AB - Background: Pharmacotherapy literacy (PTHL) is a specific ability to safely access, appraise and understand the available information concerning medication and to calculate and act accordingly. The concept of PTHL is mostly unknown for the majority of diabetes mellitus type 2 (DMT2) patients in Serbia. With diabetes being one of the major public health problems in Serbia with a prevalence of 9.1%, this two-study research aims at constructing performance-based instrument and estimating the prevalence of PTHL levels and identification of predictors of low PTHL scores in patients with DMT2. Methods: Multistage study was performed to adapt the existing performance–based instrument (PTHL-SR) into specific questionnaire for DMT2 population (PTHL-DM instrument). PTHL levels were assessed through cross-sectional study categorising patients into groups of low, medium, and high PTHL levels. We considered 19 predictors for low PTHL scores, from sociodemographic characteristics, health behaviours and health characteristics, access to health-related information and empowerment-related indicators. Univariate and multivariate logistic regression analyses were used to determine independent predictors of low PTHL. Results: The final 15-item PTHL-DM instrument proved to have satisfactory reliability (KR20 = 0.475) and internal reliability [ICC for the whole instrument was 0.97 with 95% confidence intervals (0.95–0.99)]. Positive correlation (rho = 0.69) between PTHL-DM score (15 questions) and the total PTHL-SR score (14 questions) was also observed. It was demonstrated that the majority of 350 patients had low PTHL (62%), and only 5% high PTHL level. Mean score on PTHL-DM was 7.8 ± 2.3. Probability of low PTHL increased among smokers, patients with low interest in health and those who estimated their health as bad. Patients who used pharmacists as sourse of information were less likely to be pharmacotherapy illiterate. Combined therapy with insulin and Oral Hypoglycemic Agents was associated with higher PTHL. Conclusions: Our data indicate that specific PTHL-DM tool is objective, valid, and reliable. It was found that low level of PTHL prevailed among DMT2 patients. Medication literacy is influenced by age, residence, education, and family status. Patients with better health literacy also reported better health behaviours. Different patient empowerment programs and approaches aimed at raising PTHL would be essential to improve self-management and control of this widespread chronic disease in Serbia.

AB - European pharmacopoeia (Ph. Eur.) is unique reference for quality of medicines in
Europe (First issue, 1969). Collaboration of Serbia is done through expert groups,
participation at Ph. Eur. Commission, quality control of medicines in National Control
laboratory (NKL), and network of Official medicines control laboratories (OMCL). The aim of
work was to present historical data on the collaboration between Serbia, Ph. Eur. and OMCL,
by searching data from ALIMS database, which coordinates this collaboration. Collaboration
started in 1991. when Yugoslavia ratified Convention on Ph. Eur. As an independent state,
Serbia was participating in the work of Ph.Eur. Commission since 2003. Serbia has 7
representatives in expert groups: 10B,11 (Organic Chemistry), 15 (Human Vaccines and
Sera), P4 (Patent medicines), PaedF (Pediatric Formulas), POW (Powders) and ST (Standard
Terms). These experts have developed 16 new monographs, European Pediatric Formulas
and participated in revision of over 10 existing monographs. Representatives of Serbia chair
P4 and POW groups. NKL is a full member of the OMCL network since 2004, and in 2013 it
was certified against ISO 17025. Since 2006, NKL was participating in the development of
new chemical reference substances (CRS), monitoring their quality, doing laboratory
performance studies, European systematic drug quality control and studies of suspicious and
unknown samples. By active participation in development of monographs and quality
control of medicines, the competence of experts has been improved. By application of the
modern technologies this has enabled patients in Serbia to have available medicines of
equivalent quality as on the EU market.
u Evropi (prvo izdanje, 1969). Saradnja Srbije sa Ph. Eur. se sprovodi kroz članstvo u
ekspertskim grupama, učešće u Komisiji Ph. Eur. i kroz kontrolu kvaliteta lekova u
Nacionalnoj kontrolnoj laboratoriji (NKL) u okviru mreže nacionalnih kontrolnih
Saradnja
počinje 1991. godine, kada je Jugoslavija ratifikovala Konvenciju o izradi Ph. Eur. Kao
samostalna država, Srbija od 2003.godine učestvuje u radu Komisije Ph. Eur. Srbija ima 7
predstavnika u sledećim ekspertskim grupama: 10B i 11 (Organska hemija), 15 (Humane
vakcine i serumi), P4 (Lekovi pod patentnom zaštitom), PaedF (Evropske pedijatrijske
formule), POW (Karakterizacija praškova) i ST (Standardni termini). Ovi eksperti su do sada
razvili 16 novih monografija Ph. Eur. i Evropskih pedijatrijskih formula i učestvovali u
reviziji preko 10 postojećih monografija. Predstavnici Srbije predsedavaju grupama P4 i
POW.
performansi laboratorije, evropskoj sistematskoj kontroli kvaliteta lekova i studijama
ispitivanja sumnjivih i nepoznatih uzoraka. Aktivnim učešćem Srbije u razvoju monografija i

AB - Pharmacotherapy literacy is literacy in the context of medicines. It represents ability
of individuals to find, evaluate, calculate and understand information about medicines,
needed for decisions, regardless transmission and content of information (written, spoken,
image, symbol) in order to reduce risk of poor pharmacotherapy outcomes (1). Adequate
pharmacotherapy literacy is strategy for proper and safe medicines use. Daily, many people
misuse medicines due to inadequate pharmacotherapy literacy. Recently, specific
assessment tools and interventions were developed to alleviate patients' needs for
simplification, education, and support during medicines use. The objective of study was to
review and present pharmacotherapy literacy researches in the adult population (in healthy
individuals and specific patient populations). Previous researches included respondents at
population level and in health institutions. It was developed 13 instruments for evaluation of
pharmacotherapy literacy, 9 for particular medicines group, and 4 instruments were general.
Most instruments included only dimensions of functionality and numeracy. In all
populations, predictors of pharmacotherapy literacy levels were: education, age, sex and
ability for self-medication. Persons with lower pharmacotherapy literacy need assistance
and simpler information when dispensing medicines. As a guideline for identifying patients
with lower pharmacotherapy literacy, a RALPH (Recognize and Address Limited
Pharmaceutical Literacy) interview was created to support pharmacists for easier
identification of patients with limited literacy (2). The number of pharmacotherapy literacy
studies is rising. Pharmacists, as the most accessible health professionals, have key role in
reducing patient confusion and improving pharmacotherapy literacy, through
communication tailored to their needs, after identification of pharmacotherapy literacy
levels.
AB - Farmakoterapijska pismenost predstavlja specifičnu pismenost u kontekstu upotrebe

bezbednu upotrebu lekova. Svakodnevno, mnogo osoba pogrešno primeni lek usled
neadekvatne farmakoterapijske pismenosti. Poslednjih godina razvijeni su specifični
intrumenti za procenu i intervencije usmerene ka olakšanju potreba pacijenata radi
pojednostavljenja, edukacije i podrške prilikom upotrebe lekova. Ovom studijom prikazana
pojedincima i u specifičnim populacijama pacijenata).
funkcionalnu i numeričku dimenziju pismenosti. U svim populacijama, kao prediktori nivoa
farmakoterapijske pismenosti izdvajaju se obrazovanje, starost, pol i sposobnost
samomedikacije. Osobe niže farmakoterapijske pismenosti traže asistenciju i jednostavnije
informacije prilikom izdavanja lekova. Kao smernica za identifikaciju pacijenata sa nižim
nivoom farmakoterapijske pismenosti kreiran je RALPH (Recognize and Address Limited
profesionalci, imaju ključnu ulogu u smanjenju konfuzije pacijenata i poboljšanju njihove
farmakoterapijske pismenosti, kroz komunikaciju prilagođenu njihovim potrebama, kada
procene nivo pismenosti.

funkcionalnu i numeričku dimenziju pismenosti. U svim populacijama, kao prediktori nivoa
farmakoterapijske pismenosti izdvajaju se obrazovanje, starost, pol i sposobnost
samomedikacije. Osobe niže farmakoterapijske pismenosti traže asistenciju i jednostavnije
informacije prilikom izdavanja lekova. Kao smernica za identifikaciju pacijenata sa nižim
nivoom farmakoterapijske pismenosti kreiran je RALPH (Recognize and Address Limited
profesionalci, imaju ključnu ulogu u smanjenju konfuzije pacijenata i poboljšanju njihove
farmakoterapijske pismenosti, kroz komunikaciju prilagođenu njihovim potrebama, kada

AB - Experiences from pharmacy practice shows that there are as many errors during use of medicines among pre-school children, mainly because of inappropriate dosage and parental misunderstanding of information about medicines use. The largest number of mistakes is due to inappropriate use of analgesics and antipyretics as the most commonly used non-prescription medicines, as a result of low level of literacy related to the use of medicines – pharmacotherapy literacy. According to King et al, pharmacoptherapy literacy is the „personal capacity to obtain, evaluate, calculate, and comprehend basic information about pharmacotherapy and pharmacy related services necessary to make appropriate medication-related decisions, regardless of the mode of content delivery (e.g. written, oral, visual images and symbols)“. The aim of this study was to construct questionnaire for assessment of pharmacotherapy literacy of parents of pre-school children related to understanding of information about use of most commonly used medicines (analgetics and antipyretics), and evaluation of its psychometric characteristics of that questionnaire, as well as to assess pharmacotherapy literacy of parents of pre-school children related to understanding of information about most commonly used medicines (analgesics and antipyretics) with constructed and validated questionnaire for assessment of understanding of information about medicines use among parents of pre-school children (Parental pharmacotherapy literacy Questionnaire in Serbian, PTHL-SR). The study also included assessment of functional health literacy among parents of pre-school children and understanding information about medicines use, and evaluation of impact of functional health literacy on understanding information of over- the-counter medicines for pre-school children. AB - Background and objectives: Pharmacotherapy literacy (PHTL) is an individual’s capacity to obtain, evaluate, calculate, and comprehend basic information about pharmacotherapy and pharmacy-related services necessary to make appropriate medication-related decisions, regardless of the mode of content delivery (e.g., written, oral, visual images and symbols). It is already proven that low PHTL of parents can cause serious problems in the treatment of a pediatric population. We aimed to identify the dierences in parental PHTL levels, socio-demographic and health-related characteristics (chronic disease of a child, breastfeeding of a child, annual visits to a pediatrician, parental-self-estimation of health status) between rural and urban areas and to investigate the influence of living in rural areas on a low PHTL level. Materials and methods: Our study was cross-sectional with a validated 14-item instrument (“Parental pharmacotherapy literacy questionnaire—Serbian”), which assessed overall PHTL and its three domains of knowledge, understanding and numerical skills necessary for the safe use of medicines. We analyzed 250 parents of pre-school children (1–7 years old) in rural areas and 182 parents from urban areas in Serbia. Results: Every tenth parent from rural and every fourth parent from urban areas had the highest PHTL level or more than 85% correct answers. However, 51% and 28% of parents in rural and urban areas, respectively, had a low PHTL level (less than 65% correct answers), [Õ2(1, n = 432) = 33.2; p < 0.001]. Parents from dierent areas statistically diered in age, education level, employment, breastfeeding and annual visits to pediatrician rate. Those from rural areas had almost twice the probability of low PHTL levels (ORa = 2.033; p = 0.003) than their urban counterparts, independently of other examined parental characteristics. Conclusions: Parents from rural areas have more diculties to obtain, evaluate, calculate and comprehend basic information related to pharmacotherapy than parents from urban areas.

AB - Background and objectives: Pharmaceutical literacy skills of parents are crucial for appropriate and safe medication use in pre-school children (ages 1-7 years). A recent study on pharmacotherapy literacy from Serbia showed that one in five parents have difficulty understanding common information about the use of medicines. Because antipyretics are considered to be the most frequently used group of over-the-counter (OTC) medications during the pre-school period, we aimed to: (i) examine parental practice and expectations in antipyretic medication use, and (ii) analyze associations of parental practice and expectations related to socio-economic status and pharmacotherapy literacy. Materials and methods: A cross-sectional survey using a self- report validated specific instrument was conducted with the parents of pre-school children in kindergartens in Belgrade, Serbia. Pharmacotherapy literacy refers to the knowledge and personal skills needed to meet the complex demands of medicine use in both healthcare and non-healthcare settings. A comprehensive literature review, expert-focus group consultation, and pre-testing were employed in 4-item multiple-choice test development to explore practice and expectations related to the use of OTC pediatric antipyretic medicines. Results: The final analytical cohort was comprised of 813 participants, the majority (63.3%) chose a medicine based on a physician's suggestion and only 15.4% of parents reported they would follow the advice of a pharmacist. More than a half of parents (54.1%) would need advice about antipyretic medicine from a pharmacist, firstly in a simpler language. Parents satisfied with the information given by a pharmacist had higher pharmacotherapy literacy, compared to parents with lower levels (OR-0.718, 95%CI (0.597-0.865), p lt 0.001). Men had a higher expectation of pharmacists to explain medicine use in a simpler language (OR-1.630, 95%CI (1.063-2.501), p = 0.025), as well as parents with three or more children (OR-2.527, 95%CI (1.43-4.459), p = 0.001). Parents with higher knowledge about medicine use were less likely to ask for simpler information (OR-0,707; 95%CI (0,583-0,856), p lt 0,001). Conclusions: Our main finding is that practice in antipyretic OTC medicine use was associated with levels of parental pharmacotherapy literacy. The expectations of pharmacists were higher among parents with lower levels of pharmacotherapy literacy, who expected more information in a simpler and more precise language. This study highlighted the need for pharmacists to identify risks in parental practice and to provide information about medicines to parents of pre-school children in a simpler and more appropriate way.

abstract = "Background and objectives: Pharmaceutical literacy skills of parents are crucial for appropriate and safe medication use in pre-school children (ages 1-7 years). AB - Parental health literacy plays an important role in children's health. Experiences from pharmacy practice show that is necessary to check if parents understand instructions about use of medicines for children. This study aimed to assess pharmacotherapy literacy of parents of pre-school children and to examine association of parental pharmacotherapy literacy level with parent's socio-demographic characteristics. The study was cross-sectional, conducted among parents of pre-school children (1-7 years of age), in kindergartens in several municipalities of Belgrade, Serbia, during regular parents meetings, from May to October 2016. Functional health literacy was measured by the Serbian version of the Short Test of Functional Health Literacy in Adults (S-TOFHLA). Parental pharmacotherapy literacy was assessed with newly constructed PTHL-SR questionnaire with good psychometric characteristics (Parental pharmacotherapy literacy questionnaireSerbian). Overall, 813 parents participated in the study, mostly females (81.30%), between 30 to 40 years of age (70.85%) with two children (56.70%). Almost all of our study participants (99%) had adequate health literacy as assessed by S-TOFHLA. Mean score on PTHL-SR was 72.83% (standard deviation was 13.37), with better results among females than males (72% of women were in the group of highest PTHL-SR results). Our study showed that many parents (76.5%) knew the appropriate usage of non-prescription medicine for children, 57.2% parents were able to correctly calculate the dose of oral syrup for a child, and only 43.3% were able to interpret non-prescription dosage information written on the package. The majority of parents (61.3%) would make a dosage to child based on age and not on their weight. Every fifth parent with adequate functional health literacy measured by S-TOFHLA test, achieved the lowest results measured by PTHL-SR. Higher performance of the PTHL-SR was significantly correlated with education (p lt 0.001), female sex (p lt 0.001), married parents and those living in common-law (p lt 0.001), older parents (p lt 0.05) and parents who have more children (p lt 0.05), and are non-smokers (p lt 0.05). These results provide evidence that limitations in understanding common information about use of medicines are widespread among parents of pre-school children and encourage efforts for further investigation. PTHL-SR questionnaire may be a useful tool for identification of parents who need more instructions and assistance from healthcare providers, above all in providing better communication, written or spoken at community pharmacy settings.

AB - Health literacy measurement means appliance of different instruments which determine the degree of understanding of read and written information, as well as numerical skills necessary for making relevant health decisions. The objective of this study is to identify instruments which are used to measure health literacy at a primary healthcare level, to compare them and analyze their structure in order to examine their relevance for health literacy evaluation in public pharmacies. Collection of data related to health literacy is done by literature search, and for every instrument for measuring health literacy given is its content and description of performance and conditions for use (with special view if it is applicable in pharmacy settings). Analysis of instruments is done by method of comparison. There were identified 5 instruments for direct measuring of health literacy and each could be used in pharmacy settings. It was concluded that the NVS (Newest Vital Sign) which measures functional and critical health literacy in a very short time would be the most suitable instrument for health literacy measurement in pharmacy settings.
