AB - Occupational and environmental toxicology specialists find catecholamine fluctuations in brain tissue relevant for research of neurotoxicity, such as that induced by manganese or zinc, pesticides, industrial solvents, plastic, air pollution, or irradiation. Considering that catecholamine tissue concentrations are generally very low, their extraction requires a reliable and optimal method that will achieve maximum recovery and minimise other interferences. This study aimed to evaluate whether the aluminium (III) oxide (Al2O3, alumina) based cartridges designed for catecholamine isolation from plasma could be used for solid-phase extraction (SPE) of catecholamine from the brain tissue. To do that, we homogenised Wistar rat brain tissue with perchloric acid and compared three extraction techniques: SPE, the routine filtration through a 0.22 µm membrane filter, and their combination. In the extracts, we compared relative chromatographic catecholamine mobility measured with high performance liquid chromatography with electrochemical detection. Chromatographic patterns for norepinephrine and epinephrine were similar regardless of the extraction technique, which indicates that the alumina cartridge is good enough to isolate them from brain tissue. However, the dopamine pattern was unsatisfactory, and further experiments are needed to identify the issue and optimise the protocol. © 2023 Duško Mirković et al., published by Sciendo. Niske koncentracije KAT-a u tkivu zahtijevaju pouzdanu i učinkovitu tehniku ekstrakcije kojom se postiže maksimalni „prinos“ katekolamina i minimalni sadržaj interferirajućih spojeva. AB - A randomized, double-blind, placebo-controlled study was conducted to investigate the influence of supplementation with a superoxide dismutase (SOD)-rich plant extract on markers of oxidative stress, zonulin levels and the performance of elite athletes. Participants were 30 international-level rowers, divided into an experimental group (n = 15) and a control group (n = 15). The rowers performed a maximal effort incremental test on a rowing ergometer at the beginning and at the end of the study. Markers of oxidative stress (total antioxidant status (TAS), total oxidant status (TOS), oxidative stress index (OSI), superoxide dismutase (SOD), glutathione peroxidase (GPx), advanced oxidation protein products (AOPPs), malondialdehyde (MDA), sulfhydryl (SH) groups, bilirubin, uric acid, albumin and zonulin) were determined in serum. A lower TOS (p = 0.010) and OSI (p = 0.004), a lower MDA (p = 0.001) and a higher level of SH groups (p = 0.031) were observed in the experimental group after supplementation. Physical performance was evaluated through metabolic efficiency, taking lactate levels and power output on the ergometer into account. After 6 weeks of supplementation, the relative increase in metabolic efficiency at a 4 mmol/L lactate concentration and maximal effort was significantly higher in the experimental group (p = 0.004 and p = 0.015, respectively). These results suggest that supplementation with a SOD-rich extract promotes lower oxidative stress, better antioxidant protection and, consequently, the better work performance of athletes.

AB - Background: Multiple sclerosis (MS) is characterized by
inflammation, demyelination and axonal degeneration.
Oxidative stress (OS) plays a significant role in the pathogenesis of the disease. The aim of the study was to examine the association between OS and smoking on the MS
development.
Methods: The study included 175 patients with relapsing-
remitting multiple sclerosis (RRMS) (76 males, 99 females)
and 254 healthy subjects (81 males and 173 females).
Oxidative stress biomarkers in serum, Total Antioxidant
Status (TAS) and Total Oxidative Status (TOS) were determined spectrophotometrically. Oxidative Stress Index (OSI)
was calculated as the ratio of TOS and TAS. Urinary 8-oxo-
7,8-dihydro-2’-deoxyguanosine were determined by
HPLC-MS/MS and expressed as 8-oxodG/creatinine.
Results: In females with RRMS were higher TOS, OSI and
8-oxodG/creatinine than in females in control group. The
group of males with RRMS had lower level of TAS than the
males in control group. Higher levels of 8-oxodG/creatinine was obtained in active, passive and former smokers
with RRMS than in control group with the same exposition
to tobacco smoke. Independent predictors of MS are passive smoking, increased OSI and increased levels of urinary
8-oxodG/creatinine.
Conclusions: Our results demonstrate that the OS parameters should be included in the assessment of the risk for MS
development. Due to the more sensitivity to oxidative
stress, females may be at higher risk of MS development.
This data indicates the importance of introducing the
antioxidant therapy as a complementary treatment in
patients with RRMS.
AB - Uvod: Multipla skleroza (MS) se karakteriše upalom, demijelinizacijom i degeneracijom aksona. Oksidativni stres (OS) igra značajnu ulogu u patogenezi bolesti.

AB - Background: Vitamin D is a steroid hormone, known to be involved in the pathogenesis of various neurodegenerative disorders, including Parkinson's disease (PD). We aimed to clarify the relationship between hypovitaminosis D and the predisposition for PD and its clinical presentation. An additional aim was to examine the specific gene polymorphisms associated with vitamin D level. Material and methods: Total level of 25(OH)-vitamin D (25(OH)D) was measured in the serum of parkinsonian patients (n = 113) and controls (n = 82) using a commercial immunoassay. Genetic analyses were performed using Taqman assays on Real Time PCR amplification system. Results: Higher frequency of vitamin D deficiency (<50 nmol/L) was observed in PD patients, compared to controls (40.7% and 23.2%, respectively, P = 0.010). It was also a positive predictive marker of PD (OR, 2.27; 95% CI, 1.206–4.298; P < 0.011). Significantly higher UPDRS (35.85 ± 1.35 and 32.09 ± 0.99, respectively, P = 0.023) and HY scores (2(1.5–2.5) and 1.5(1.0–2.0), respectively, P = 0.005) were present in patients with 25(OH)D level < 50 nmol/L compared to patients with 25(OH)D level ≥ 50 nmol/L. Despite some trends observed, differences in allelic and genotypic distribution between controls and patients, as well as between subgroups, did not reach the level of significance (P > 0.05). Conclusions: Findings of this study confirm the hypothesis of a significant relationship between hypovitaminosis D and PD. We demonstrated higher prevalence of vitamin D deficiency in PD patients, as well as its predictive potential for the onset and progression of PD.

AB - According to ISO 15189:2022, analytical performance specifications (APS) should relate to intended clinical use and impact on patient care. Therefore, we aimed to develop a web application for laboratory professionals to calculate APS based on a simulation of the impact of measurement uncertainty (MU) on the outcome using the chosen decision limits, agreement thresholds, and data of the population of interest. We developed the "APS Calculator"allowing users to upload and select data of concern, specify decision limits and agreement thresholds, and conduct simulations to determine APS for MU. The simulation involved categorizing original measurand concentrations, generating measured (simulated) results by introducing different degrees of MU, and recategorizing measured concentrations based on clinical decision limits and acceptable clinical misclassification rates. The agreements between original and simulated result categories were assessed, and values that met or exceeded user-specified agreement thresholds that set goals for the between-category agreement were considered acceptable. The application generates contour plots of agreement rates and corresponding MU values. We tested the application using National Health and Nutrition Examination Survey data, with decision limits from relevant guidelines. We determined APS for MU of six measurands (blood total hemoglobin, plasma fasting glucose, serum total and high-density lipoprotein cholesterol, triglycerides, and total folate) to demonstrate the potential of the application to generate APS. The developed data-driven web application offers a flexible tool for laboratory professionals to calculate APS for MU using their chosen decision limits and agreement thresholds, and the data of the population of interest.

AB - Several studies report the important role of an altered gut microbiota in the development of obesity, highlighting the potential use of probiotics in the treatment of obesity. The aim of this study is to investigate the effect of a novel probiotic approach on the expression of specific miRNAs and mRNAs associated with obesity in combination with the hypocholesterolemic octacosanol. Twenty overweight/obese women participated in a randomized, placebo-controlled, double-blind study and were randomly divided into two groups: the intervention group (daily one capsule containing Lactobacillus plantarum 299v (DSM9843), Saccharomyces cerevisiae var. boulardii, and 40 mg octacosanol; N = 12) and the placebo group (N = 8). Changes in lipid parameters and expression of miRNAs and mRNAs were assessed before (T0) and after the 12-week intervention (T1). After the intervention, the expression of miR-155-5p (9.38 ± 0.85 vs. 8.38 ± 1.06, p = 0.05) and miR-24-3p (3.42 ± 0.38 vs. 2.71 ± 0.97, p = 0.031) showed significant decreases in the intervention group when compared to the control group. At T1, the expression of miR-155-5p (8.69 ± 1.31 vs. 9.3 ± 0.85, p = 0.04), miR-125b-5p (5.41 ± 1.18 vs. 5.99 ± 1.36, p = 0.049), and TNF-α (10.24 ± 1.66 vs. 11.36 ± 1.12, p = 0.009) were significantly decreased in the intervention group. No changes in lipids and anthropometric parameters were observed. The novel probiotic approach had a positive effect on regulating the expression of certain miRNAs and mRNAs important for regulating inflammation and adipogenesis, which are essential for obesity onset and control.

AB - Background: The unpredictable course of Coronavirus
Disease 19 (COVID-19) is making good severity assessment tools crucial. This study aimed to assess the usefulness of serum amyloid A (SAA) and other acute-phase
reactants (APRs) in ambulatory care COVID-19 patients
and identified relationships between these markers and disease outcomes.
Methods: From August to November 2020, patients seen
in the outpatient department of the Clinic for Infectious and
Tropical Diseases (Belgrade, Serbia) with confirmed
COVID-19 were included. Patients were classified into
mild, moderate, and severe disease groups based on World
Health Organization criteria. SAA, C-reactive protein
(CRP), interleukin-6 (IL-6), procalcitonin (PCT), ferritin, fibrinogen, D-dimer, albumin, and transferrin were measured.
The median values of all APRs were compared between
COVID-19 severity groups, hospitalized and non-hospitalized patients, and survivors and non-survivors. The
Receiver operator characteristic (ROC) curve analysis was
used for the classification characteristics assessment of
individual APRs for the severity of illness, hospitalization,
and survival.
Results: Higher levels of SAA, CRP, IL-6, PCT, and lower
levels of transferrin and albumin were observed in severe
cases, hospitalized patients, and non-survivors. Based on
ROC curve analysis AUC for SAA has fair classification performance for disease severity (0.794) and death (0.732)
and good performance for hospitalization (0.853).
Conclusion: SAA is a valuable marker in everyday practice
for assessing COVID-19 severity and prognosis in ambulatory patients.

antitela (BioRad) (II), SARS-CoV-2 IgG II Quant (Abbott) (III), SARS-CoV-2 IgG (Abbott) (IV) i
Elecsys Anti-SARS-CoV-2 (Roche) (V). Paralelno je ispitivano metodom I i II 44 uzorka, I i III 84
uzorka, I i IV 53 uzorka, I i V 80 uzoraka.
Rezultati. II i III metodom je dobijen značajno veći broj reaktivnih rezultata u odnosu na I
metod (65,9% i 91,7% vs 52,3%). I i IV metode su pokazale slaganje u reaktivnosti (60,4% vs
69,8%), dok I i V metodom nije dobijeno slaganje u reaktivnosti rezultata (58,8% vs 93,8%).
imunizacije (P=0,032 i P=0,012).
godina starosti ispitanika (P=0,197).
Zaključak
Iako serološka imunoodređivanja pokazuju značajne analitičke i kliničke karakteristike za
detekciju SARS-CoV-2 i dalje postoji problem kompletne evaluacije testova i neizvesnosti u
pogledu tačnosti rezultata i njihove uporedivosti.

AB - Uvod i cilj. Pandemija bolesti izazvane teškim akutnim respiratornim sindromom korona-
virus 2 (SARS-CoV-2) dovela je do ekspanzije seroloških testova za određivanje antitela i an-
tigena virusa.
de dijagnostičke karakteristike i klinički značaj Access SARS-CoV-2 IgG testa.
CoV-2 infekcijom (PCR metodom) i 104 kontrolna ispitanika. IgG antitela su određena u se-
rumu ispitanika primenom Access Sars-CoV-2 IgG testa, proizvođača Beckman Coulter, na
imunohemijskom Access 2 imunoanalizatoru iste firme.
Rezultati.

AB - Previous research suggests that point-of-care (POCT) determination of glycated hemoglobin (HbA1c) is a diagnostic test that can be an adequate alternative to measuring HbA1c in the laboratory. The main goal of this study was to examine the analytical characteristics of the novel INCLIX POCT method for HbA1c determination in order to test its performance before introducing this method into routine use. HbA1c is measured in a duplicate in 44 EDTA blood samples parallel on INCLIX POCT device (Sugitech, Inc.) and using automated turbidimetric immunoinhibition test on Olympus AU400 (Beckman Coulter). The within run imprecision was 7.58%, between runs imprecision was 6.63% and 6.22%, and day-to-day imprecision was 8.80% and 7.51%. Total laboratory imprecision was in agreement with those stated by the manufacturer. A statistically significant Pearson correlation coefficient was calculated (r = 0.871, P < 0.01; linear R2 = 0.757). Using Deming regression analysis, the following equation was obtained: y = - 1.80 + 1.304x. Our results indicate statistically significant correlation, linear relationship, and a significant degree of compatibility between the two analyzed methods. However, the negative bias of the HbA1c values determined on the POCT analyzer compared to the Olympus AU400 was confirmed, highlighting the need to standardize the INCLIX method.

AB - Background: The aim of this study was to determine the reference intervals (RIs) for thyroid stimulating hormone (TSH), free thyroxine (FT4), free triiodothyronine (FT3) and FT3/FT4 ratio using indirect methods. Methods: We analyzed 1256 results TSH, FT4 and FT3 collected from a laboratory information system between 2017 and 2021. All measurements were performed on a Siemens ADVIA Centaur XP analyzer using the chemiluminescent immunoassay. We calculated the values of the 2.5th and 97.5th percentiles as recommended by the IFCC (CLSI C28-A3). Results: The RIs derived for TSH, FT4, FT3 and FT3/FT4 ratio were 0.34–4.10 mIU/L, 11.3–20.6 pmol/L, 3.5–6.32 pmol/L and 0.21–0.47, respectively. We found a significant difference between calculated RIs for the TSH and FT4 and those recommended by the manufacturer. Also, FT3 values were significantly higher in the group younger than 30 years relative to the fourth decade (5.26 vs. 5.02, p=0.005), the fifth decade (5.26 vs. 4.94, p=0.001), the sixth decade (5.26 vs. 4.87, p<0.001), the seventh decade (5.26 vs. 4.79, p<0.001) and the group older than 70 years old (5.26 vs. 4.55, p<0.001). Likewise, we found for TSH values and FT3/FT4 ratio a significant difference (p <0.001) between different age groups. Conclusions: The establishing RIs for the population of the Republic of Srpska were significantly differed from the recommended RIs by the manufacturer for TSH and FT4. Our results encourage other laboratories to develop their own RIs for thyroid parameters by applying CLSI recommendations.

abstract = "Background: The aim of this study was to determine the reference intervals (RIs) for thyroid stimulating hormone (TSH), free thyroxine (FT4), free triiodothyronine (FT3) and FT3/FT4 ratio using indirect methods. Our results encourage other laboratories to develop their own RIs for thyroid parameters by applying CLSI recommendations., Uvod: Cilj ove studije bio je da se odrede referentni intervali (RI) tireotropnog hormona (TSH), slobodnog tiroksina (FT4), slobodnog trijodotironina (FT3) i odnosa FT3/FT4 indirektnom metodom procene referentnih intervala. AB - This study aimed to investigate the effect of supplementation with plant origin superoxide dismutase (SOD), GliSODin, on parameters of muscle damage, metabolic, and work performance at international level rowers. Twenty-eight rowers were included in a randomized, double-blind study. The study was conducted during a 6-week preparation period. At the beginning of the study and after 6 weeks of the supplementation period, all rowers were tested on a rowing ergometer. Blood samples were taken from the antecubital vein before and after every ergometer testing. Muscle damage markers creatine kinase (CK) and lactate dehydrogenase (LDH), total antioxidant capacity (TAC), inflammation parameters interleukin-6 (IL-6), and C-reactive protein (CRP) were measured. Rowing performance was assessed by lactate level in capillary blood and power output on the rowing ergometer. After supplementation, experimental group had significantly lower CK (p = 0.049) and IL-6 (p = 0.035) before and IL-6 (p = 0.050) after exhausting exercise on ergometer. Relative change of power output at 4 mmol/L concentration of lactate in blood, considering the initial and final test, was significantly higher (p = 0.020) in the supplemented group. It was concluded that GliSODin could be considered a good supplement in preventing some deleterious effects of intensive physical activity, including inflammation and muscle damage, and consequently, to enable a better rowing performance of elite rowers.

Lab- oratories fulfil their responsibility for accurate and consistent results by utilizing an arsenal of approaches, ranging from validation and verification experiments to daily quality control procedures. All these procedures verify, on different moments, that the results of a certain examination procedure have analytical performance characteristics (APC) that meet analytical performance specifications (APS) set for a particular intended use. The APC can in part be determined by esti- mating the measurement uncertainty component under con- ditions of within-laboratory precision (uRw), which comprises all components influencing the measurement uncertainty of random sources. To maintain the adequacy of their mea- surement procedures, laboratories need to distinguish aspects that are manageable vs. those that are not. One of the aspects that may influence uRw is the momentary significant bias caused by shifts in reagent and/or calibrator lots, which, when accepted or unnoticed, become a factor of the APC. In this paper, we postulate a model for allocating a part of allowable uRw to between-reagent lot variation, based on the need for long-term consistency of the measurement variability for that specific measurand. The allocation manages the ratio between short-term and long-term variation and indicates laboratories when to reject or correct certain variations due to reagent lots.

AB - This study aimed to investigate the short-term effects of three magnesium (Mg) dietary supplements containing mineral immediately available for absorption on Mg biochemical status indices (ionized and total Mg), as well as their effects on electrolytes levels in healthy female young adults (n = 61). After a 10-days intervention period supplementation with powder/granulate containing Mg oxide led to an increase in both ionized Mg concentration and % in total Mg in comparison with the baseline. Supplementation with Mg citrate was associated with the significant increase in % of ionized fraction and decrease in serum total Mg concentration. By contrast, among participants consuming Mg carbonate in the form of effervescent tablets ionized Mg concentration and % in total Mg decreased, without detectable changes in serum total Mg. In conclusion, after the short-term supplementation period, Mg oxide demonstrated superior bioavailability compared to the other examined Mg supplements without affecting other minerals’ levels.

AB - Background: Literature data indicate the benefit of magne-
sium (Mg) supplementation. The aim of this study was to
examine the effect of short-term Mg supplementation on
iron status in healthy female participants.
Methods: One hundred healthy female students of the
University of Belgrade - Faculty of Pharmacy participated the
study during eleven intervention days. Students ingested Mg
preparations with the same dose of the active substance. The
analysis included the measurement of serum iron, unsaturat-
ed iron binding capacity (UIBC), total iron binding capacity
(TIBC), total Mg (tMg), ionized Mg (iMg), complete blood
count, met-, carboxy- and oxy-haemoglobin (metHgb,
COHgb, O2Hgb). Transferrin concentrations and percentage
of transferrin saturation (SAT) were calculated manually. The
association among the analyzed biochemical parameters was
examined using polynomial regression. A principal compo-
nent analysis (PCA) was used for the evaluation of interde-
pendence between the analyzed parameters.
Results: A statistically significant trend for change in O2Hgb
(%) by tertiles of iMg concentrations was found (P = 0.029).
Serum tMg reached significant positive correlation with the
SAT at concentration levels greater than 0.9 mmol/L, after
11 days of intervention (R2=0.116). Ionized Mg in a con-
centration higher than 0.6 mmol/L is positively correlated
with SAT and serum Fe (R2=0.214; 0.199, respectively).
PCA revealed variability of 64.7% for two axes after 11 days.
Conclusions: Mg supplementation leads to an improvement
in the certain iron status parameters even in individuals with
optimal levels of these indices. However, caution should be
exercised when supplementing Mg, and laboratory monitor-
ing of the interaction is required.

abstract = "Background: Literature data indicate the benefit of magne-
sium (Mg) supplementation.
However, caution should be
exercised when supplementing Mg, and laboratory monitor-
ing of the interaction is required., Uvod: Literaturni podaci ukazuju na benefit suplementacije magnezijumom (Mg).

AB - Obesity could be associated with enhanced platelet (PL) function and thrombosis.
Also, changes in gut microbiota composition were observed in obese patients, and probiotics
supplementation has been shown to improve some of the obesity-associated metabolic
parameters. This study aimed to determine the possible effects of probiotic supplementation
on platelet morphology in overweight/obese premenopausal women.Twenty overweight
(BMI = 25.0-29.9 kg/m2) and obese (BMI ≥ 30.0 kg/m2) women participated in double-
blind, randomized, placebo-controlled study. They were randomly assigned to receive one
capsule daily of probiotics (7x1010 CFU Lactobacillus plantarum 299v (DSM9843), 5x109
CFU Saccharomyces cerevisiae var. boulardii and 40mg octacosanol; N=10) or placebo
(N=10) for 3 months. To determine changes in the parameters of platelet activation - the
platelet count (PC), platelet distribution width (PDW), Feret diameter, and hypogranular
appearance, blood samples were obtained for an automated hematology analyser and
peripheral smears staining. Although difference in PC was not found between groups,
supplementation with probiotics decreased PDW compared to the placebo group (p<0.05).
Regarding platelets diameter, there is an decrease in group supplemented with probiotics
compared to placebo, however, a statistical difference has not been reached. Further, the
proportion of hypogranular PL was increased in a probiotic-supplemented group compared
to the placebo group (p<0.01). In conclusion, probiotic supplementation achieved
improvements in platelets morphology and hematological indices in overweight/obese
premenopausal women.
karakteristike krvnih pločica u randomizovanoj, dvostruko-slepoj, placebo-kontrolisanoj
studiji.
tokom tri meseca (kombinaciju soja Lactobacillus plantarum 299v (DSM9843),
pločica i mogućnost primene probiotika u regulaciji njihove aktivnosti u patološkim
stanjima.

AB - Nuts are nutrient-dense foods with complex matrices rich in unsaturated fatty acids and other bioactive compounds. Numerous reported health benefits of nuts may be attributed to their unique nutrient profiles and synergistic interaction of their constituents, including macronutrients, micronutrients and phytochemicals. The objective of this study was to explore pharmacy students’ nuts consumption practice, as well as their attitudes and knowledge concerning nuts nutritional and health properties. A cross-sectional survey based on the online self-administered questionnaire, was conducted in September 2020. Among respondents (n=136, 90.4% female and 9.6% male, mean age 23.7±2.0 years), more than half agreed or strongly agreed that regular consumption of nuts might have beneficial effects on health, including reducing the risk of cardiovascular disease. Furthermore, the majority of participants perceived nuts as healthy sources of proteins and monounsaturated and polyunsaturated fats. The most common reason cited by respondents for eating nuts were taste (80.2%), followed by health effects (53.1%) and nutrient profiles (45.0%). The most commonly consumed nuts among pharmacy students were almonds (35.2%) and peanuts (28.4%), followed by hazelnuts (19.1%) and walnuts (15.6%). Based on self-reported data, students predominantly consume raw nuts (54.0%) as a snack between meals (78.5%). Nutrition-related courses within the faculty curriculum were the main sources of acquiring information regarding the beneficial effects nuts exert on health outcomes. Although certain gaps in knowledge were determined, in most senior students, the attitudes and dietary practices regarding the consumption of nuts were at an adequate level. Nevertheless, expanded efforts and additional educational programs may be useful in strengthening the capacities of these future health professionals for providing adequate nutrition advice and proactive advocacy of nuts health benefits.

AB - In medical laboratories, bias presents quantitative expression of trueness, as
analytical performance characteristic that describes systematic errors. Despite bias evaluation
is an integral part of basic lessons of laboratory quality management, it is still difficult to achieve
adequate bias estimation in routine laboratory practice. There are several principles that could
be used to estimate bias, but fundamental for evaluating bias in routine medical laboratories is
the availability of a suitable reference materials. Besides, it is important that the measuring
method and measuring analytical system are in a stable state, and that they have completed
an appropriate validation or verification process. This could be checked routinely by the results
of internal and/or external quality control. In addition, information about the uncertainty of the
certified reference material is a prerequisite for testing the bias. Significant efforts were made
to minimized bias in routine laboratory practice, using reference measurement methods and
commutable reference materials. Still, there are some challenges that remain for the laboratory
professionals as the end users, due to remaining bias between measurement methods and
systems from different and/or the same manufacturers.

AB - The ISO 15189:2012 standard section 5.9.1 requires laboratories to review results before release, considering quality control, previous results, and clinical information, if any, and to issue documented procedures about it. While laboratory result reporting is generally regarded as part of the post-analytical phase, the result release process requires a general view of the total examination process. Reviewing test results may follow with troubleshooting and test repetition, including reanalyzing an individual sample or resampling. A systematic understanding of the result release may help laboratory professionals carry out appropriate test repetition and ensure the plausibility of laboratory results. In this paper, we addressed the crucial steps in the result release process, including evaluation of sample quality, critical result notification, result reporting, and recommendations for the management of the result release, considering quality control alerts, instrument flags, warning messages, and interference indexes. Error detection tools and plausibility checks mentioned in the present paper can support the daily practice of results release.

AB - Objective: Chronic obstructive pulmonary disease (COPD) is multi-factorial disorder which
results from environmental influences and genetic factors. We aimed to investigate whether
methionine sulfoxide reductase A (MSRA) rs10903323 gene polymorphism is associated with
COPD development and severity in Serbian adult population.
Methods: The study included 155 patients with COPD and 134 healthy volunteers. Genotyping
was determined performing home-made polymerase chain reaction-restriction fragment length
polymorphism (PCR-RFLP). The difference between the inhibitory activities of normal and oxi-
dized Alpha-1-Antitrypsin (A1AT) against elastase and trypsin was used for determination of Oxi-
dized Alpha-1-Antitrypsin (OxyA1AT) (expressed as % and g/L). Functional activity of A1AT was
presented as a specific inhibitor activity to elastase (SIA-Elastase, kU/g).
Results: Frequencies of the genotypes AA, AG and GG were 80.0%, 20.0%, 0% in COPD patients
and 80.5%, 18.5% and 1.5% in the control group, and there was no significant difference in geno-
type or allele distributions between groups. Serum level of A1AT (g/L) and OxyA1AT was signifi-
cantly higher in COPD patients than in the control group, but functional activity of A1AT (SIA-
Elastase) was significantly lower in COPD patients than in the control group. In COPD group,
increased level of OxyA1ATwas present in G allele carriers who were smokers relative to G allele
carriers who were not smokers. In the smoker group of patients with severe and very severe
COPD (GOLD3+4), significant increase in OxyA1AT level was present in G allele carriers compared to AA homozygotes.
Conclusion: These findings suggest that MSRA rs10903323 gene polymorphism is probably not a
risk for COPD by itself but could represent a COPD modifier, since minor, G allele, is associated with an increased level of oxidized A1AT, indicating impaired ability of MSRA to repair oxidized
A1AT in COPD-smokers, and in severe form of COPD.

AB - Background: Problem of the variability between the different methods using for bone alkaline phosphatase(bALP) determination greately influences the clinical significance of bALP as direct marker of bone metabolism. The aim of this study was to compare immunoassay with electrophoresis technique for bALP determination. Methods: We measured bALP in 71 patients on hemodialysis with agar gel electrophoresis (ISO-PAL, SEBIA) and immunoassay (OSTASE, Beckman Coulter). Results: The analyzed methods showed significant correlation (Spearman’s rho: 0.776, P < 0.01), but we found statistically significant (P < 0.01) positive bias (27%)for the results measured by immunoassay. In support of this, using electrophoresis technique we have detected presence of the intestinal isoenzymes of alkaline phosphatase in 55% of patients with median value of 30% of the total alkaline phoshatase and presence of liver-2 alkaline phosphatase isoform in 42% of patients with median value of 16.6%. The Kendall’s W of 0.787 (P<0.0001) revealed significant concordance between two analysed methods.Cusum test showed no significant deviation from linearity(P=0.850).Conclusions:Despite good agreement between immunoassay methods and electrophoresis technique for bALP determination, interchangeability between these two methods is questionable. Although immunoassays are increasingly used, as fully automated methods, in a large number of laboratories and become routine methods for bALP determination, it should be beared in mind, besidesvarious interferences, also the heterogeneity of the bALP itself, especially in patients on hemodialysis.

AB - Measurement uncertainty (MU) of results is one of the basic recommended and accepted statistical
methods in laboratory medicine, with which analytical and clinical evaluation of laboratory
test quality is assessed. Literature data indicate that the calculation of MU is not a simple process,
but that its assessment in daily laboratory practice should be reduced to routine and simple
presentation, understandable to both laboratory professionals and physicians. In order to achieve
this, it is necessary to understand the purpose of the test for which MU is to be determined.
Various suggestions have been given for presentation of MU as a quantitative indicator of the
quality of the final measurement result in the medical laboratory. Although MU refers to the final
measurement result, this metrological concept reflects the entire laboratory measurement process.
The data on estimated MU is used to interpret the measured numerical result, and represents
quantitatively the quality of the measurement itself, i.e. how different are the results of
multiple measurements of the analyte of interest in the same sample, as well as whether the
method of determination itself is subjected to significant random and systematic deviation.
Initially, in the metrological concept, the MU is viewed in relation to the true value of the analyte
of interest. However, the true value of the analyte measured in the biological fluid matrix of the
study population cannot be known. It is therefore considered the closest value obtained by the
perfect method, for which the bias and inaccuracy, as measures of systematic and random error,
are equal to zero, which is practically impossible to achieve in routine laboratory practice.
Although current standards require accredited medical laboratories to estimate MU, none of
these guidelines provide clear guidance on how this can be achieved in daily laboratory work.
This review examines literary data and documents dealing with MU issues, but also highlights
what additional terms and data should be considered when interpreting MU. This paper ultimately
draws attention, and once again points out, that a simpler solution is needed for this universal
concept to be formally and universally applicable in routine laboratory medicine practice.

AB - Background:Some observational studies indicate an association of 25-hydroxy vitamin D (25(OH)D) insufficiency and atherogenic cholesterol concentrations. The aim of this study was to investigate relationship between 25(OH)D concentrations and lipid parameters in end stage renal disease (ESRD) patients, separately for predialysis, hemodialysis and peritoneal dialysis patients. Methods:We have adjusted 25(OH)D concentrations for seasonal variability with cosinor analysis, and performed all further analysis using these corrected 25(OH)D concentrations. Concentrations of 25(OH)D and the lipid parameters were determined in 214 ESRD patients and 50 control group participants. The analysis included the measurement of 25(OH)D by HPLC, apolipoprotein (Apo) AI, ApoB andLp(a) by nephelometry, total cholesterol (TC), high-densitylipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) and triglyceride (TG) by spectrophotometry and manually calculated ApoB/ApoAI and LDL-C/HDL-C ratio. Results:ESRD patients with adjusted 25(OH)D concentrations of ≤ 50 nmol/L had significantly higher TC (P =0.005) and ApoAI (P = 0.049). Significantly higher HDL-C (P = 0.011) and ApoAI (P = 0.020) were found in hemodialysis patients with the 25(OH)D concentrations of ≤ 50 nmol/L. The other analyzed lipid parameters differed significantly between predialysis, hemodialysis and peritoneal dialysis patients with 25(OH)D concentrations of <50 nmol/L. Conclusions:Our study indicate the significant relationship between 25(OH)D repletion and optimal concentrations of lipid parameters in ESRD patients. Further research is necessary to explain whether joint evaluation of vitamin D status and lipid abnormalities could improve cardiovascular outcome in ESRD patients.
Metode: Podesili smo koncentracije 25(OH)D za sezonsku varijaciju koristeći kosinor analizu, i u daljem ispitivanju smo koristili korigovane koncentracije 25(OH)D. Koncentracije 25(OH)D i lipidnih parametara su određene za 214 ESRD bolesnika i 50 učesnika kontrolne grupe. Analizirali smo koncentracije 25(OH)D metodom HPLC, apolipoproteina (Apo) AI, ApoB i Lp(a) nefelometrijski, ukupnog holesterola (TC), lipoproteina velike gustine (HDL-C), lipoproteina male gustine (LDL-C) i triglicerida (TG) spektro fotometrijski i ručno preračunavali indekse ApoB/ApoAI i LDL-C/HDLC.

AB - Literature data indicate that increased uric acid (UA) levels are an important risk factor for cardiovascular disease. The aim of this study was to examine the association of UA with cardiovascular risk factors in patients with hemodialysis using retrospective analysis. In 110 patients, we studied the values of basic renal function parameters, bone metabolism and cardiovascular risk factors in the period from 2010 to 2017. The results indicate a significant increase in urea (P=0.004), creatinine (P=0.028) and inorganic phosphate (P=0.001), as well as significant differences in gender (P=0.013), in risk categories defined on the basis of the UA decision limit (cut off <350 μmol/L). After controlling the effects of most parameters, statistically significant correlation coefficients were obtained for UA and urea (r=0.361; P=0.0013), creatinine (r=0.388; P=0.0005) and inorganic phosphate (r=0.366; P=0.0011). Significant regression coefficients were obtained for UA and male gender (β=-0.227, P=0.004), age (β=-0.298, P˂0.001), urea (β=0.271, P=0.005) and inorganic phosphate (β=0.232, P=0.009). The predictive value of independent parameters in relation to UA was confirmed for male gender (OR=3.595; 95% CI: 1.421-9.094; P=0.007) and inorganic phosphate (OR=14.842; 2.518-87.472, P=0.003). By Cox regression analysis of proportional hazard ratio, we obtained the most significant combined effect of the body mass index, dialysis and diastolic pressure on UA concentration in relation to the duration of hemodialysis (P <0.0001). The results of this long-term study suggest that UA can not be considered an independent cardiovascular risk factor, but that HD patients need to strategically control the level of MK in order to reduce the resulting complications, morbidity and mortality.
