme, njihova primena zahteva obučeno
osoblje i „in house” procedure validacije i verifikacije metoda. Protokoli validacije hromato-
grafskih metoda se oslanjaju na preporuke date u naučnim publikacijama i različitim smer-
nicama, te su istraživački orijentisani. Najčešće korišćene su EMA, FDA i CLSI smernice za
postupke i procedure validacije metoda. Međutim, ove smernice dozvoljavaju različita tuma-
čenja i ostavljaju analitičaru da odluči koji od parametara validacije su neophodni. Različiti
preanalitički i analitički aspekti hromatografskih metoda diktiraju složenost kriterijuma vali-
dacije. Zbog toga je neophodno izdvojiti najvažnije postupke validacije hromatografskih teh-
nika (određivanje linearnosti, LOQ i LOD vrednosti, tačnosti i preciznosti metode) i primeniti
dostupnu opremu i naučna saznanja.

Interleukin-6: uloga u laboratorijskoj dijagnostici inflamacije i ograničenja pri određivanju

AB  - Uvod i cilj. Određivanje koncentracije interleukina-6 (IL-6) od velikog je značaja za pra-
ćenje toka bolesti i oporavka pacijenata od COVID-19 infekcije. Cilj ovog ispitivanja je da
se objasni uticaj koncentracije biotina kao interferencije na određivanje koncentracije IL-6
nekompetitivnom ECLIA metodom.
Metode. Korišćene su ECLIA i hemiluminiscentne metode za određivanje koncentracije
IL-6 (Roche e411 - ECLIA metoda i IMMULITE 1000 - hemiluminiscencija). Za procenu interfe-
rencije od značaja su bili anamnestički, kao i podaci o terapiji koju pacijent koristi.
Rezultati. Dobijene su značajne razlike u koncentracijama IL-6 prilikom određivanja sa
ove dve različite metode. Koncentracije IL-6 su bile višestruko niže kod ECLIA metode (<1.5
pg/mL za Roche e411-ECLIA i 8.5 pg/mL za IMMULITE 1000). Interferencija usled prisustva
egzogenog biotina u ispitivanom uzorku može uzrokovati „lažno“ negativne rezultate prilikom
određivanja ovog parametra ECLIA metodom.
Zaključak. Određivanje IL-6 imunohemijskim testovima je ograničeno mogućim interferen-
cijama. Egzogeni biotin u visokim koncenracijama značajno utiče na nivo IL-6 u serumu kada
se određivanje vrši ECLIA metodom. Preporuka je informisati pacijente o adekvatnoj pripremi
za imunohemijske analize ukoliko primenjuju oralne preparate koji sadrže biotin i time svesti
mogućnost greške pri izdavanju laboratorijskih rezultata na najmanju moguću meru

AB  - Background and aims: Obstructive sleep apnea (OSA) is closely linked to obesity and related adverse metabolic changes, including dyslipidemia. However, it is not clear whether OSA is an independent contributing factor to dyslipidemia, or the observed association is a reflection of a concomitant presence of obesity. Additionally, dyslipidemia is usually evaluated through measurement of parameters of routine lipid status, while more precise evaluation of lipid homeostasis is rarely performed in OSA. In this study, we analyzed markers of cholesterol synthesis and absorption in patients with OSA with respect to the presence of obesity and the disease severity. Methods and results: This study enrolled 116 OSA patients. Concentrations of non-cholesterol sterols (NCS), measured by LC-MS/MS, were used as markers of cholesterol synthesis and absorption. Apnea-hypopnea index (AHI) and oxygen saturation (SaO2) were utilized as markers of OSA severity. Serum lipid status parameters were determined by routine enzymatic methods. Markers of cholesterol synthesis were increased (P = 0.005), whilst markers of cholesterol absorption decreased (P = 0.001) in obese OSA patients. Cholesterol synthesis/absorption ratio was elevated in obese subjects (P < 0.001). Concentration of cholesterol synthesis marker lathosterol was significantly higher in subjects with severe OSA (P = 0.014) and we observed a trend of decreased cholesterol absorption in these patients. AHI was revealed as an independent determinant of lathosterol concentration (P = 0.022). Conclusions: Our results suggest that the presence of obesity and severe forms of OSA is characterized by elevated endogenous cholesterol synthesis. AHI was singled out as an independent determinant of the serum level of cholesterol synthesis marker lathosterol.

AB  - Metabolic disorders in pregnancy, particularly gestational diabetes mellitus (GDM), are associated with an increased risk for adverse pregnancy outcome and long-term cardiometabolic health of mother and child. This study analyzed changes of serum cholesterol synthesis and absorption markers during the course of high-risk pregnancies, with respect to the development of GDM. Possible associations of maternal lipid biomarkers with neonatal characteristics were also investigated. The study included 63 women with high risk for development of pregnancy complications. Size and proportions of small low-density (LDL) and high-density lipoprotein (HDL) particles were assessed across trimesters (T1–T3), as well as concentrations of cholesterol synthesis (lathosterol, desmosterol) and absorption markers (campesterol, β-sitosterol). During the study, 15 women developed GDM, while 48 had no complications (non-GDM). As compared to the non-GDM group, women with GDM had significantly higher triglycerides in each trimester, while having a lower HDL-C level in T3. In addition, they had significantly lower levels of β-sitosterol in T3 (p < 0.05). Cholesterol synthesis markers increased across trimesters in both groups. A decrease in serum β-sitosterol levels during the course of pregnancies affected by GDM was observed. The prevalence of small-sized HDL decreased in non-GDM, while in the GDM group remained unchanged across trimesters. Newborn’s size in the non-GDM group was significantly higher (p < 0.01) and inversely associated with proportions of both small, dense LDL and HDL particles (p < 0.05) in maternal plasma in T1. In conclusion, high-risk pregnancies affected by GDM are characterized by altered cholesterol absorption and HDL maturation. Advanced lipid testing may indicate disturbed lipid homeostasis in GDM.

AB  - Introduction  The aim of this study was to investigate lipoprotein particle distributions and the likelihood of achieving cholesterol homeostasis in the remission phase of nephrotic syndrome (NS) in paediatric patients. We hypothesized that lipoprotein particle distributions moved toward less atherogenic profile and that cholesterol homeostasis was achieved. Materials and methods  Thirty-three children, 2 to 9 years old with NS were recruited. Blood sampling took place both in the acute phase and during remission. Serum low-density lipoprotein particles (LDL) and high-density lipoprotein particles (HDL) were separated using non-denaturing polyacrylamide gradient gel (3-31%) electrophoresis. Serum non-cholesterols sterols (NCSs), desmosterol, lathosterol, 7-dehydrocholesterol (7-DHC), campesterol and β-sitosterol were measured by high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS). Results  All patients had desirable serum HDL cholesterol concentrations during remission. The dominant lipoprotein diameters and LDL subclass distribution did not change significantly during follow-up. In contrast, HDL lipoprotein particle distribution shifted towards larger particles. The absolute concentration of desmosterol was significantly lower during remission (P = 0.023). β-sitosterol concentration markedly increased during remission (P = 0.005). Desmosterol/β-sitosterol (P < 0.001) and 7-DHC/β-sitosterol (P = 0.005) ratios significantly declined during disease remission. Conclusions  Favourable changes in the serum lipid profiles, HDL particle subclass distribution and cholesterol metabolism in paediatric patients with NS during remission took place. For the first time, we found that cholesterol homeostasis changed in favour of increased cholesterol absorption during disease remission. Nevertheless, complete cholesterol homeostasis was not achieved during disease remission.

AB  - Colorectal cancer (CRC) is a highly prevalent malignancy. Previous studies suggested that cholesterol might play a signficant role in malignant transformation and proliferation. Non-cholesterol sterols (NCS), which are transported by serum lipoproteins alongside cholesterol, are regarded as cholesterol synthesis and absorption markers. Quantification of NCS in serum and HDL fraction (NCSHDL), could provide a better insight into the cholesterol metabolism. The aim of this study was to examine the status of cholesterol synthesis and cholesterol absorption markers in serum and HDL fraction and explore their interrelation in CRC patients. Current study was designed as observational, case-control study. The study included 73 CRC patients and 95 healthy subjects. NCS and NCSHDL concentrations were determined by HPLC-MS/MS. Based on NCS and NCSHDL concentrations, different cholesterol homeostasis indices were calculated. Patients had significantly lower NCS (P<0.001) and NCSHDL concentrations (P<0.001 for desmosterolHDL; P<0.05 for lathosterolHDL, P=0.001 for campesterolHDL, P<0.001 for β-sitosterolHDL). NCSHDL/NCS (P<0.005 for desmosterolHDL/desmosterol; P<0.05 for lathosterolHDL/lathosterol; P<0.001 for both β-sitosterolHDL/β-sitosterol and campesterolHDL/campesterol) and synthesis to absorption ratio (CSI/CAI) (P<0.005) were increased in CRC patients. Additionally, low serum concentrations of desmosterol (P<0.001; OR=0.329; 95%CI (0.199–0.542)) and campesterol (P<0.001; OR=0.540; 95%CI (0.424–0.687)) were independent predictors of CRC presence. Our data suggest that cholesterol homeostasis in CRC is shifted towards increased synthesis. Relative abundance of NCS in HDL particles is increased, suggesting the possible overproduction of cholesterol precursors in peripheral tissues.

NCSHDL/NCS (P<0.005 for desmosterolHDL/desmosterol; P<0.05 for lathosterolHDL/lathosterol; P<0.001 for both β-sitosterolHDL/β-sitosterol and campesterolHDL/campesterol) and synthesis to absorption ratio (CSI/CAI) (P<0.005) were increased in CRC patients. Additionally, low serum concentrations of desmosterol (P<0.001; OR=0.329; 95%CI (0.199–0.542)) and campesterol (P<0.001; OR=0.540; 95%CI (0.424–0.687)) were independent predictors of CRC presence. Our data suggest that cholesterol homeostasis in CRC is shifted towards increased synthesis. Relative abundance of NCS in HDL particles is increased, suggesting the possible overproduction of cholesterol precursors in peripheral tissues.",

Novel biomarkers in preeclampsia risk assessment

AB  - Despite significant progress in improving pregnancy outcomes in recent decades,
predicting the risk and treatment of preeclampsia are still major challenges in clinical
practice (1). The aim of this study was to examine non-routine biomarkers in preeclampsia
risk assessment. The study involved 90 women with high-risk pregnancies, 20 of whom
developed preeclampsia by the end of pregnancy. Biochemical parameters were determined
between the 12th and 13 th weeks of gestation. The results of the study showed that women
who later developed preeclampsia had higher concentrations of lathosterol, cholesterol
synthesis marker (p <0.05), inflammatory proteins - monocyte chemoattractant protein-1
(MCP-1), and resistin (p <0.01, both), as well as paraoxonase-1 (PON1) activity (p <0.05).
Binary logistic regression analysis showed that higher concentrations of lathosterol, MCP-1,
resistin, and PON-1 were associated with preeclampsia development. To determine whether
the parameters significant in univariate analysis, are independent predictors of
preeclampsia, we applied multivariate regression analysis. Clinical markers commonly used
in risk assessment (maternal age and body mass index, mean arterial pressure, and uterine
blood flow), lathosterol, MCP-1, resistin, and PON-1 were included in the model. MCP-1 and
resistin stood out as significant independent predictors of preeclampsia. The diagnostic
accuracy of the investigated model was excellent (AUC=0.859). The study results indicated
the importance of a multi-marker approach in risk assessment for preeclampsia
development.
AB  - Uprkos značajnom napretku u poboljšanju ishoda trudnoće poslednjih decenija,
predviđanje rizika i terapija preeklampsije su još uvijek veliki izazovi u kliničkoj praksi (1).
Cilj ove studije je bio ispitivanje biomarkera koji se ne koriste u rutinskoj praksi u proceni
rizika za razvoj preeklampsije. U studiji je učestvovalo 90 žena sa visokorizičnim
trudnoćama, od kojih je 20 razvilo preeklampsiju do kraja trudnoće. Biohemijski parametri

AB  - Insulin resistance, hyperlipidemia, oxidative stress and low-grade inflammation are
mechanisms underlying endothelial dysfunction and atherosclerosis development in non-
pregnant state, but in pregnancy they are adaptive mechanisms responsible for fetal
development without health risks for mothers. A special feature of lipids profile in pregnancy
is increase in high density lipoprotein (HDL) cholesterol concentration. Protective role of
HDL particles resides on its complex composition that goes beyond its cholesterol
concentration and exhibits vasodilatory, antioxidant, anti-inflammatory and
antithrombogenic effects. Even the importance of HDL functionality in pathogenesis of
cardiovascular disease has been investigated and confirmed its role in persevering metabolic
health in pregnancy are almost unexplored. Project "HDL research to improve pregnancy
outcome" investigate the pattern of HDL particle distribution and functionality (HDL-ome)
with different pregnancy outcomes. With the sophisticated analytical methods we determine
HDL-ome in uncomplicated and high-risk pregnancies. We mark the HDL-ome component
with a different pattern of change during high-risk pregnancies and test their potential for
pregnancy complications risk assessment. In this this research we investigate molecular
mechanisms crucial for HDL remodelling during pregnancy and possible associations
between maternal metabolic health during pregnancy and maternal and child health in the
future. Such comprehensive approach will provide a better insight into HDL role in maternal
metabolic health during pregnancy and will open a door for further research in this area.
AB  - Insulinska rezistencija, hiperlipidemija, oksidativni stress i stanje inflamacije niskog
stepena, koji se u negravidnom stanju povezuju sa razvojem endotelne disfunkcije i
aterosklerozom, u trudnoći predstavljaju adaptivne mehanizme koji omogućavaju pravilan
rast i razvoj fetusa, a ne ugrožavaju zdravlje trudnice. Specifičnost dislipidemije koja se
razvija u trudnoći, a koja nije karakteristična za dislipidemiju u negravidnim stanjima, jeste
porast koncentracije holesterola u lipoproteinskim česticama visoke gustine (high density
lipoprotein, HDL). Međutim, HDL je mnogo više od sadržaja holesterola u njemu. Ovaj
lipoprotein ima izuzetno kompleksnu i heterogenu strukturu i ispoljava vazodilatatorne,
antioksidativne, antiinflamatorne i antitrombogene efekte. Iako je gubitak funkcionalnosti
ovog lipoproteina u patogenezi kardiovaskularnih bolesti detaljno ispitan i potvrđen,
njegova uloga u očuvanju metaboličkog zdravlja u trudnoći još uvek nije rasvetljena. Projekat
"Ispitivanje HDL-metaboloma u cilju unapređenja ishoda trudnoće" bavi se istraživanjem
povezanosti obrazaca promena strukture i funkcionalnosti HDL (HDL-om) sa različitim

tokom trudnoće i, kao takvo, predstavljaće polaznu tačku za dalja istraživanja u ovoj oblasti.

AB  - Vitamin D deficiency is among important healthcare challenges today. Traditionally, vitamin D status is assessed through determination of 25-hydroxy metabolite (25(OH)D), but novel data point to 24,25(OH) 2 D and 25(OH)D/24,25(OH) 2 D ratio (VDMR) as promising biomarkers. It is widely accepted that the biological role of vitamin D exceeds its well-known contribution to bone turnover. However, its effects on overall energy metabolism and lipid status alterations are not completely understood. In this study, we analyzed the relationship of vitamin D status assessed as concentrations of 25(OH)D3 and 24,25(OH) 2 D3 determined by liquid chromatography-tandem mass spectrometry, as well as VDMR with advanced lipid status parameters. Vitamin D status biomarkers, routine parameters of lipid status and size and distribution of lipoprotein subclasses were determined in 89 healthy adults (35 with adequate vitamin D status and 54 with vitamin D deficiency). Our results indicated a preponderance of pro- atherogenic small, dense LDL particles (sdLDL) in vitamin D deficient subjects. Both 25(OH)D and 24,25(OH) 2 D were associated with a relative proportion of sdLDL (B: -0.410; SE: 0.154; P=0.010; and B: -2.041; SE: 0.969; P=0.039, respectively). Positive correlation was found for VDMR and relative proportion of HDL 3a particles (ρ=0.251; P=0.024). VDMR value was decreased in subjects with vitamin D deficiency (P=0.001), thus implying its usefulness as a biomarker. A thorough investigation of novel vitamin D biomarkers and advanced lipid status parameters can be useful in the estimation of individual risk for the development of cardiometabolic alterations.

AB  - Introduction The link between preeclampsia and dyslipidemia has been established. Even though lipid profile parameters have been intensively investigated in the pathology of preeclampsia, their accurate molecular mechanisms of action have not been fully decoded. Objectives We aimed to identify the specifics of cholesterol metabolism in women affected by late‑onset preeclampsia and single out potential biomarkers associated with late‑onset syndrome. Patient s and methods A total of 90 pregnant women with a priori risk for preeclampsia were monitored at 4 time points during gestation and, based on the outcome of pregnancy, they were classified into the high‑risk group (70 women) and the preeclampsia group (20 women). Cholesterol metabolic profiling was done using liquid chromatography‑tandem mass spectrometry. Result s The only significant change in the preeclampsia group was an increase in the lathosterol level (P = 0.001). The first‑trimester lathosterol level was higher in the preeclampsia group compared with the high‑risk group (P = 0.02). Further, in the preeclampsia group, positive correlations were found between desmosterol and β‑sitosterol (ρ = 0.474; P = 0.03) in the third trimester, desmosterol and campesterol changes between the second and the first (ρ = 0.546; P = 0.02), and the third and first trimesters (ρ = 0.754; P <0.001), as well as between the desmosterol and β‑sitosterol differences between the third and first trimesters (ρ = 0.568; P = 0.01). No similar correlations were found in the high‑risk group. Conclusions Late‑onset preeclampsia could be associated with an altered lipid profile. By studying the quantitative metabolic signatures of cholesterol, we might assume that both cholesterol synthesis and absorption are increased, that is, there is an imbalance in the cholesterol homeostasis regulation in women affected by the disease.

AB  - Simultaneous quantification of multiple psychiatric drugs is important for the therapeutic
drug monitoring of psychiatric patients. In addition, it would be highly advantageous if the
method could be simple, straightforward, and not time-consuming. A 200 µl plasma sample was
deproteinized, drugs were separated by a ZORBAX Eclipse XDB-Phenyl column with the
mobile phase composed of acetonitrile and 0.1 % formic acid in water (60:40, v/v), and recorded
in the MRM mode by using a positive electrospray source with tandem mass spectrometry
detection. The dynamic range was 2–256 ng/ml for all the analyzed drugs, except escitalopram
(8-256 ng/ml). Quality control samples were prepared in quintuplicates in three relevant
concentrations for each drug. Coefficients of determination (R2
) were higher than 0.99, while the
relative difference between nominal and measured concentrations (RE) and CV were lower than
15% for all targets. High performance liquid chromatography coupled with the mass detector
(HPLC-MS/MS) method for simultaneous determination of sertraline, escitalopram, risperidone

and paliperidone in human plasma was validated with respect to selectivity, linearity, accuracy,
precision, matrix effect and stability. This method has significant advantages in terms of low
sample volume (200 µl), short preparation time (3 hours) and short runtime per sample (4
minutes).

In line with this, previous studieshave also suggested that the serum high-density lipoprotein cholesterol (HDL-C) level decreases inpatients with CRC, but more recently, the focus of investigations has shifted toward the explorationof qualitative properties of HDL in this malignancy. Herein, a comprehensive overview of availableevidences regarding the putative role of HDL in CRC will be presented. We will analyze existingfindings regarding alterations of HDL-C levels but also HDL particle structure and distribution inCRC. In addition, changes in HDL functionality in this malignancy will be discussed.  Moreover,we will focus on the genetic regulation of HDL metabolism, as well as the involvement of HDL indisturbances of cholesterol trafficking in CRC. Finally, possible therapeutic implications related toHDL will be presented. Given the available evidence, future studies are needed to resolve all raisedissues concerning the suggested protective role of HDL in CRC, its presumed function as a biomarker,and eventual therapeutic approaches based on HDL.

Matrix effect was examined by comparing different surrogates  by  simple  regression  analysis.  Validation  was conducted according to the FDA-ICH guideline. 20 healthy volunteers were recruited for testing of method application.Results:The  observed  matrix  effect  was  30%,  and  matrix comparison  showed  that  cholesterol  was  the  dominant contributor  to  the  matrix  effect.  Cholesterol  concentration was  adjusted  by  construction  of  the  calibration  curve  for serum  and  HDL  fraction  (5  mmol/L  and  2.5  mmol/L,respectively). The intra- and inter- run variabilities for NCSs were 4.7–10.3% for serum NCS and 3.6–13.6% for HDL-NCS  and  4.6–9.5%  for  serum  NCSs  and  2.5–9.8%  for HDL-NCS,  respectively.  Recovery  studies  showed  satisfactory  results  for  NCSs:  89.8–113.1%  for  serum  NCS  and85.3–95.8% for HDL-NCS. Conclusions:The  method  was  successfully  developed  and optimized. The matrix interference was solved by customising  calibration  curves  for  each  method  and  sample  type.The  measurement  of  NCS  in  HDL  fraction  was  proposed for the first time as potentially useful procedure in biomedical researches.
AB  - Uvod: Neholesterolski steroli (NHS) predstavljaju potencijalne biomarkere homeostaze holesterola. Pored toga, određivanje NHS u HDL frakciji (HDL-NHS) moglo bi da pruži dodatne informacije o efluksu holesterola. Međutim, određivanje NHS metodom tečne hromatografije - masene spektrometrije (LC-MS) je podložno uticaju matriksa usled čega dolazi do sniženja osetljivost metode. Studija je prvenstveno imala za cilj razvoj, optimizaciju i validaciju LC-MS metode za kvantifikaciju NHS u serumu i HDL-NHS. Pored toga, detaljno je ispitan efekat matriksa i metoda primenjena za analizu uzoraka seruma zdravih dobrovoljaca. Metode: Izolacija HDL frakcije je izvršena upotrebom precipitirajućeg reagensa. Efekat matriksa je ispitan poređenjem različitih surogat matriksa regresionom analizom. Validacija je sprovedena u skladu sa FDA-ICH vodičem za validaciju bioanalitičkih metoda. Interferencija usled prisustva holesterola je korigovana upotrebom odgovarajućih kalibracionih krivih za svaku vrstu uzorka. Određivanje HDL-NHS je po prvi put predloženo kao potencijalno korisna procedura u biomedicinskim istraživanjima.

AB  - Background:Vitamin D deficiency is repeatedly reported in colorectal  cancer  (CRC).  Since  cholesterol  and  vitamin  D share common precursor 7-dehydrocholesterol (7-DHC), it would be important to explore the associations of key vitamin D metabolites and serum lipid parameters in patients with high and low grade CRC. The aim of this study was toanalyze    relationships    between    serum    25(OH)D3,24,25(OH)2D3  and  7-DHC  levels  and  serum  lipids  inpatients with CRC, and to evaluate their potential for prediction of risk for development of high grade CRC. Methods:We  recruited  82  patients  CRC  and  77  controls.7-DHC, 25(OH)D3 and 24,25(OH)2D3 were quantified by LC-MS/MS methods.Results:7-DHC, 25(OH)D3 and vitamin D metabolic ratio(VDMR)  were  significantly  lower  in  CRC  patients  than  in control   group   (P<0.001,   P<0.010,   P<0.050   and P<0.050,  respectively).  25(OH)D3  levels  were  higher  inpatients  with  grade  I  CRC  when  compared  to  grade  II(P<0.050). All vitamin D metabolites positively correlated with  total  cholesterol  (TC)  concentration  in  CRC  patients.25(OH)D3 was significant predictor of increased CRC risk(P<0.010).   After   adjustment   for   TC   concentration,25(OH)D3 lost its predictive abilities. However, 25(OH)D3 remained significant predictor of poorly differentiated type of cancer (P<0.050). Conclusions:We  found  significant  positive  association between  vitamin  D  status  and  serum  total  cholesterol.Although low 25(OH)D3 was found to be a significant risk factor for CRC development, the obtained results primarily suggest profound impact of cholesterol level on vitamin D status  in  CRC.  However,  our  results  suggest  that  low 25(OH)D3 might independently contribute to development of poorly differentiated tumor.

Kako holesterol i vitamin D dele zajednički prekursor 7-dehidroholesterol (7-DHC), bilo bi značajno ispitati povezanost ključnih metabolita vitamin D i serumskih lipidnih parametara kod pacijenata sa CRC klasifikovanih na osnovu gradusa. AB  - Kolorektalni karcinom (CRC) je maligno oboljenje sa visokom prevalencijom urazvijenim zemljama. Holesterol poseduje mnogobrojne funkcije, te se pretpostavlja daima ulogu i u procesima maligne transformacije i proliferacije. Procena efikasnosti procesasinteze i apsorpcije holesterola, određivanjem cirkulišućih nivoa neholesterolskih sterola(NHS), može pružiti značajne podatke o metabolizmu holesterola, kao i njegovom uticajuna status drugih biomolekula (vitamin D, steroidni hormoni). Ciljevi ovog istraživanja bilisu uvođenje i validacija metoda tečne hromatografije sa tandem masenom detekcijom(HPLC-MS/MS) za kvantifikaciju markera sinteze (dezmosterol i latosterol) i apsorpcijeholesterola (kampesterol, β-sitosterol) u serumu i izolovanoj serumskoj frakcijilipoproteina visoke gustine (HDL), kao i razvoj i validacija metode za kvantifikacijuprekursora (7-dehidroholesterol, 7-DHC) i metabolita vitamina D (25(OH)D3,24,25(OH)2D3) u serumu. Osim toga, ispitivali smo i status markera sinteze i apsorpcijeholesterola, kao i metabolizma vitamina D kod osoba sa CRC, i nastojali da utvrdimopovezanost između metabolizma ovih jedinjenja, kao i da procenimo njihov prediktivnipotencijal za CRC.U istraživanju je učestvovao 101 pacijent sa dijagnozom CRC i 114 zdravihispitanika. Pacijenti su podeljeni prema patološkim karakteristikama na sledeće grupe:karcinom kolona (CA kolona) i karcinom rektuma (CA rektuma); gradus I i objedinjenigradusi II i III; stadijumi A i B, te stadijumi C i D. Koncentracije NHS u serumu i HDLfrakciji, kao i koncentracije parametara statusa vitamina D u serumu, određivani su HPLCMS/MS metodama. Na osnovu koncentracija NHS u serumu i HDL frakciji, izračunati surazličiti indeksi homeostaze holesterola, dok je odnos metabolita vitamina D (VDMR)izračunat na osnovu koncentracija 25(OH)D3 i 24,25(OH)2D3 u serumu.Rezultati studije su pokazali da je holesterol glavni interferent prilikom određivanjaNHS, pa je u tom smislu izvršena priprema odgovarajućeg matriksa za kvantitativnuanalizu. AB  - Colorectal cancer (CRC) is a malignancy highly prevalent in developed countries.Cholesterol is a biomolecule with multiple functions and suspected important role inmalignant transformation and proliferation. Assessing the efficiency of cholesterolsynthesis and absorption processes, by determining circulating levels of non-cholesterolsterols (NCS), can provide significant data on cholesterol metabolism as well as its effecton other biomolecules (vitamin D, steroid hormones). The objectives of this study were tointroduce and validate liquid chromatography - tandem mass detection (HPLC-MS/MS)methods for quantification of cholesterol synthesis (desmosterol and lathosterol) andcholesterol absorption (campesterol, β-sitosterol) markers in serum and serum high densitylipoprotein fraction (HDL), as well as the method for quantification of vitamin D statusparameters (7-dehydrocholesterol (7-DHC), 25(OH)D3, 24,25(OH)2D3). In addition, weexamined the status of cholesterol synthesis and absorption markers, as well as vitamin Dmetabolites in individuals with CRC, and explored the association between these twometabolic pathways, as well as evaluated their predictive potential for CRC.The study involved 101 patients with CRC diagnosis and 114 healthy subjects.Patients were divided according to pathological characteristics into the following groups:colon cancer (colon CA) and rectal cancer (rectal CA); grade I, and grade II and IIIcombined; stages A and B and stages C and D. Vitamin D metabolite ratio (VDMR) wascalculated based on serum 25(OH)D3 and 24,25(OH)2D3 concentrations.The study results showed that cholesterol was the main interferent in thedetermination of NCS, and in this respect, the appropriate matrix for quantitative analysiswas prepared. Within-series variations were 4.7-10.3% for serum NHS and 3.6-13.6% forNCS in HDL fraction, whereas between series variations were 4.6-9.5% for serum NHSand 2.5-9.8% for NCS in HDL fraction. AB  - Background: Previous studies revealed decreased level of high-density lipoprotein cholesterol (HDL-C) as important factor for development of colorectal cancer (CRC). Quantity and structure of HDL particles depend on activities of lipid transfer proteins lecithin:cholesterol acyltransferase (LCAT) and cholesteryl ester transfer protein (CETP), but this topic is largely unexplored in CRC. The main objective of this study was to investigate activities of LCAT and CETP in patients with CRC. Additionally, we analyzed activity of paraoxonase-1 (PON-1), as a main carrier of HDL-antioxidant function. Materials and methods: Ninety-nine CRC patients and 101 healthy individuals were included. LCAT and CETP activities were assessed by measuring rates of formation and transfer of cholesteryl esters. PON-1 paraoxonase and arylesterase activities were measured. Results: Lower levels of HDL-C (p  lt  .001) were observed in cohort of patients, alongside with decreased LCAT (p  lt  .050) and increased CETP activity (p  lt  .050). Both PON-1 activities were diminished in CRC (p  lt  .050 and p  lt  .001 respectively). Univariate logistic regression singled out HDL-C level (OR = 0.218, p  lt  .001), CETP activity (OR = 1.010, p  lt  .01) and mass (OR = 0.994, p  lt  .001) as possible markers of elevated CRC risk. CETP mass maintained its predictive significance when adjusted for traditional risk factors and level of oxidative stress (OR = 0.993, p  lt  .001; OR = 0.982, p  lt  .050, respectively). Conclusion: Our results demonstrated increased CETP and decreased LCAT and PON-1 activities in CRC patients. In preliminary analysis CETP mass was identified as potential significant predictor of CRC development, suggesting that alterations in HDL-C levels, alongside with changes in HDL structure might have a role in carcinogenesis.

Preanalytical and analytical challenges in gas chromatographic determination of cholesterol synthesis and absorption markers

AB  - Introduction: Cholesterol homeostasis disruption contributes to the development of different pathologies. Non cholesterol sterols (NCSs) serve as cholesterol synthesis markers (desmosterol and lathosterol), and cholesterol absorption surrogate markers (campesterol, stigmasterol and beta-sitosterol). The study aimed to resolve certain new pre-analytical and analytical problems and ensure a reliable and validated method. Materials and methods: Method optimization, validation and stability studies were executed in human serum and plasma. Freeze-thaw cycles were done with and without antioxidant. Gas chromatography-mass spectrometer (GC-MS) was used for NCSs confirmation and plasticizer identification, while GC-flame ionization detector (GC-FID) was used for NCSs quantitation. Results: Infra-and inter-assay variabilities for all NCSs were 2.75-9.55% and 5.80-7.75% for plasma and 3.10-5.72% and 3.05-10.92% for serum, respectively. Recovery studies showed satisfactory percentage errors for all NCSs: 93.4-105.7% in plasma and 87.5-106.9 in serum. Derivatized samples were stable up to 7 days at 20 degrees C and derivatization yield was affected by presence of plasticizers. Fatty acid amids were identified as interfering plastic leachates. Statistically different NCSs concentrations were observed after the 1st freeze-thaw cycle, in antioxidant-free samples, and after the 4th cycle in antioxidant-enriched samples. Conclusions: All of the in-house procedures proved to be useful for minimizing the preanalytical and analytical variations, as proven by the validation results.

Non cholesterol sterols (NCSs) serve as cholesterol synthesis markers (desmosterol and lathosterol), and cholesterol absorption surrogate markers (campesterol, stigmasterol and beta-sitosterol). The study aimed to resolve certain new pre-analytical and analytical problems and ensure a reliable and validated method. Materials and methods: Method optimization, validation and stability studies were executed in human serum and plasma. Freeze-thaw cycles were done with and without antioxidant. Gas chromatography-mass spectrometer (GC-MS) was used for NCSs confirmation and plasticizer identification, while GC-flame ionization detector (GC-FID) was used for NCSs quantitation. Results: Infra-and inter-assay variabilities for all NCSs were 2.75-9.55% and 5.80-7.75% for plasma and 3.10-5.72% and 3.05-10.92% for serum, respectively. Recovery studies showed satisfactory percentage errors for all NCSs: 93.4-105.7% in plasma and 87.5-106.9 in serum. Derivatized samples were stable up to 7 days at 20 degrees C and derivatization yield was affected by presence of plasticizers. Fatty acid amids were identified as interfering plastic leachates. Statistically different NCSs concentrations were observed after the 1st freeze-thaw cycle, in antioxidant-free samples, and after the 4th cycle in antioxidant-enriched samples. Conclusions: All of the in-house procedures proved to be useful for minimizing the preanalytical and analytical variations, as proven by the validation results.",
