Novel anthropometric parameters in the adult population with prediabetes

AB  - OBJECTIVE: Although it is assumed that novel-derived anthropometric indices can better reflect cardiometabolic risk than traditional ones, the results are conflicting. Previous studies have mainly focused on patients with type 2 diabetes mellitus. However, studies conducted on populations with prediabetes are scarce. The present study aimed to examine the potential relationship between prediabetes and novel anthropometric parameters [that is, cardiometabolic index (CMI), visceral adiposity index (VAI), lipid accumulation product index (LAP), body roundness index (BRI), and body adiposity index (BAI)] and traditional parameters [that is, waist circumference (WC), hip circumference (HC), body mass index (BMI), waist-to-height ratio (WHtR), and waist-to-hip ratio (WHR)] in adults with prediabetes. PATIENTS AND METHODS: This case-control cross-sectional study included 177 patients with prediabetes and 609 control subjects. Biochemical and simple anthropometric parameters were measured (WC, HC, body weight, and height), whereas the other parameters were calculated. RESULTS: WC, CMI, VAI, and LAP independently correlated with prediabetes. Principal component analysis (PCA) was used to extract several factors that correlated with prediabetes. Significant predictive capability was demonstrated for non-traditional anthropometric/lipid-related factors and WHipR-related factors for prediabetes (OR=1.334 and OR=1.202, respectively). However, only non-traditional anthropometric/lipid-related factors (i.e., VAI, CMI, and LAP) demonstrated an independent significant positive relationship with prediabetes in multivariate binary regression analysis. CONCLUSIONS: CMI, VAI, and LAP could be superior to BAI, BRI, and conventional anthropometric parameters for discriminating patients with prediabetes in the adult population. Prospective trials are needed to confirm our results.

AB  - OBJECTIVE: There are no studies that investigated the association between red blood cell (RBC) and platelet (PLT) indices in relation to obesity in a cohort of exclusively late adolescents. Hence, we aimed to explore this potential relationship. PATIENTS AND METHODS: A cohort of adolescents (n=156) aged between 16-19 years was included. Iron homeostasis parameters [i.e. RBC, hemoglobin (HGB), hematocrit (HCT), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), and mean corpuscular hemoglobin concentration (MCHC) and red cell distribution width (RDW)] and platelet indices [i.e., PLT, mean platelet volume (MPV), plateletcrit (PCT) and platelet distribution width (PDW)] were determined on the automatic hematology analyzer. Their indexes (i.e., MCV/RBC, MCH/RBC, RDW/MCV, MPV/ PLT and PDW/PCT) were calculated. RESULTS: Univariate binary regression analysis showed negative associations between body mass index (BMI) and RDW, PDW, and PDW/PCT, respectively, and positive associations between BMI and MPV and PCT, respectively. However, only RDW kept the independent negative association with BMI in multivariate binary regression analysis [Odds Ratio (95% Confidence Interval)=0.734 (0.548-0.983); p=0.038]. CONCLUSIONS: Lower RDW values are the independent predictor of higher BMI in the adolescent population. As a low-cost and simply measured parameter, RDW could be a useful diagnostic biomarker in young populations with overweight/obesity.

The correlation of metabolic and renal biomarkers with vitamin D status in postmenopausal women

AB  - Background: To our knowledge, the mutual involvement of a variety of metabolic and renal biomarkers and vitamin D (determined as 25-hydroxyvitamin D [25(OH)D]) in postmenopausal women has not been examined yet. Therefore, we aimed to explore such a relationship by a thorough statistical multimarker approach. Methods: A total of 150 (diabetes and cardiovascular disease-free) postmenopausal women were included. Anthropometric and biochemical parameters were measured. The fatty liver index (FLI) and Homeostasis model assessment of insulin resistance (HOMA-IR) were calculated. Univariate and multivariate binary logistic regression analyses were used to test the predictions of cardiometabolic markers for [25(OH)D] status. Principal component analysis (PCA) was applied to explore the effect of examined biomarkers on [25(OH)D] status. Results: Univariate binary regression analysis revealed positive associations between FLI and [25(OH)D] [OR=1.036 (1.020–1.052), P<0.001] and HOMA-IR with [25(OH)D] [OR=2.385 (1.601–3.551), P<0.001]. Multivariate logistic regression analysis with backward selection enabled us to find the best model consisted of two markers, i.e. FLI [OR=1.032 (1.016–1.048), P<0.001] and glucose [OR=2.167 (1.001–4.693), P=0.050] associated with the first tertile of [25(OH)D]. PCA extracted 2 significant factors: Renal function-related factor and Metabolic syndrome-related factor. Renal function-related factor [i.e. creatinine, uric acid, retinol-binding protein 4 (RBP4) and cystatin C] explained 29% of the variance. Also, Metabolic syndrome-related factor (i.e. HOMA-IR, FLI, hsCRP and fibrinogen) explained 29% of the variance of the tested parameters. Moreover, binary logistic regression analysis revealed significant positive predictive ability of both factors, i.e. Renal function-related factor and Metabolic syndrome-related factor (P=0.044 and P<0.001, respectively) towards [25(OH)D] status.
Conclusion: The joint involvement of renal function
biomarkers and metabolic syndrome-related features (i.e.
insulin resistance, inflammation and non-alcoholic fatty
liver disease) can differentiate postmenopausal women
with lower [25(OH)D] levels from other women.
AB  - Uvod: Prema našem saznanju zajednički uticaj različitih
metaboličkih i renalnih biomarkera na nivo vitamin D
(odre|enog kao 25-hidroksivitamin D [25(OH)D]) kod
žena u postmenopauzi još uvijek nije ispitivan. Stoga je cilj
istraživanja bio da se ispita povezanost ovih biomarkera
primenom temeljne statističke analize koja uključuje više
navedenih biomarkera.
Metode: Ukupno 150 žena u postmenopauzi (koje nemaju
dijabetes, ni kardiovaskularna oboljenja) je uključeno u
istraživanje. Mereni su antropometrijski i biohemijski parametri. Indeks masne jetre (FLI) i indeks insulinske rezistencije (HOMA-IR) su izračunati. Univarijantna i multivarijantna
binarna logistička regresija su primenjene da bi se odredili
kardiometabolički prediktori nivoa [25(OH)D].
Rezultati: Univarijantna binarna regresiona analiza je
pokazala pozitivnu korelaciju izme|u FLI i [25(OH)D]
[OR=1.036 (1.020–1.052), P<0,001] i HOMA-IR sa
[25(OH)D] [OR=2.385 (1.601–3.551), P<0,001].
Multivarijantna logistička regresiona analiza je pokazala
najbolji model sa 2 markera, tj. FLI [OR=1.032 (1.016–
1.048), P<0,001] i glukoza [OR=2.167 (1.001–4.693),
P=0,050] koji su korelirali sa prvom tercilom vrednosti
[25(OH)D]. PCA je izdvojila 2 značajna faktora: Renal
function-related faktor i Metabolic syndrome-related faktor.
Renal function-related faktor [tj. kreatinin, mokraćna kiselina, retinolvezujui protein 4 (RBP4) i cistatin C] su
objasnili 29% varijanse. Takođe, Metabolic syndrome-
related faktor (tj. HOMA-IR, FLI, hsCRP i fibrinogen) su
objasnili 29% varijanse ispitivanih parametara. štaviše,
binarna logistička regresiona analiza je pokazala pozitivnu
prediktivnu sposobnost oba faktora, tj. Renal function-
related faktora i Metabolic syndrome-related faktora
(P=0,044 i P<0,001) za nivo [25(OH)D].
Zaključak: Zajedničko dejstvo renalnih biomarkera i
markera metaboličkog sindroma (tj. insulinska rezistencija,
inflamacija i nealkoholna steatoza jetre) mogu diferencirati
žene u postmenopauzi sa nižim nivoom [25(OH)D] u
odnosu na ostale žene.

Multimarker Approach as More Reliable Method Than Single Vitamin D in Relationship with Type 2 Diabetes Mellitus in Montenegrin Postmenopausal Women

AB  - Objective: Previous studies suggested that ethnic differences, sex and obesity could modify the relationship between 25-hydroxyvitamin D [25(OH)D], glycometabolic markers and/or type 2 di- abetes mellitus (T2D). We aimed to examine the potential relationship between [25(OH)D] and T2D in postmenopausal women in Montenegro. In addition, we aimed to explore if a set of biomarkers, rather than [25(OH)D] as a single biomarker, could better explain its potential association with T2D. Patients and Methods: A total of 116 postmenopausal, otherwise healthy women and 48 postmenopausal women with T2D were included. Univariable and multivariable binary logistic regression analy- sis, along with principal component analysis (PCA), were applied to test the associations between examined biomarkers/set of biomarkers with T2D. Results: Women with T2D had lower serum [25(OH)D] levels than healthy controls (p = 0.024). No independent relationship between [25(OH)D] and T2D was found. PCA extracted three significant factors that were associated with T2D, i.e., age-glycometabolic-related factor (i.e., with positive loadings of age, glucose and insulin; OR = 11.321, p < 0.001), obesity-inflammation- related factor (i.e., with positive loadings of hsCRP and WC, and negative loading of [25(OH)D]; (OR = 2.079, p < 0.001)) and lipid-related factor (i.e., with positive loadings of TG and LDL-c, and negative loading of HDL-c; OR = 1.423, p = 0.044). Conclusions: The relationship between [25(OH)D] and T2D is modulated by central obesity (as measured by WC) and inflammation (as measured with hsCRP) in postmenopausal women. Their joint measurement, rather than [25(OH)D] itself, could provide better information for the risk assessment for T2D in postmenopausal women.

abstract = "Objective: Previous studies suggested that ethnic differences, sex and obesity could modify the relationship between 25-hydroxyvitamin D [25(OH)D], glycometabolic markers and/or type 2 di- abetes mellitus (T2D). We aimed to examine the potential relationship between [25(OH)D] and T2D in postmenopausal women in Montenegro. In addition, we aimed to explore if a set of biomarkers, rather than [25(OH)D] as a single biomarker, could better explain its potential association with T2D. Patients and Methods: A total of 116 postmenopausal, otherwise healthy women and 48 postmenopausal women with T2D were included. Univariable and multivariable binary logistic regression analy- sis, along with principal component analysis (PCA), were applied to test the associations between examined biomarkers/set of biomarkers with T2D. Results: Women with T2D had lower serum [25(OH)D] levels than healthy controls (p = 0.024). No independent relationship between [25(OH)D] and T2D was found. PCA extracted three significant factors that were associated with T2D, i.e., age-glycometabolic-related factor (i.e., with positive loadings of age, glucose and insulin; OR = 11.321, p < 0.001), obesity-inflammation- related factor (i.e., with positive loadings of hsCRP and WC, and negative loading of [25(OH)D]; (OR = 2.079, p < 0.001)) and lipid-related factor (i.e., with positive loadings of TG and LDL-c, and negative loading of HDL-c; OR = 1.423, p = 0.044). Conclusions: The relationship between [25(OH)D] and T2D is modulated by central obesity (as measured by WC) and inflammation (as measured with hsCRP) in postmenopausal women. Their joint measurement, rather than [25(OH)D] itself, could provide better information for the risk assessment for T2D in postmenopausal women.",

AB  - Introduction: Markers of iron homeostasis are related to insulin resistance (IR) in adults. However, studies in children and adolescents are scarce and show contradictory results. The aim was to evaluate the potential relationship between iron status markers and IR. Additionally, no previous study has explored the mutual effect of biomarkers of iron homeostasis and inflammation (i.e. high sensitivity C-reactive protein (hsCRP)), and adipokines (i.e. retinol-binding protein 4 (RBP4)) on IR in the cohort of adolescent girls. Material and methods: A total of 60 girls age between 16 and 19 years were included in the study. Serum levels of ferritin, transferrin, soluble transferrin receptor (sTfR), hsCRP, and RBP4 were measured by immunonephelometry. Homeostasis model assessment of insulin resistance (HOMA-IR) and iron homeostasis indexes were calculated. Univariate and multivariate binary logistic regression analysis were used to investigate the possible independent associations of the examined biomarkers. Principal component analysis was used to examine their mutual effect on HOMA-IR in the studied girls. Results: Ferritin, sTfR, hsCRP and RBP4 were significant predictors for higher HOMA-IR in univariate analysis (p = 0.020, p = 0.009, p = 0.007, p = 0.003, respectively). Multivariate regression analysis after adjustment for waist circumference (WC) showed that serum sTfR levels remained positively associated with higher HOMA-IR (p = 0.044). Factorial analysis revealed that the obesity-inflammation related factor (i.e., WC and hsCRP) and adipokine- acute phase protein related factor (i.e., RBP4 and ferritin) showed significant differences between HOMA-IR < 2.5 and HOMA-IR ≥ 2.5. Conclusions: Serum sTfR levels are independently associated with HOMA-IR, whereas higher serum ferritin levels together with higher RBP4 are related to higher HOMA-IR in adolescent girls.

Malondialdehyde as an independent predictor of body mass index in adolescent girls

oxidative stress in relation to obesity that included late
adolescents are scarce and show inconclusive results we
aimed to investigate a wide spectrum of nitro-oxidative
stress biomarkers  i.e., malondialdehyde (MDA), xanthine
oxidase (XO), xanthine oxidoreductase (XOD), xanthine
dehydrogenase (XDH), advanced oxidation protein
products (AOPP) and nitric oxide products (NOx), as well
as an antioxidative enzyme, i.e., catalase (CAT)  in relation
with obesity in the cohort of adolescent girls ages between
16 and 19 years old.
Methods: A total of 59 teenage girls were included in this
cross-sectional study. Binary logistic regression analysis was
performed to examine possible associations between bio-
chemical and nitro-oxidative stress markers and body mass
index (BMI).
Results: There were not significant differences between
oxidative stress markers between normal weight and
overweight/obese girls (i.e., AOPP, XOD, XO, XDH) and
CAT, except for MDA (p<0.001) and NOx (p=0.010)
concentrations which were significantly higher in overweight/obese adolescent girls. Positive associations
were evident between BMI and high sensitivity C-reactive
protein (hsCRP) (OR=2.495), BMI and uric acid
(OR=1.024) and BMI and MDA (OR=1.062).
Multivariable binary regression analysis demonstrated sig-
nificant independent associations of BMI and hsCRP
(OR=2.150) and BMI and MDA (OR=1.105). Even
76.3% of the variation in BMI could be explained with this
Model.
Conclusions: Inflammation (as measured with hsCRP) and
oxidative stress (as determined with MDA) independently
correlated with BMI in teenage girls.
AB  - Uvod: Imajući u vidu činjenicu da je malo studija koje su ispitivale povezanost oksidativnog stresa i gojaznosti kod adolescenata i da iste pokazuju oprečne rezultate, cilj istraživanja je bio da se ispita povezanost širokog spektra biomarkera nitro-oksidativnog stresa tj. malondialdehida (MDA), ksantin oksidaze (XO), ksantin oksidoreduktaze (XOD), ksantin dehidrogenaze (XDH), produkata uznapredovale oksidacije proteina (AOPP) i produkata azot-monoksida (NOx), kao i enzima antioksidativne zaštite, tj. katalaze (CAT) i gojaznosti u kohorti adolescentkinja starosne dobi izme|u 16 i 19 godina. Metode: Ukupno 59 tinejdžerki je uključeno u ovu studiju preseka. AB  - Current clinical data show that, despite constant efforts to develop novel therapies and clinical approaches, atherosclerotic cardiovascular diseases (ASCVD) are still one of the leading causes of death worldwide. Advanced and unstable atherosclerotic plaques most often trigger acute coronary events that can lead to fatal outcomes. However, despite the fact that different plaque phenotypes may require different treatments, current approaches to prognosis, diagnosis, and classification of acute coronary syndrome do not consider the diversity of plaque phenotypes. Long non-coding RNAs (lncRNAs) represent an important class of molecules that are implicated in epigenetic control of numerous cellular processes. Here we review the latest knowledge about lncRNAs’ influence on plaque development and stability through regulation of immune response, lipid metabolism, extracellular matrix remodelling, endothelial cell function, and vascular smooth muscle function, with special emphasis on pro-atherogenic and anti-atherogenic lncRNA functions. In addition, we present current challenges in the research of lncRNAs’ role in atherosclerosis and translation of the findings from animal models to humans. Finally, we present the directions for future lncRNA-oriented research, which may ultimately result in patient-oriented therapeutic strategies for ASCVD.

AB  - Purpose: Obstructive sleep apnea (OSA) is characterised by increased systemic inflammation, and is often accompanied with type 2 diabetes mellitus (T2DM) and cardiovascular disease. The aim of this investigation was to evaluate gene expression of resistin, its receptor CAP1 and CD36 as the indicators of the inflammatory changes in PBMCs in relation to the severity of OSA, and the presence of type 2 diabetes mellitus (T2DM) in OSA. Methods: Severity of OSA was defined by the apnea/hypopnea index (AHI): AHI < 30: mild to moderate OSA (MM-OSA), AHI ≥ 30: severe OSA (S-OSA). Presence of T2DM was captured: OSA with T2DM (OSA + T2DM), OSA without T2DM (OSA-T2DM). PBMC resistin, CAP1, and CD36 mRNA were determined by real-time PCR. Results: Resistin mRNA was significantly upregulated in S-OSA (N = 54) compared to the MM-OSA (N = 52, P = 0.043); CAP1 and CD36 mRNA levels did not differ between the groups (P = 0.302; P = 0.166, respectively). Resistin mRNA was significantly upregulated in OSA + T2DM (N = 29) compared to the OSA-T2DM (N = 77, P = 0.029); CAP1 and CD36 mRNA levels did not differ between the groups (P = 0.662; P = 0.108, respectively). AHI and T2DM were independent predictors of resistin mRNA above the 75th percentile (OR = 3.717 [1.152–11.991]; OR = 3.261 [1.000–10.630], P = 0.042 respectively). Conclusion: Resistin gene upregulation in S-OSA indicates its possible contribution to increased inflammation in S-OSA and makes it a possible marker of the disease severity. Resistin gene upregulation in OSA + T2DM suggests that a joint effect of these two comorbidities may have a major contribution to increased inflammation and complications that arise from this state.

AB  - Globalna pandemija teškim akutnim respiratornim sindromom korona virusom 2 (SARSCoV-2) još uvek traje. Od samog početka pandemije, brza i tačna detekcija SARS-CoV-2 je bila
od suštinskog značaja. Lančana reakcija polimeraze (eng. Polymerase chain reaction, PCR)
sa reverznom transkripcijom (RT) u realnom vremenu (eng. Real-time), rtRT-PCR ostaje zlatni standard za detekciju genoma SARS-CoV-2 u biološkom materijalu. Preporučeni biološki
materijal za rutinsku kliničku praksu je nazofaringealni ili orofaringealni bris. Vreme uzorkovanja za analizu je veoma značajno i treba da se izvrši što je pre moguće od pojave simptoma. rtRT-PCR se može izvesti na uzorcima bez ili sa izolovanom ribonukleinskom kiselinom
(RNK). Ekstrakcija ukupne humane i virusne RNK može se obaviti različitim komercijalnim
kompletima. S druge strane, biološki materijali iz kojih nije izolovana RNK se koriste direktno
u rtRT-PCR nakon inaktivacije virusa i oslobađanja RNK pomoću lizirajućeg pufera. rtRT-PCR
metodologija obično uključuje dva koraka. Prvi korak je RT, a drugi korak je PCR sa detekcijom PCR produkata u realnom vremenu. U većini dijagnostičkih testova, ove dve reakcije se
odvijaju sukcesivno u istoj reakcionoj smeši. Dijagnostički reagensi sadrže oligonukleotide
kojima se amplifikuje jedna humana sekvenca (koja predstavlja internu pozitivnu kontrolu) i
oligonukleotide kojima se amplifikuju od jedne do tri virusne sekvence. Fluorescentni signal
za virusne gene mora da se pojavi pre 40. ciklusa i kriva amplifikacije treba da ima tipičan
S-oblik. Pouzdanost rezultata rtRT-PCR zavisi od mnogih faktora koji uključuju prikupljanje i
obradu uzoraka, metode izolacije RNK, tipove enzima za RT i PCR, vrste proba, izbor instrumenata, metoda analize rezultata i obučenost laboratorijskog osoblja

AB  - In parallel with the rapid growth of obesity, there is also an increase in the prevalence of type 2 diabetes mellitus (T2D) worldwide. Due to its compli- cations, cardiovascular diseases are the leading cause of death in those patients. In the last two decades, special attention has been given to ox- idative stress and inflammation, as the underlying mechanisms related to T2D occurrence and progression. Moreover, micro-ribonucleic acids (miRNAs) as new genetic biomarkers take an important place in the investigation of different metabolic pathways of insulin signaling. In this review article, we discuss microRNA modulation with oxidative stress and inflammation in pa- tients with T2D. Better insight into the novel potential therapeutic targets for treatment of diabetes and its complications is of utmost importance for public health.

AB  - Due to the increased prevalence of obesity-related complications, especially type 2 diabetes
mellitus (DM2), great attention has been paid to the variety of adipokines and cytokines, i.e.
biomarkers, which are secreted by the enlarged visceral adipose tissue. Those pro-inflammatory
mediators diminish insulin sensitivity via impairing insulin signalling pathways. If these
processes persist, insulin resistance and DM2 occur, with a typical finding of low-grade chronic
inflammation. The current review focuses on some hematological indices that reflect the
inflammation level, such as white blood cell count, its subsets, and derived indexes. Those
parameters are easy to measure, cost-effective, and widely available in a primary care setting. A
deeper knowledge of the changes in the mentioned hematological parameters in DM2, in addition
to glucoregulation biomarkers, may enable physicians to act promptly when patients with this
metabolic disorder are concerned.
AB  - Zbog povećane prevalence komplikacija povezanih sa gojaznošću, naročito diabetes mellitus-a tip (DM2), posebna pažnja se posvećuje različitim adipokinima i citokinima, tj. biomarkerima koje luči uvećano visceralno masno tkivo. Pomenuti medijatori inflamacije smanjuju insulinsku senzitivnost vršeći uticaj na signalne puteve insulina. Ukoliko ovi procesi potraju duže dolazi do pojave insulinske rezistencije i DM2, sa tipičnim nalazom hronične inflamacije niskog stepena. Ovaj revijalni članak akcenat stavlja na određene hematološke parametre koji odražavaju stepen inflamacije, kao što je broj leukocita, njihove podvrste, kao i indeksi izvedeni iz istih. Ovi parametri su lako merljivi, ekonomski isplativi i široko dostupni u ambulantama primarne zdravstvene zaštite. Bolje poznavanje promena navedenih hematoloških parametara u DM2, pored biomarkera glikoregulacije, može omogućiti lekarima pravovremenu reakciju kada se radi o pacijentima sa ovim metaboličkim poremećajem.

Novel leukocyte and thrombocyte indexes in patients with prediabetes and type 2 diabetes mellitus

AB  - OBJECTIVE: To our knowledge, there are no studies that investigated the relationship between diabetes mellitus type 2 (T2D) and some novel indexes, such as monocyte/granulocyte to lymphocyte ratio (M/GLR), derived neutrophil to lymphocyte ratio (dNLR), and platelets to neutrophil ratio (PNR). The aim of this study was to examine the association between these novel indexes and glycated hemoglobin (HbA1c) in patients with prediabetes and T2D. PATIENTS AND METHODS: A total of 827 participants were consecutively recruited. According to the American Diabetes Association (ADA) criteria, participants were divided into control, prediabetes, and T2D group. RESULTS: White blood cell count (WBC), neutrophil count, NLR, dNLR, and M/GLR were higher in T2D patients than in the other two groups, whereas PNR was the lowest in T2D group. Lymphocyte count was higher in prediabetes and T2D patients than in control group. Multivariable ordinal regression analysis showed that WBC, neutrophil count, lymphocyte count, NLR, dNLR, and M/GLR were positively associated [OR (95% CI) 1.287 (1.191-1.390), p<0.001; 1.427 (1.275-1.594), p<0.001; 1.347 (1.130- 1.606), p=0.001; 1.350 (1.090-1.670), p=0.006; 1.662 (1.189-2.326), p=0.003; 1.275 (1.057-1.540), p=0.012, respectively] with HbA1c. However, PNR was negatively associated with HbA1c [0.987 (0.981-0.993), p<0.001]. CONCLUSIONS: Novel, modified NLR indexes, such as dNLR and M/GLR were independently correlated with HbA1c. Also, PNR showed superiority over platelets (PLT) in relation to HbA1c. These novel indexes might give a significant contribution to the timely recognition of disturbances of glucose homeostasis in patients with prediabetes and overt diabetes.

abstract = "OBJECTIVE: To our knowledge, there are no studies that investigated the relationship between diabetes mellitus type 2 (T2D) and some novel indexes, such as monocyte/granulocyte to lymphocyte ratio (M/GLR), derived neutrophil to lymphocyte ratio (dNLR), and platelets to neutrophil ratio (PNR). The aim of this study was to examine the association between these novel indexes and glycated hemoglobin (HbA1c) in patients with prediabetes and T2D. PATIENTS AND METHODS: A total of 827 participants were consecutively recruited. According to the American Diabetes Association (ADA) criteria, participants were divided into control, prediabetes, and T2D group. RESULTS: White blood cell count (WBC), neutrophil count, NLR, dNLR, and M/GLR were higher in T2D patients than in the other two groups, whereas PNR was the lowest in T2D group. Lymphocyte count was higher in prediabetes and T2D patients than in control group. Multivariable ordinal regression analysis showed that WBC, neutrophil count, lymphocyte count, NLR, dNLR, and M/GLR were positively associated [OR (95% CI) 1.287 (1.191-1.390), p<0.001; 1.427 (1.275-1.594), p<0.001; 1.347 (1.130- 1.606), p=0.001; 1.350 (1.090-1.670), p=0.006; 1.662 (1.189-2.326), p=0.003; 1.275 (1.057-1.540), p=0.012, respectively] with HbA1c. However, PNR was negatively associated with HbA1c [0.987 (0.981-0.993), p<0.001]. CONCLUSIONS: Novel, modified NLR indexes, such as dNLR and M/GLR were independently correlated with HbA1c. Also, PNR showed superiority over platelets (PLT) in relation to HbA1c. These novel indexes might give a significant contribution to the timely recognition of disturbances of glucose homeostasis in patients with prediabetes and overt diabetes.",

used to explore mRNA fingerprints of different signaling pathways best associated with
ADIPOR1/ADIPOR2 gene expression levels. Transcriptomic datasets GSE44076 (1) and
GSE47756 (2) were downloaded from the NCBI Gene Expression Omnibus database. In the
GSE44076 dataset, three groups were formed: 98 colon tumor tissue and matched tumor-
adjacent mucosa samples and tissue samples from 50 healthy volunteers. The other set
(GSE47756) contained information on PBMs' gene signature in CRC, by forming two groups:
38 samples from healthy subjects and 55 CRC patients. GSEA analysis of the GSE44076
dataset implied that ADIPOR1 mRNA levels were in negative association with MTORC1 and
TNF-α NF-κB signaling pathways in tumor tissue. At the same time, ADIPOR2 was positively
associated with metabolic gene sets such as cholesterol homeostasis, glycolysis and PPAR
signaling. Quite opposite to the GSE44076 dataset, GSEA analysisis of GSE47756 revealed
that ADIPOR1 was a metabolically active receptor in PBMs of CRC patients. Surprisingly, the
TNF-α NF-κB signaling pathway gene sets were positively associated with ADIPOR1 mRNA
levels in monocytes of the cancer group. Different types of metabolic and immune regulation
achieved through ADIPOR1/ADIPOR2 in tumor and PBMs suggest possible novel therapeutic
targets in CRC.

Različite vrste metaboličke i imune regulacije koje se postižu preko ADIPOR1/ADIPOR2 u
tumoru i PBM sugerišu moguć e nove terapeutske ciljeve u CRC.

AB  - Adiponectin (ADIPOQ) as both a regulator of metabolic homeostasis and a protein involved in immune response might be of particular interest to contemporary laboratory medicine, especially in terms of minimally invasive diagnostics. The diverse roles of ADIPOQ with regard to the immune and metabolic aspects of colorectal carcinogenesis have been proposed. However, the expression of its receptors ADIPOR1 and ADIPOR2 is scarcely explored in peripheral blood mononuclear cells (PBMCs). Moreover, ADIPORs’ relationships with the immune response mediator TNF-α have not been previously investigated in the PBMCs of CRC patients. This study used both in silico and observational case–control analyses with the aim of exploring the association of ADIPOR gene expression and ADIPOQ single nucleotide polymorphisms (SNPs) with the inflammatory marker TNF-α and lipid status parameters in patients with CRC. Publicly available transcriptomic datasets (GSE47756, GSE44076) obtained from analyses of monocytes and CRC tissue samples were employed for the in silico evaluation of ADIPORs’ specific genetic traits. GSE47756 and GSE44076 datasets were processed with GSEA software to provide a genetic fingertip of different signaling pathways associated with ADIPORs’ mRNA levels. The case–control aspect of the study included the PBMC samples of 73 patients diagnosed with CRC and 80 healthy volunteers. The PCR method was carried out for the PBMC gene expression analysis (ADIPOR1, ADIPOR2, TNF-α mRNA levels) and for the subjects’ genotyping (ADIPOQ rs266729, ADIPOR1 rs7539542). GSEA showed significant associations of ADIPOR mRNA expression with gene sets related to metabolic and immune homeostasis in both datasets. The case–control study revealed the association of ADIPOR1 rs7539542 with reduced lipid status parameters in CRC. In addition, PBMC ADIPOR1 mRNA levels decreased in CRC (p < 0.001), whereas ADIPOR2 mRNA did not differ between the groups (p = 0.442). A reduction in PBMC TNF-α mRNA levels was noted in CRC (p < 0.05). Our results indicate that ADIPOR1 and ADIPOR2 play a significant role in the alteration of both metabolic and immune homeostasis during the progression of CRC. For the first time, ADIPOR1 is shown to be a specific receptor for mediating ADIPOQ’s effects in the PBMCs of CRC patients.

AB  - Objectives: Obstructive sleep apnea (OSA) is a common condition closely related to obesity, insulin resistance, dyslipidemia, and cardiovascular disease. The aim of this study was to explore the possible relationship between OSA and proprotein convertase subtilisin/kexin type 9 (PCSK9). Methods: Full-night polysomnography was performed on 150 participants who were divided into three groups: controls, OSA patients on statin therapy, and OSA patients not on statin therapy. Biochemical markers, plasma low-density lipoprotein (LDL) and high-density lipoprotein (HDL) subclasses, and PCSK9 were determined. Results: PCSK9 was highest in OSA patients on statins compared to the control group and to OSA patients not on statins (p = 0.036 and p = 0.039, respectively), after adjustment for body mass index (BMI). LDL diameter was greater in OSA patients not on statins compared to OSA patients on statins (p = 0.032). PCSK9 was highest in the group of patients with all three risk factors (diagnosed OSA, statins, BMI ≥25 kg/m2) compared to groups with no, one, and two risk factors (p = 0.031, p = 0.001, and p = 0.029, respectively). Presence of OSA, statin therapy, and BMI ≥25 kg/m2 when combined were independently associated with higher levels of PCSK9 when adjusted for antihypertensive therapy, small dense LDL, and HDL 3c subclass (odds ratio = 2.849; interquartile range [1.026–7.912], p = 0.044). Conclusion: Statin therapy was closely related to PCSK9. OSA along with obesity and statin use induces elevation of PCSK9.

AB  - Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disease,
present in up to 30% of the adult population worldwide (1). Triglycerides accumulation in
hepatocytes (steatosis) represents the root cause of NAFLD and is associated with oxidative
stress, which could further lead to fibrosis and cell death of hepatocytes (2). The aim of this
research was to identify redox status markers for predicting the risk of developing steatosis.
158 participants were included. Steatosis was confirmed by ultrasound in 101 subjects,
while the remaining 57 were in the control group. The following markers of redox status
were determined in serum and plasma samples of all subjects: superoxide dismutase 1
(SOD1), paraoxonase (PON1), malondialdehyde (MDA) and superoxide anion (O2.-). For this
purpose, spectrophotometric methods and enzyme immunosorbent assays were used. SOD1
was statistically significantly higher (P<0.001), while O2 .- was significantly lower in the
patient group (P<0.001). SOD1 was significantly negatively correlated with O2.- (ρ= -0.494,
P<0.001) and MDA (ρ= -0.242, P=0.002). Univariate binary logistic regression analysis
showed a positive association between SOD1 and the presence of steatosis (OR=1.018, 95%
CI 1.005-1.031; P=0.005), as well as a negative association between O2 .- and the presence of
steatosis (OR=0.959, 95% CI 0.941-0.978; P<0.001). Multivariate analysis singled out SOD1
(OR=1.024, 95% CI 1.006-1.041; P=0.007) and O2.- (OR=0.965, 95% CI 0.942-0.989;
P=0.004) as independent predictors for the presence of steatosis in our subjects. The redox
status parameters, SOD1 and O2, respectively, showed a positive and negative prediction of
the presence of statosis in our subjects.
AB  - Nealkoholna masna bolest jetre (eng. nonalcoholic fatty liver disease, NAFLD) je
najčešće hronično oboljenje jetre, prisutno i u do 30% adultne populacije širom sveta (1).
Značajnu ulogu u nastanku NAFLD ima akumulacija triglicerida u hepatocitima – steatoza,
koja je povezana sa oksidativnim stresom, a koji dalje vodi fibrozi i ćelijskoj smrti hepatocita
(2). Cilj ovog istraživanja bio je identifikacija markera redoks statusa za predviđanje rizika za
nastanak steatoze. Studija je obuhvatila 158 ispitanika iz Kliničko bolničkog centra Zemun.
Steatoza je potvrđena ultrazvukom kod 101 ispitanika, dok je preostalih 57 činilo kontrolnu
grupu. U uzorcima seruma i plazme svih ispitanika određeni su sledeći markeri redoks
statusa: superoksid dismutaza 1 (SOD1), paraoksonaza (PON1), malondialdehid (MDA) i
superoksidni anjon (O2.-). U tu svrhu korišćene su spektrofotometrijske metode i enzimski
imunosorbentni testovi. SOD1 je bila statistički značajno viša (P<0,001), dok O 2.- značajno
niži u grupi pacijenata (P<0,001), dok se PON1 i MDA nisu značajno razlikovali između

negativnu asocijaciju između O 2.- i prisustva steatoze (OR=0,959, 95% CI 0,941-0,978;
P<0,001). Multivarijantna analiza je izdvojila SOD1 (OR=1,024, 95% CI 1,006-1,041;
P=0,007) i O2.- (OR=0,965, 95% CI 0,942-0,989; P=0,004) kao nezavisne prediktore za
prisustvo steatoze kod naših ispitanika. Parametri redoks statusa, SOD1 i O 2.- redom, su
pokazali pozitivnu, odnosno negativnu predikciju prisustva statoze kod naših ispitanika.

AB  - Background and aims: Obstructive sleep apnea (OSA) is closely linked to obesity and related adverse metabolic changes, including dyslipidemia. However, it is not clear whether OSA is an independent contributing factor to dyslipidemia, or the observed association is a reflection of a concomitant presence of obesity. Additionally, dyslipidemia is usually evaluated through measurement of parameters of routine lipid status, while more precise evaluation of lipid homeostasis is rarely performed in OSA. In this study, we analyzed markers of cholesterol synthesis and absorption in patients with OSA with respect to the presence of obesity and the disease severity. Methods and results: This study enrolled 116 OSA patients. Concentrations of non-cholesterol sterols (NCS), measured by LC-MS/MS, were used as markers of cholesterol synthesis and absorption. Apnea-hypopnea index (AHI) and oxygen saturation (SaO2) were utilized as markers of OSA severity. Serum lipid status parameters were determined by routine enzymatic methods. Markers of cholesterol synthesis were increased (P = 0.005), whilst markers of cholesterol absorption decreased (P = 0.001) in obese OSA patients. Cholesterol synthesis/absorption ratio was elevated in obese subjects (P < 0.001). Concentration of cholesterol synthesis marker lathosterol was significantly higher in subjects with severe OSA (P = 0.014) and we observed a trend of decreased cholesterol absorption in these patients. AHI was revealed as an independent determinant of lathosterol concentration (P = 0.022). Conclusions: Our results suggest that the presence of obesity and severe forms of OSA is characterized by elevated endogenous cholesterol synthesis. AHI was singled out as an independent determinant of the serum level of cholesterol synthesis marker lathosterol.

AB  - Background: Type 1 diabetes mellitus (T1DM) is one of the
most common endocrine diseases in children. T-cell autore-
activity toward b-cells is controlled by significant changes in
metabolism of T cells. Mammalian target of rapamycin
(mTOR) is an important intracellular regulator of metabolism
and cell growth. MAPK/MAK/MRK overlapping kinase 1
(MOK1) is one of the less known regulators of mTOR. We
sought to investigate if MOK1 and mTOR mRNA levels in
peripheral blood mononuclear cells (PBMCs) of T1DM pedi-
atric patients are different compared to healthy subjects.
Methods: This study included 172 adolescents with T1DM
and 36 healthy adolescent volunteers designated for control
group (CG). MOK1 and mTOR mRNA levels were deter-
mined in PBMCs by qPCR.
Results: T1DM patients have significant downregulation of
MOK1 mRNA levels in PBMCs compared CG (P=0.018),
while there was no significant difference in mTOR mRNA
levels (P=0.891). Furthermore, in T1DM patients, MOK1
significantly correlated with age, triglycerides and mTOR,
while mTOR correlated significantly with BMI and systolic
blood pressure. Overweight T1DM subjects had significantly
lower MOK1 (P=0.034) and mTOR (P=0.017) mRNA
levels, together with significantly higher levels of systolic
blood pressure (P<0.001), total cholesterol (P=0.001),
LDL-cholesterol (P=0.001) and CRP (P<0.001). Multi-
variate analysis showed that MOK1 was independently
negatively associated with T1DM when adjusted for sex, age, HDL-C and CRP (OR=0.417 (95%CI: 0.175–0.997),
p=0.049).
Conclusions: Our study demonstrated for the first time that
T1DM is associated with MOK1 downregulation. In addition,
downregulation of both mTOR and MOK1 gene expressions
was associated with cardiovascular risk factors in overweight
T1DM patients.
AB  - Uvod: Dijabetes melitus tip 1 (T1DM) jedno je od najčešćih endokrinih oboljenja kod dece. Autoreaktivnost T-ćelija prema b-ćelijama kontroliše se značajnim promenama u metabolizmu T ćelija. Cilj delovanja rapamicina kod sisara je važan unutarćelijski regulator metabolizma i rasta ćelija. Zaključak: Naša studija je prva koja je pokazala da je T1DM udružen sa nishodnom regulacijom MOK1. Pored toga, snižena regulacija ekspresije gena mTOR i MOK1 bila je povezana sa kardiovaskularnim faktorima rizika kod pacije nata sa T1DM sa prekomernom težinom.

Circulating non-coding RNAs as biomarkers in coronary artery disease

AB  - Coronary artery disease (CAD) is a leading cause of mortality worldwide. Atherosclerosis
involves an interplay of different pathological mechanisms, such as progressive inflammation,
abnormal lipid metabolism, and oxidative stress, and as such represents the basic pathological
phenomenon underlying CAD. Atherosclerotic plaque narrows the lumen of coronary arteries,
creating an ischemic environment for the heart muscle, which finally leads to clinical
complications, such as acute myocardial infarction. Currently, there are no biomarkers that could
predict plaque stability or major adverse cardiovascular events (MACE). Numerous functional
non-coding RNA (ncRNA) species influence basic cellular functions, and as such play a role in
the development and progression of CAD. Of these ncRNAs, micro RNAs (miRNAs) and long
non-coding RNAs (lncRNAs) are the most investigated. Considering that ncRNAs detected in
extracellular fluids can originate from different cells, circulating ncRNAs are being intensively
investigated as potential biomarkers in the diagnosis and prognosis of CAD. In the following
paper, we provide current insights into potential molecular mechanisms by which miRNAs and
lncRNAs contribute to the pathology of CAD and discuss their potential role as biomarkers in
diagnosis and prognosis of disease.
AB  - Koronarna arterijska bolest (KAB) predstavlja vodeći uzrok smrtnosti širom sveta. Osnovni patološki proces koji karakteriše KAB je ateroskleroza, koju odlikuju različiti patofiziološki mehanizmi kao što su progresivna inflamacija, poremećen metabolizam lipida, oksidativni stres, itd. Aterosklerotski plak sužava lumen koronarnih arterija, što dovodi do nastanka ishemijskih promena na srčanom mišiću, posledično dovodeći do kliničkih komplikacija ove bolesti, kao što je akutni infarkt miokarda. Trenutno ne postoje biomarkeri koji bi mogli da predvide sudbinu aterosklerotskog plaka, kao ni razvoj akutnih koronarnih događaja ili razvoj velikih uznapredovalih kardiovaskularnih događaja. Mnogobrojne vrste nekodirajućih RNK molekula utiču na osnovne ćelijske funkcije i kao takve igraju ulogu u nastanku i progresiji KAB. Do sada su od svih vrsta nekodirajućih RNK-a najviše istražene mikro RNK-a (miRNK) i dugolančane nekodirajuće RNK-a (dnkRNK). Uzimajući u obzir da nekodirajuće RNK-a dospevaju u ekstracelularnu tečnost iz različitih ćelija, trenutno se veliki napori ulažu u ispitivanje mogućnosti upotrebe cirkulišuih nekodirajućih RNK-a kao potencijalnih biomarkera u dijagnostici i prognostici KAB. U ovom naučno-istraživačkom članku sažećemo trenutna saznanja o molekularnim mehanizmima preko kojih miRNK i dnkRNK doprinose razvoju KAB, kao i o potencijalnoj primeni ovih molekula kao budućih biomarkera u dijagnozi i prognozi ove bolesti.

The role of microRNAs in oxidative stress regulation

AB  - MicroRNAs (miRNAs) are small, 22-24 nucleotides long, noncoding RNAs that act as
pivotal posttranscriptional regulators in various biological processes. Interaction of miRNAs
with their target RNAs in most cases leads to the suppression of gene expression, by
promoting degradation of RNAs or inhibiting translation. These interactions mainly occur
with 3′ untranslated regions (UTR), but can also occur with 5′ UTR or with the coding part of
the target RNA molecule. It is estimated that miRNAs regulate more than 30–60% of protein
coding genes in the human genome. Oxidative stress refers to an imbalance between reactive
oxygen species (ROS) generation and body's capability to detoxify the reactive mediators or
to fix the relating damage. ROS can regulate miRNA transcription, maturation, and function.
ROS directly modulate the activity of vital proteins that control posttranscriptional events in
the biogenesis of miRNAs (Di George critical region-8 protein; Dicer). A certain group of
transcription factors involved in the regulation of miRNA expression is upregulated under
oxidative stress and directly activates the transcription of a subset of miRNAs. ROS have
been directly implicated in epigenetic alternations such as DNA methylation and histone
modifications that control specific microRNA transcription (1). On the other hand, miRNAs
may in turn modulate the redox signalling pathways, altering their integrity, stability, and
functionality, thus contributing to the pathogenesis of multiple diseases: cancer,
neurodegenerative diseases, diabetes mellitus (2), ROS-related cardiac diseases, including
myocardial infarction, ischemia/reperfusion injury, cardiac hypertrophy and heart failure,
and are also considered as potential therapeutic targets and novel diagnostic tools.
AB  - MikroRNK (miRNK) predstavljaju male, nekodirajuće RNK, dužine 22-24 nukleotida,
koje regulišu ekspresiju gena na post-transkripcionom nivou. Interakcija miRNK sa ciljnim
informacionim RNK (iRNK) u većini slučajeva dovodi do supresije ekspresije gena,
degradacijom ciljne iRNK ili inhibicijom translacije. Ove interakcije se dešavaju na
nekodirajućim 3’krajevima iRNK, ali mogu da se dese i na 5’ kraju ili čak na kodirajućem
regionu ciljne iRNK. Smatra se da na ovaj način, u humanom genomu, miRNK kontrolišu oko
30-60% gena koji kodiraju proteine. Oksidativni stres je stanje u kome postoji neravnoteža
između stvaranja reaktivnih vrsta kiseonika (slobodni radikali, ROS) i njihovog neutralisanja
od strane antioksidativne zaštite organizma. Pokazano je da ROS mogu da utiču na procese
transkripcije i sazrevanja miRNK kao i na njihovu funkciju. ROS direktno modulišu aktivnost
glavnih proteina koji učestvuju u post-transkripcionoj obradi u procesu biogeneze miRNK
(Di George critical region-8 protein i Dicer). Pod uticajem oksidativnog stresa može se
aktivirati transkripcija određenih familija miRNK, dejstvom na transkripcione faktore koji
učetvuju u tom procesu. Takođe, ROS utiču na epigenetske mehanizme regulacije ekspresije
miRNK, kao što su DNK metilacija i modifikacija histona (1). Sa druge strane, miRNK mogu da
utiču na aktivnost redoks signalnih puteva, menjajući njihovo funkcionisanje. Na taj način
miRNK doprinose patogenezi mnogih bolesti kao što su: različite vrste kancera,
neurodegenerativne bolesti, dijabetes melitus (2), bolesti srca, uključujući infarkt miokarda,
povredu nastalu ishemijom/reperfuzijom, srčanu hipertrofiju i srčanu insuficijenciju.
Takođe, miRNK se razmatraju kao potencijalni dijagnostički markeri i novi terapijski ciljevi.

oxidative stress and directly activates the transcription of a subset of miRNAs. ROS have
been directly implicated in epigenetic alternations such as DNA methylation and histone
modifications that control specific microRNA transcription (1). On the other hand, miRNAs
may in turn modulate the redox signalling pathways, altering their integrity, stability, and
functionality, thus contributing to the pathogenesis of multiple diseases: cancer,
neurodegenerative diseases, diabetes mellitus (2), ROS-related cardiac diseases, including
myocardial infarction, ischemia/reperfusion injury, cardiac hypertrophy and heart failure,
and are also considered as potential therapeutic targets and novel diagnostic tools., MikroRNK (miRNK) predstavljaju male, nekodirajuće RNK, dužine 22-24 nukleotida,
koje regulišu ekspresiju gena na post-transkripcionom nivou. Interakcija miRNK sa ciljnim
informacionim RNK (iRNK) u većini slučajeva dovodi do supresije ekspresije gena,
degradacijom ciljne iRNK ili inhibicijom translacije. Ove interakcije se dešavaju na
nekodirajućim 3’krajevima iRNK, ali mogu da se dese i na 5’ kraju ili čak na kodirajućem
regionu ciljne iRNK. Smatra se da na ovaj način, u humanom genomu, miRNK kontrolišu oko
30-60% gena koji kodiraju proteine. Oksidativni stres je stanje u kome postoji neravnoteža
između stvaranja reaktivnih vrsta kiseonika (slobodni radikali, ROS) i njihovog neutralisanja
od strane antioksidativne zaštite organizma. Pokazano je da ROS mogu da utiču na procese
transkripcije i sazrevanja miRNK kao i na njihovu funkciju. ROS direktno modulišu aktivnost
glavnih proteina koji učestvuju u post-transkripcionoj obradi u procesu biogeneze miRNK
(Di George critical region-8 protein i Dicer). Pod uticajem oksidativnog stresa može se
aktivirati transkripcija određenih familija miRNK, dejstvom na transkripcione faktore koji
učetvuju u tom procesu. Takođe, ROS utiču na epigenetske mehanizme regulacije ekspresije
miRNK, kao što su DNK metilacija i modifikacija histona (1). Sa druge strane, miRNK mogu da
utiču na aktivnost redoks signalnih puteva, menjajući njihovo funkcionisanje. Na taj način
miRNK doprinose patogenezi mnogih bolesti kao što su: različite vrste kancera,
neurodegenerativne bolesti, dijabetes melitus (2), bolesti srca, uključujući infarkt miokarda,
povredu nastalu ishemijom/reperfuzijom, srčanu hipertrofiju i srčanu insuficijenciju.
Takođe, miRNK se razmatraju kao potencijalni dijagnostički markeri i novi terapijski ciljevi.",

AB  - Various studies have reported contradictory results regarding the relationship of total bilirubin and high‑sensitivity C‑reactive protein levels (hsCRP) with diabetes mellitus Type 2 (DM2). Therefore, we aimed to examine which one of them could be more convenient for the estimation of DM2 risk in postmenopausal women. Materials and Methods: A total of 150 healthy postmenopausal women (mean age 57[53–60] years) and 79 postmenopausal women with DM2 (mean age 66 [61–71] years) were enrolled in cross‑sectional study. Examinees were recruited consecutively in the study during their regular check‑up visit in the Primary Health Care Center in Podgorica, Montenegro, in a period from October 2012 to May 2016. Anthropometric measurements, biochemical parameters, and blood pressure were obtained. Multivariable logistic regression analysis was used to find the independent predictors for DM2 development in postmenopausal women. Results: Age, waist circumference, and total bilirubin were the independent predictors for DM2 development in postmenopausal women (odds ratio [OR] =1.224, 95% confidence interval [CI] [1.117–1.341], P < 0.001; OR = 1.137, [95% CI = 1.036–1.215], P < 0.001, and OR = 0.727, [95% CI = 0.611–0.866], P < 0.001, respectively), whereas hsCRP lost its independent predictive role (OR = 1.155, [95% CI = 0.854–1.560], P = 0.349). Conclusion: Unlike hsCRP, total bilirubin independently correlated with DM2 in postmenopausal women.

Oxidative stress and cardiometabolic biomarkers in patients with non-alcoholic fatty liver disease

AB  - Oxidative stress is assumed to be the underlying feature of non-alcoholic fatty liver disease (NAFLD). To our knowledge, the mutual involvement of redox status homeostasis parameters [i.e., advanced oxidation protein products (AOPP), pro-oxidant-antioxidant balance (PAB), total oxidant status (TOS), total antioxidant status (TAS) and oxidative-stress index (OSI)] and cardiometabolic biomarkers in subjects with NAFLD has not been examined yet. Accordingly, we aimed to investigate this potential relationship. A total of 122 subjects with NAFLD were compared with 56 participants without NAFLD. The diagnosis of NAFLD was confirmed by abdominal ultrasound. Anthropometric and biochemical parameters were measured. OSI, Castelli’s Risk Index I (CRI-I) and Castelli’s Risk Index II (CRI-II) were calculated. Univariate and multivariate binary logistic regression analysis were used to test the predictions of oxidative stress and cardiometabolic markers, respectively for NAFLD. Principal component analysis (PCA) was applied to explore its mutual effect on NAFLD status. Significant positive associations of CRI-I, CRI-II, high sensitivity C-reactive protein (hsCRP) and AOPP with NAFLD were found. PCA analysis extracted 3 significant factors: Oxidative stress-cardiometabolic related factor (i.e., triglycerides, AOPP, HDL-c and HbA1c)-explained 36% of variance; Pro-oxidants related factor (i.e., TOS and PAB)-explained 17% of variance; and Antioxidants related factor (i.e., TAS)- explained 15% of variance of the tested parameters. Moreover, binary logistic regression analysis revealed significant predictive ability of Oxidative stress-cardiometabolic related factor (p < 0.001) and Pro-oxidants related factor (p < 0.05) for NAFLD status. In addition to oxidative stress (i.e., determined by higher AOPP levels), dyslipidemia (i.e., determined by higher lipid indexes: CRI-I and CRI-II) and inflammation (determined by higher hsCRP) are independently related to NAFLD status. The mutual involvement of pro-oxidants (i.e., TOS and PAB), or the joint involvement of pro-oxidants (i.e., AOPP) and cardiometabolic parameters (i.e., HbA1c, triglycerides and HDL-c) can differentiate subjects with NAFLD from those individuals without this metabolic disorder. New studies are needed to validate our results in order to find the best therapeutic approach for NAFLD.

AB  - OBJECTIVE: No studies investigated total leukocytes, their subpopulations and novel indexes based on different ratios of leukocyte subsets concerning cardiovascular risk (CV) risk in late adolescents. Therefore, the aim of the present study was to explore such potential relationships. PATIENTS AND METHODS: A total of 156 adolescents were included. CV risk score was calculated by summarizing each risk factor (i.e., female sex, low high density lipoprotein cholesterol (HDL-c), high non-HDL-c, smoking, blood pressure, and fasting glycemia). Adolescents were divided into a low CV risk score (i.e., -2≤ CV risk score ≤1) and moderate/ higher CV risk score (i.e., CV risk score ≥ 2). White blood cell count (WBC) and its subsets were analyzed on an automatic device. The indexes were calculated. RESULTS: Total and differential WBC counts except basophil count were higher in moderate/higher CV risk participants. Multivariate binary regression analysis showed that total WBC count independently increased CV risk score by 1.623 times (p=0.001). Neutrophil and eosinophil counts (p=0.027 and p=0.010, respectively) were independently able to increase CV risk score by 1.486 and 1.556 times, respectively. On the contrary, indexes were not independently correlated with CV risk. CONCLUSIONS: WBC, neutrophil, and eosinophil count are the independent predictors of increased CV risk in adolescents. The associations may indicate the different pathways that lead to CV disease in adulthood.

Taking  into  consideration  thatobesity  is  an  independent  risk  factor  for  many  cardio  -metabolic  diseases,  we  aimed  to  explore  the  relationshipbetween endocan levels and novel anthropometric indices i.e.,  body  adiposity  index  (BAI),  cardiometabolic  index(CMI), a body shape index, body roundness index, conicityindex,  lipid  accumulation  product  index  and  visceraladiposity  index and  traditional  ones   i.e.,  waist  circum  -ference,  hip  circumference,  body  mass  index,  waist-to-height ratio and waist-to-hip ratio in adult population.Methods:A  total  of  177  participants  were  included.Anthropometric indices and biochemical parametres weremeasured. Results:Univariate  regression  analysis  demonstrated  posi-tive correlations of endocan and almost all anthropometricdata. To explore independent associations of endocan andanthropometric parameters, the Model which fulfilled crite-ria  for  ordinal  regression  testing  was  created.  Adjusted odds  for  BAI  given  in  the  Model  (OR=1.120,  95%  CI1.036–1.212, P=0.004), demonstrated that a rise in BAIby 1 unit increased the probability of higher endocan con-centration  by  12%.  As  well,  a  rise  in  CMI  for  1  unit,increased the probability for higher endocan levels for 2.6times  (OR=2.599,  95%  CI  1.006–6.712,  P=0.049).  Atotal  of  20.1%  of  variation  in  endocan  levels  could  beexplained by this Model.Conclusions:Non-traditional obesity indices, BAI and CMIindependently correlated with higher serum endocan levelsin adult population.
AB  - Uvod: Povezanost endokana sa netradicionalnim antropometrijskim parametrima, kao različitim faktorima rizika za kardiovaskularne bolesti, nije ispitivana u prethodnim studijama. Endokan je novi biomarker inflamacije, čije su veće vrednosti zabeležene kod kardiometaboličkih poremećaja. Znajući da je gojaznost nezavisan faktor rizika za mnoga kardiometabolička oboljenja, cilj je bio da se ispita povezanost endokana i novih antropometrijskih pokazatelja tj. indeks telesne gojaznosti (BAI), kardiometabolički indeks (CMI), indeks oblika tela, indeks zaokruženosti tela, indeks koniciteta, indeks produkata lipidne akumulacije i indeks visceralne gojaznosti i onih tradicionalnih tj. obim struka, obim kukova, indeks telesne mase, odnos obim struka/telesna visina, odnos obim struka/obim kukova u populaciji odraslih. Metode: Ukupno 177 ispitanika je učestvovalo u istraživanju. Mereni su antropometrijski i biohemijski parametri. Rezultati: Univarijantna regresiona analiza je pokazala pozitivnu korelaciju endokana i skoro svih ispitivanih antropometrijskih parametara. U cilju daljeg ispitivanja postojanja nezavisnih korelacija endokana i antropometrijskih parametara, kreiran je Model koji je zadovoljio kriterijume za ordinalnu regresiju. Prilagođeni Odds-ovi za BAI u Modelu (OR=1,120, 95% CI 1,036-1,212, P=0,004), su pokazali da je porast BAI za 1 jedinicu povećao verovatnoću za porast koncentracije endokana za 12%. Takođe, porast CMI za 1 jedinicu, povećao je verovatnoću za veće vrednosti endokana za 2,6 puta (OR=2,599, 95% CI 1,006-6,712, P=0,049). Ukupno 20,1% varijabiliteta u vrednostima koncentracije endokana može biti objašnjeno ovim Modelom. Zaključak: Netradicionalni pokazatelji gojaznosti, BAI i CMI su nezavisno povezani sa većim vrednostima endokana u populaciji odraslih.
