AB  - The aim of the present study was to develop a population pharmacokinetic model of carbamazepine from routine therapeutic drug monitoring data. Steady-state carbamazepine plasma concentrations determined by homogenous enzyme immunoassay technique, dosing history including cotherapy, schedule of blood sampling, and patients' demographic characteristics were collected retrospectively from patients' chart histories. A one-compartment model was fitted to the data using nonlinear mixed effects modeling. The influence of weight, age, gender, smoking, allergy, carbamazepine daily dose, and cotherapy on clearance (CL/F) was evaluated. Additionally, bioavailability of controlled-release relative to immediate-release tablets was assessed. Two hundred sixty-five patients (423 concentrations) were used to develop a population pharmacokinetic model. The population estimate of CL/F from the base model was 5.14 L/h with interindividual variability of 50.20%. Patients' gender, age, smoking, allergy, cotherapy with lamotrigine and benzodiazepines had no effect on CUE Patient weight (WT), daily carbamazepine dose (DCBZ), daily dose of phenobarbitone (DPB) and valproic acid (VPA), when its daily dose exceeded 750 mg, significantly influenced CL/F and were included in the final model: CL/F [L/h] = 5.35 (DCBZ [mg/da/kg]/15)(0.591) (1 + 0.414 (DPB [mg/day/kg]/2) (WT [kg]/70)(0.564) 1.18(VPA) where VPA is 1 if dose is greater than 750 mg or 0 otherwise. No difference in bioavailability of carbamazepine between controlled and immediate-release tablets was detected. The model predictions in the validation set had no bias and satisfactory precision. The model can be used for estimation of carbamazepine CL/F in individual patients in the postautoinduction phase and for selection of optimum dosing regimen in routine patient care.

Websites as tools of promoting health in South East Europe

tion of the healthcare market and drug producers, as well.
This new mass media is valuable not only as a source of
information for patients and healthcare providers but
also as a platform for selling products and services (e-commerce). OBJECTIVES: The goal of this study is create
a database of health websites originated from South East
Europe (SEE), and determine the quality of information’s
regarding the customer needs using the World Health
Organization’s (WHO) standard. METHODS: The data-
base, named pluspharma™, was constructed by an
Internet search and an e-mail questionnaire. The quality
of information was measured according to recommenda-
tion of WHO (a rating of 1 to 5). RESULTS: The data-
base contains about 300 web addresses of useful sources
of health and health business information. Of the six
countries (Bosnia and Herzegovina, Bulgaria, Croatia,
Yugoslavia, Macedonia, Romania), Romania showed the
most (37 public health institutions and 26 public tools
promoting health) Web health information sites. The
most websites originated from the primary health care
institutions, government body or pharmaceutical indus-
try. Also, there is the numerous high quality health
promotional website designed with specific task groups
(example tobacco groups). Direct public communication
is evident through about 60% of web sites. Websites of
pharmaceutical industry and pharmacies marking with
relatively high credit (3.89), compared with other health
and service information’s (public and government health
institution). The special task groups (usually evidenced as
NGO) obtained high credit (4.74) of quality and valuable
of Web information’s. CONCLUSIONS: Health-related
websites from South East Europe are becoming good
tools for health promotion in the SEE region.

The clinical pharmacological correlation between amitriptyline dosage and side effects

AB  - The application of tryciclic antidepressants amitriptyline in the therapy of depression is followed by some side effects both those muscarinic and antihistaminic and alpha-adrenergic effects. The manifested degree of side effects directly correlates with the dosage and concentration in plasma, so that maximal effects are manifested when drug concentration in patient's plasma is maximal. When amitriptyline is used in the therapy an effective drug dosage enabling the maximal therapeutic effect and minimal side effects should be determined. abstract = "The application of tryciclic antidepressants amitriptyline in the therapy of depression is followed by some side effects both those muscarinic and antihistaminic and alpha-adrenergic effects. The manifested degree of side effects directly correlates with the dosage and concentration in plasma, so that maximal effects are manifested when drug concentration in patient's plasma is maximal. When amitriptyline is used in the therapy an effective drug dosage enabling the maximal therapeutic effect and minimal side effects should be determined. By this the maximal outcome of the therapy is made possible.",
