AB  - Hemodialysis (HD) removes nitrogenous waste products from patients’ blood through a semipermeable mem- brane along a concentration gradient. Near-infrared spectroscopy (NIRS) is an underexplored method of monitoring the concentrations of several molecules that reflect the efficacy of the HD process in dialysate samples. In this study, we aimed to evaluate NIRS as a technique for the non-invasive detection of uremic solutes by assessing the correlations between the spectrum of the spent dialysate and the serum levels of urea, creatinine, and uric acid. Blood and dialysate samples were taken from 35 patients on maintenance HD. The absorption spectrum of each dialysate sample was measured three times in the wavelength range of 700-1700 nm, resulting in a dataset with 315 spectra. The artificial neural network (ANN) learn- ing technique was used to assess the correlations between the recorded NIR-absorbance spectra of the spent dialysate and serum levels of selected uremic toxins. Very good correlations between the NIR-absorbance spectra of the spent dialysate fluid with serum urea (R=0.91) and uric acid (R=0.91) and an excellent correlation with serum creatinine (R=0.97) were obtained. These results support the application of NIRS as a non-invasive, safe, accurate, and repetitive technique for online monitoring of uremic toxins to assist clinicians in assessing HD efficiency and individualization of HD treatments.

AB  - Background and Objectives: Diabetic foot (DF) development is driven by complex interactions of hyperglycemia, inflammation, and oxidative stress (OS). We aimed to investigate OS and inflammatory biomarkers in patients with DF and their potential to improve early diagnosis and management of DF. Materials and Methods: The prooxidant–antioxidant balance (PAB), superoxide dismutase (SOD), total oxidative status (TOS), total sulfhydryl groups (SHG), routine biochemical parameters, and complete blood count were determined in 42 patients with type-2 DM, of which 23 patients had DF, while 19 patients were without DF complications. The neutrophils-to-lymphocyte ratio (NLR) was evaluated as a biomarker of inflammation. Results: Patients with DF had significantly higher (p < 0.05) PAB levels (170 ± 33.9 U/L) compared to those without DF complications (142 ± 31.3 U/L). In addition, patients with DF had significantly reduced SOD activities (p < 0.01). NLR values were significantly higher in the DF group (median: 2.8; interquartile range: 2.0–4.3) than in the group without DF (median: 1.4; interquartile range: 1.4–2.1; p < 0.01). A positive correlation was found between the PAB and NLR index (r = 0.449; p < 0.05). The diagnostic accuracy of both PAB (AUC = 0.741; p < 0.01) and NLR (AUC = 0.760; p < 0.01) was estimated as acceptable. Conclusions: In conclusion, the development of DF is associated with enhanced OS and inflammation processes. PAB and NLR could be useful non-invasive biomarkers of DF development.

AB  - Background: Thyrotropin receptor autoantibodies (TSH-R-Ab) are indispensable biomarkers in the laboratory assessment of thyroid-associated orbitopathy (TAO). Clinical sensitivity of three different assays for TSH-R-Ab determination was evaluated in patients with TAO.

Methods: 87 consecutive TAO patients were enrolled and their serum samples analyzed in parallel with three assays. An ECLIA competitive binding and a chemiluminescent bridge immunoassay were used to measure total and bind-ing TSH-R-Ab concentration, while their functional activity was determined using a stimulatory TSH-R-Ab (TSAb) cell-based bioassay.

Results: Compared to the two binding assays (ECLIA p<0.001, bridge p=0.003), the TSAb bioassay was more sensitive pertaining to the positive detection of TSH-R-Ab in TAO patients. No difference (p=0.057) was noted between the ECLIA and bridge assays regarding sensitivity rate. All patients with active and/or moderate-to-severe TAO tested positive in the TSAb bioassay (100% and 100%, respectively), while the positivity rates for bridge and ECLIA binding assays were 89.7% and 82.1% for active TAO, and 90.2% and 86.3% for severe TAO, respectively. Negative predictive values of the bioassay, bridge, and ECLIA assays were 100%, 75%, and 71%, respectively for active TAO, and 100%, 86%, and 71%, respectively for moderate-to-severe TAO. The superiority of the bioassay was most prominent in euthyroid (ET) TAO. Positivity rates of the TSAb bioassay, bridge and ECLIA binding assays were 89.6%, 75%, and 64.6%, respectively for inactive TAO; 86.1%, 69.4%, and 52.8%, respectively for mild TAO; 87.5%, 62.5%, and 12.5%, respectively for euthyroid TAO. The bridge assay correlated better with the ECLIA binding assay (r=0.893, p<0.001), compared to the bioassay (r=0.669, p<0.001).

Conclusions: In patients with TAO of various activity and severity, the TSAb bioassay demonstrates a superior clinical performance compared to both ECLIA and bridge binding assays.
AB  - Uvod: Autoantitela na receptore za tireostimulišući hormon (TSH-R-Ab) su nezamenljivi biomarkeri u laboratorijskoj proceni orbitopatije udru'ene sa štitnom žlezdom (TAO). U radu je procenjena klinička osetljivost tri različita testa za određivanje TSH-R-Ab kod pacijenata sa TAO. Metode: U studiju je ključeno 87 uzastopnih pacijenata sa TAO i njihovi uzorci seruma su analizirani paralelno sa tri testa. Za merenje ukupne i vezujuće koncentracije TSH-RAb korišćeni su ECLIA imunohemijski test kompetitivnog vezivanja i hemiluminiscentni imunohemijski "sendvič" test, dok je njihova funkcionalna aktivnost određena pomoću ćelijskog biološkog testa (bioeseja) za određivanje stimulatornih TSH-R-Ab (TSAb). Rezultati: U poređenju sa dva imunohemijska testa vezivanja (ECLIA P<0,001, "sendvič test" P=0,003), TSAb bioesej se pokazao najosetljivijim u pogledu pozitivne detekcije TSH-RAb kod TAO pacijenata. Nikakva razlika nije detektovana (P=0,057) između ECLIA i "sendvič" testa u pogledu stope osetljivosti. AB  - Objective: Polycystic ovary syndrome (PCOS) is associated with altered lipid profile and increased small, dense LDL particles (sdLDL). Considering that paraoxonase 1 (PON1) is an anti-oxidative enzyme located on high-density lipoprotein (HDL) particles, the aim of this study was to investigate the connection between oxidative stress (OS) and PON1 activity with lipoprotein subclasses in PCOS depending on obesity. Methods: In 115 PCOS patients lipoprotein subclasses distributions were determined by gradient gel electrophoresis. OS status was assessed by total oxidative status (TOS), advanced oxidation protein products (AOPP), malondialdehyde (MDA), prooxidant-Antioxidant balance (PAB), total antioxidant status (TAS) and superoxide dismutase (SOD) and PON1 activity. Results: Overweight/obese PCOS patients (n=55) had increased OS compared to normal weight patients (n=60). In addition, overweight/obese group had lower HDL size and higher proportion of HDL 3a subclasses (P<0.05). PAB was in negative correlation with HDL 2a (P<0.001), whereas MDA and SOD correlated positively with HDL 3 subclasses (P<0.05). Serum PON1 activity was positively associated with proportions of PON1 activity on HDL 2b (P<0.05) and 2a (P<0.01), but negatively with the proportion on HDL 3 particles (P<0.01). LDL B phenotype patients had increased TAS, SOD and PON1 activity on HDL 2b, but decreased PON1 activity on HDL 3 subclasses. Conclusion: OS is associated with altered lipoprotein subclasses distribution in PCOS patients. Obesity in PCOS affects the profile of HDL subclasses, reflected through the reduced proportion of PON1 activity on HDL 3 subclasses in the presence of sdLDL particles.

AB  - Non-enzymatic glycation, oxidative stress (OS) and dyslipidemia are the main
metabolic alterations behind the development of macrovascular
complications (cardiovascular diseases) of diabetes mellitus (DM).
However, clinical relevance of biomarkers of these processes in patients with microvascular
complications (nephropathy, neuropathy, retinopathy) is less understood. Therefore, the aim
of this study was to examine advanced glycation products (AGEs), biomarkers of OS, and
dyslipidemia in 100 DM patients (33 without microvascular complications and 77 with
complications) and 30 subjects without DM. AGEs levels were higher in patients with
complications than in those without complications (median: 5.72; interquartile range: 4.60-
6.54 U/mL vs. median: 4.84; interquartile range: 4.10-5.40 U/L; P<0.05). In addition to
AGEs, the group with diabetic retinopathy had higher plasma total antioxidant capacity
(P<0.05), while the group with diabetic nephropathy had smaller LDL size than the patients
without these complications (25.48±1.26 nm vs. 26.21±1.19 nm; P<0.05). The patients
with co-existing cardiovascular disease were further characterized by dysfunctional HDL
particles, as evidenced by increased small HDL particles (P<0.05) and reduced paraoxonase-
1 activities. Significant increase in both pro-oxidant-antioxidant balance and ischemia-
modified albumin (P<0.05), with simultaneously decreased activity of superoxide-dismutase
(P<0.05) was found in patients with progressive diabetic neuropathy, indicating the highest
degree of oxidative damage. It can be concluded that patients with microvascular
complications of DM have aggravated redox imbalance and lipid profile alterations. In
addition to metabolic control, strategies aimed at lowering OS and correcting dyslipidemia
can contribute to the prevention of microvascular complications of diabetes.

procesi koji dovode do razvoja makrovaskularnih komplikacija (kardiovaskularnih bolesti)
dijabetesa melitusa (DM). Međutim, klinički značaj određivanja biomarkera ovih procesa
kod pacijenata sa mikrovaskularnim komplikacijama (retinopatija, nefropatija, neuropatija)
nije dovoljno razjašnjen. Cilj ovog istraživanja je bio ispitivanje produkata uznapredovale

transmittable and heterogenic infection of the respiratory tract, characterized by a broad spectrum
of clinical manifestations with a different degree of severity. Medical laboratories play an
important role in early diagnosis and management of Coronavirus Disease 2019 (COVID-19)
patients. Indeed, the results of several laboratory tests are essential for assessing the severity of
the disease, selecting appropriate therapeutic procedures and monitoring treatment response.
Routine laboratory testing in COVID-19 patients includes biomarkers of acute phase reaction,
hematological and biochemical parameters that indicate tissue injury. The aim of this review
paper is to describe the role of these biomarkers in the diagnostics and management of adult and
pediatric COVID-19 patients.
AB  - Novi koronavirus SARS-CoV-2 je uzročnik lako prenosive i veoma heterogene infekcije respiratornog trakta, koju karakteriše širok spektar kliničkih manifestacija, različitog stepena težine. Medicinske laboratorije igraju važnu ulogu u ranoj dijagnostici i praćenju pacijenata sa COVID-19. Rezultati nekoliko laboratorijskih testova su od suštinskog značaja za procenu težine bolesti, odabir odgovarajućih terapijskih procedura i praćenje odgovora na terapiju. Rutinska laboratorijska ispitivanja kod pacijenata sa COVID-19 uključuju biomarkere akutne faze, hematološke i biohemijske parametre koji ukazuju na oštećenje tkiva. Cilj ovog rada je da se prikaže uloga tih biomarkera u dijagnostici i praćenju toka bolesti kod odraslih i pedijatrijskih pacijenata sa COVID-19.

AB  - Objective: Polycystic ovary syndrome (PCOS) is associated with altered lipid profile and increased small, dense LDL particles (sdLDL). Considering that paraoxonase 1 (PON1) is an anti-oxidative enzyme located on high-density lipoprotein (HDL) particles, the aim of this study was to investigate the connection between oxidative stress (OS) and PON1 activity with lipoprotein subclasses in PCOS depending on obesity. Methods: In 115 PCOS patients lipoprotein subclasses distributions were determined by gradient gel electrophoresis. OS status was assessed by total oxidative status (TOS), advanced oxidation protein products (AOPP), malondialdehyde (MDA), prooxidant-Antioxidant balance (PAB), total antioxidant status (TAS) and superoxide dismutase (SOD) and PON1 activity. Results: Overweight/obese PCOS patients (n=55) had increased OS compared to normal weight patients (n=60). In addition, overweight/obese group had lower HDL size and higher proportion of HDL 3a subclasses (P<0.05). PAB was in negative correlation with HDL 2a (P<0.001), whereas MDA and SOD correlated positively with HDL 3 subclasses (P<0.05). Serum PON1 activity was positively associated with proportions of PON1 activity on HDL 2b (P<0.05) and 2a (P<0.01), but negatively with the proportion on HDL 3 particles (P<0.01). LDL B phenotype patients had increased TAS, SOD and PON1 activity on HDL 2b, but decreased PON1 activity on HDL 3 subclasses. Conclusion: OS is associated with altered lipoprotein subclasses distribution in PCOS patients. Obesity in PCOS affects the profile of HDL subclasses, reflected through the reduced proportion of PON1 activity on HDL 3 subclasses in the presence of sdLDL particles.

AB  - Background: Polycystic ovary syndrome (PCOS) is a cardiornetabolic disorder whose features include dyslipidemia, increased oxidative stress (OS, oxy) and chronic inflammation. The aim of this study was to investigate the ability of a summary score for dyslipidemia, OS and inflammation (the DOI score) to discriminate PCOS patients from healthy individuals and to evaluate the effect of obesity on individual scores and the DOI score in patients. Methods: Lipid status parameters, OS status parameters (advanced oxidation protein products; total oxidative status; prooxidant-antioxidant balance; malondialdehyde; total protein sulphydryl groups and paraoxonase 1 activity) and CRP were measured in 114 patients and 50 controls using standardised assays. The DOI score was calculated as the sum of dyslipidemia, oxy and inflammation scores, determined as Z-score values for every subject in relation to the controls. Results: PCOS patients had significantly higher oxy-score compared to controls (P lt 0.001). In addition, the DOI score was significantly higher in PCOS patients (P lt 0.001) as the dyslipidemia (P lt 0.05) and inflammatory scores (P lt 0.001) were greater. According to ROC analysis, the oxy-score showed better diagnostic accuracy in discriminating PCOS patients compared to the DOI score (AUC>0.9, P lt 0.01). Furthermore, obesity affected the risk scores in patients, especially the DOI score (significantly higher DOI scores in such patients, P  lt 0.001). Conclusion: PCOS patients had greater dyslipidemia, chronic inflammation and OS compared to controls and could be segregated using all four scores. Our data suggest that weight gain could be the common factor responsible for induction and propagation of dyslipidemia, OS and inflammation in PCOS patients.

AB  - Background: Polycystic ovary syndrome (PCOS) is associated with reproductive and metabolic abnormalities. The aim of this study was to analyse risk of cardiovascular disease (CVD) in PCOS, to define individual risk factors and assess their ability to predict risk. Methods: Fifty-four young women with PCOS (22 obese and 32 normal weight) were compared to 46 respective controls (17 obese and 29 normal weight). Anthropometric parameters, lipid status parameters, inflammation markers, concentrations of glucose, transaminases, sex and anterior pituitary hormones, sex hormone binding globulin (SHBG) and androgens were measured. Cardiovascular Risk Score (CVRS), indices for identifying Non-Alcoholic Fatty Liver Disease (NAFLD) and the Index of Central Obesity (ICO) were calculated. Results: Significantly higher CVRS values (p lt  0.05) were found in obese PCOS women compared to normal weight control and normal weight PCOS groups. Anthropometric parameters, lipid status parameters and fibrinogen (p lt  0.001, p lt  0.01) were higher in women with higher CVRS. The most significant CVRS predictors in all PCOS women were SHBG, androstenedione, follicle-stimulating hormone (FSH) and dehydroepiandrosterone sulphate (DHEAS). ICO and all NAFLD indices exhibited significant positive correlation with CVRS and a model consisting of these indices provided good diagnostic accuracy (AUC> 0.8) in identifying patients with increased cardiovascular risk (CVR). Conclusions: Obesity is a higher risk for developing CVD than PCOS alone. Anthropometric parameters, lipid parameters, fibrinogen, NAFLD indices and ICO increase CVR in PCOS women. For the prediction of CVR in PCOS, we suggest a combination of NAFLD indices and ICO.
