AB - Endocrine disruptors, which include, but are not limited to, toxic metals such as Cd, have been linked to the increasing incidence of infertility and reproductive health disorders. As one of the most important mechanisms of Cd toxicity is its interaction with bioelements, further investigation of this mechanism could contribute to a better understanding of mechanisms of endocrine-disrupting Cd activities in testes and its role in human reproductive health.

AB - Purpose: The goal of the study was to examine clozapine (CLZ) and norclozapine (NCLZ) therapeutic drug monitoring (TDM) data and associated sources of pharmacokinetic variability, particularly the impact of valproic acid (VPA) use. Methods: This study included 126 patients with psychiatric disorders on mono- or co-therapy with CLZ. Patients’ data during routine TDM were collected retrospectively from clinical records. The descriptive and statistical analysis was computed using IBM SPSS Statistics software (version 22, NY, USA). Multiple linear regression, based on the last observations, was used to assess correlation between demographic characteristics, life habits and co-therapy with dose-corrected serum levels (C/D) of CLZ and NCLZ, as well as CLZ/NCLZ. Results: A total of 295 CLZ concentrations were measured in 126 patients, with a mean of 275.5 ± 174.4 µg/L, while 124 NCLZ concentrations were determined in 74 patients, with a mean of 194.6 ± 149.8 µg/L. A statistically significant effect on ln-transformed CLZ C/D was confirmed for sex and smoking, whereas sex, smoking and VPA therapy were associated with ln-transformed NCLZ C/D. According to the final models, lower values of NCLZ C/D for about 45.9% can be expected in patients receiving VPA. Concomitant use of VPA was the only factor detected to contribute in CLZ/NCLZ variability. Conclusion: The results of this study may help clinicians interpret TDM data and optimize CLZ dosing regimens, especially in patients concomitantly treated with VPA. Our results show that VPA primarily decreases NCLZ levels, while alteration of the parent drug is not statistically significant.

AB - The microelements chromium, cobalt and nickel are necessary for the proper
functioning of the organism. At higher concentrations, they can cause toxic effects (organ
disfunction, genetic damage). They are widespread in the enviroment. They reach our body
through food, water, air and/or skin. The aim of this study was to determine the reference
values of chromium, cobalt and nickel in the blood of adult residents of Belgrade and
determine the degree of influence of individual, socioeconomic factors and lifestyle habits on
blood levels of these metals. The study population consisted of adults, healthy population, 18
to 65 years of age, who were voluntary blood donors. The study included 715 men (72.7%)
and 269 women (27.3%). Blood collected in vacutainers with heparin, was used for analysis.
984 samples were analysed. The metals were determined using inductively coupled plasma
mass spectrometry (7700x, Agilent, USA), with octopol reactive system and micro-flow
nebulizer. The average age of participants was 37.2 10.8. The reference values of the
analysed metals, shown as the 95 percentile, were 0.95µg/L, 0.62µg/L and 1.46µg/L for
chromium, cobalt and nickel, respectively, and were in good correlation with the values
obtained in biomonitoring studies conducted in Europe. This study showed that population
of Serbian origin had significantly lower blood concentrations of chromium, that cobalt blood
levels in women were significantly higher and cobalt and nickel levels increased with age in
both sexes. Education and economic status, cigarette smoking and sports did not significantly
affect the levels of these metals in the blood.
većim koncentracijama mogu ispoljiti toksične efekte poput ometanja pravilne funkcije
organa i dovesti do genetskih oštećenja i karcinogeneze. Široko su rasprostranjeni u životnoj
sredini.
nivoe ovih metala u krvi. Ispitivanu populaciju je činilo odraslo, zdravo stanovništvo, ukupno
715 muškaraca (72,7%) i 269 žena (27,3%) od 18 do 65 godina starosti, koji su bili
dobrovoljni davaoci krvi.
reaktivnim sistemom i „micro‐flow“ raspršivačem. Prosečna starost ispitanika iznosila je
37,210,8 godina. Referentne vrednosti ispitivanih metala u krvi, prikazane kao 95. percentil
su 0,95 µg/L, 0,62 µg/L i 1,46 µg/L za hrom, kobalt i nikl, redom, su u dobroj korelaciji sa
vrednostima dobijenim u biomonitoring studijama sprovedenim u Evropi.
nivoe ovih metala u krvi.

chromium, cobalt and nickel, respectively, and were in good correlation with the values
obtained in biomonitoring studies conducted in Europe. This study showed that population
of Serbian origin had significantly lower blood concentrations of chromium, that cobalt blood
levels in women were significantly higher and cobalt and nickel levels increased with age in
both sexes. Education and economic status, cigarette smoking and sports did not significantly
affect the levels of these metals in the blood., Mikroelementi hrom, kobalt i nikl su potrebni za pravilno funkcionisanje organizma. U
većim koncentracijama mogu ispoljiti toksične efekte poput ometanja pravilne funkcije
organa i dovesti do genetskih oštećenja i karcinogeneze. Široko su rasprostranjeni u životnoj
sredini.
nivoe ovih metala u krvi. Ispitivanu populaciju je činilo odraslo, zdravo stanovništvo, ukupno
715 muškaraca (72,7%) i 269 žena (27,3%) od 18 do 65 godina starosti, koji su bili
dobrovoljni davaoci krvi.
reaktivnim sistemom i „micro‐flow“ raspršivačem. Prosečna starost ispitanika iznosila je
37,210,8 godina. Referentne vrednosti ispitivanih metala u krvi, prikazane kao 95. percentil
su 0,95 µg/L, 0,62 µg/L i 1,46 µg/L za hrom, kobalt i nikl, redom, su u dobroj korelaciji sa
vrednostima dobijenim u biomonitoring studijama sprovedenim u Evropi.
nivoe ovih metala u krvi.",

AB - Introduction: The modern approach to quality control (QC) in medical laboratories implies the development of a risk-based control plan. This paper aims to develop a risk-based QC plan for a laboratory with a small daily testing volume and to integrate the already optimized moving average (MA) control procedures into this plan. Materials and methods: A multistage bracketed QC plan for ten clinical chemistry analytes was made using a Westgard QC frequency calculator. Previously, MA procedures were optimized by the bias detection simulation method. Results: Aspartate aminotransferase, HDL-cholesterol and potassium had patient-risk sigma metrics greater than 6, albumin and cholesterol greater than 5, creatinine, chlorides, calcium and total proteins between 4 and 5, and sodium less than 4. Based on the calculated run sizes and characte- ristics of optimized MA procedures, for 6 tests, it was possible to replace the monitoring QC procedure with an MA procedure. For the remaining 4 tests, it was necessary to keep the monitoring QC procedure and introduce MA control for added security. Conclusion: This study showed that even in a laboratory with a small volume of daily testing, it is possible to make a risk-based QC plan and inte- grate MA control procedures into that plan.

AB - Introduction: Moving average (MA) is one possible way to use patient results for analytical quality control in medical laboratories. The aims of this study were to: (1) implement previously optimized MA procedures for 10 clinical chemistry analytes into the laboratory information system (LIS); (2) monitor their performance as a real-time quality control tool, and (3) define an algorithm for MA alarm management in a small-volume laboratory to suit the specific laboratory. Materials and methods: Moving average alarms were monitored and analysed over a period of 6 months on all patient results (total of 73,059) obtained for 10 clinical chemistry parameters. The optimal MA procedures were selected previously using an already described technique called the bias detection simulation method, considering the ability of bias detection the size of total allowable error as the key parameter for optimization. Results: During 6 months, 17 MA alarms were registered, which is 0.023% of the total number of generated MA values. In 65% of cases, their cause was of pre-analytical origin, in 12% of analytical origin, and in 23% the cause was not found. The highest alarm rate was determined on sodium (0.10%), and the lowest on calcium and chloride. Conclusions: This paper showed that even in a small-volume laboratory, previously optimized MA procedures could be successfully implemented in the LIS and used for continuous quality control. Review of patient results, re-analysis of samples from the stable period, analysis of internal quality control samples and assessment of the analyser malfunctions and maintenance log have been proposed for the algorithm for managing MA alarms.

AB - Traditionally, the internal quality control (QC) of analytical work inmedical laboratories is carried out by testing commercially available control materials atcertain time intervals. However, this type of control has drawbacks, includingintermittency, the problem of commutability, and the cost of materials and labor. Therefore, modern laboratory practice considers the development of risk-based QCplans and the introduction of patient-based real-time quality control (PBRTQC)procedures. Unlike traditional QC, PBRTQC is continuous, free of the problem ofcommutability and the cost of control material. One of the possible ways to use patientresults for the quality control of analytical work is the moving average (MA). MAinvolves calculating the average value from the obtained set of patient results andfurther using that value for control purposes. The frequency of QC measurements in arisk-based QC plan depends on the sigma metrics of the test as a measure of quality.Tests with low values of sigma metrics require control strategies that are complex andexpensive. The laboratory may consider introducing PBRTQC procedures into a routineQC plan in such tests. In the case of laboratories with a small daily testing volume, theissue of developing a risk-based QC plan and implementing MA control proceduresremains insufficiently investigated. Objectives: The objectives of this scientific research were to select and optimize MAprocedures for 10 biochemical analytes in a medical laboratory with a small dailyvolume of testing, to implement optimized MA procedures through the laboratoryinformation system (LIS) in routine laboratory work, to establish a protocol for handlingMA alarms, and to integrate MA procedures into a laboratory risk-based QC plan.

AB - The atypical antipsychotic clozapine (CLZ) is primarily used for the treatment-
resistant schizophrenia. Due to low therapeutic index and high pharmacokinetic variability,
therapeutic drug monitoring (TDM) is highly recommended (1). The aim of this retrospective
study was to analyze TDM data of CLZ and its active metabolite norclozapine (NCLZ) in adult
patient with schizophrenia. The study included CLZ TDM data obtained from 69 patients (22-
67 years) treated at the Clinic for Psychiatry, Clinical Center of Serbia, while NCLZ data were
available from 43 patients. Serum concentrations were determined at the Institute of
Forensic Medicine, Belgrade, Serbia using liquid chromatography with tandem mass
spectrometry (LC‐MS/MS). Statistical analysis was performed by SPSS software® (version
18). The daily doses of CLZ ranged between 37.5 and 600 mg. The mean value of CLZ and
NCLZ levels were 0.285 ± 0.174 mg/L and 0.189 ± 0.132 mg/L, respectively. 73.06% and
26.83% of measured CLZ and NCLZ concentrations were outside reference range (mostly
below), respectively. Significant positive correlation (p<0.05) was observed between daily
dose and CLZ levels, as well as between dose and NCLZ levels. Significant correlations of dose
and CLZ levels were confirmed in males and females, smokers and nonsmokers, separately.
Parent drug and metabolite levels varied 13 and 16-fold in patients receiving 300 mg/day,
respectively. The results indicate considerable variability in CLZ and NCLZ concentrations in
adult patients with schizophrenia, and positive association with dose. Further multivariate
analysis is required to assess, in addition to dose, potential influences of other patient and
co-therapy factors.
Zbog niskog terapijskog indeksa i velike farmakokinetičke
varijabilnosti, terapijsko prać enje lekova (therapeutic drug monitoring, TDM) se preporučuje
(LC‐MS/MS). Statistička analiza je izvršena pomoć u softvera SPSS® (verzija 18). Dnevne doze
CLZ kretale su se između 37,5 i 600 mg. Srednja vrednost je bila 0,285 ± 0,174 mg/L za CLZ i
0,189 ± 0,132 mg/L za NCLZ. 73,06% i 26,83% izmerenih koncentracija CLZ i NCLZ bile su
izvan referentnog opsega (uglavnom ispod), respektivno.
Značajna korelacija doze i
pacijenata i kombinovane terapije.

abstract = "The atypical antipsychotic clozapine (CLZ) is primarily used for the treatment-
resistant schizophrenia.
Further multivariate
analysis is required to assess, in addition to dose, potential influences of other patient and
Zbog niskog terapijskog indeksa i velike farmakokinetičke
varijabilnosti, terapijsko prać enje lekova (therapeutic drug monitoring, TDM) se preporučuje
(LC‐MS/MS). Statistička analiza je izvršena pomoć u softvera SPSS® (verzija 18). Dnevne doze
CLZ kretale su se između 37,5 i 600 mg. Srednja vrednost je bila 0,285 ± 0,174 mg/L za CLZ i
0,189 ± 0,132 mg/L za NCLZ. 73,06% i 26,83% izmerenih koncentracija CLZ i NCLZ bile su
izvan referentnog opsega (uglavnom ispod), respektivno.
Značajna korelacija doze i
pacijenata i kombinovane terapije.",

AB - The samples of plant material suspected to contain new psychoactive substances are very often the subject of chemical-toxicological analyses. Gas chromatography-mass spectrometry (MS), liquid chromatography-quadrupole time-of-flight-MS, and liquid chromatography-tandem MS were applied with the aim to identify synthetic cannabinoid, methyl 2-{[1-(cyclohexylmethyl)-1H-indol-3-yl] formamido}-3-methylbutanoate (MMB-CHMICA) without the analytical standard, which is very often the case when a new drug arrives. The structure of compound was also confirmed by one-dimensional and two-dimensional nuclear magnetic resonance spectroscopy and conformational analysis. After identification, methanolic extract of plant material containing MMB-CHMICA was successfully used for developing a multiple reaction monitoring method on liquid chromatography-tandem MS instrument. The optimization procedure is shown in detail. The complete fragmentation pattern and also the optimization of the extraction procedure of MMB-CHMICA from plant material were shown. The obtained data are useful for forensic, toxicological, and clinical purposes.

AB - Immunochromatographic strips for urine drug screening tests (UDSTs) are common and very suitable for drug abuse monitoring, but are also highly susceptible to adulterants kept in the household, which can significantly alter test results. The aim of this study was to see how some of these common adulterants affect UDST results in practice and whether they can be detected by sample validity tests with pH and URIT 11G test strips. To this end we added household chemicals (acids, alkalis, oxidizing agents, surfactants, and miscellaneous substances) to urine samples positive for amphetamine, 3,4-methylenedioxymethamphetamine (MDMA), tetrahydrocannabinol, heroin, cocaine, or benzodiazepines (diazepam or alprazolam) and tested them with one-component immunochromatographic UDST strips. The UDST for cocaine resisted adulteration the most, while the cannabis test produced the most false negative results. The most potent adulterant that barely changed the physiological properties of urine specimens and therefore escaped adulteration detection was vinegar. Besides lemon juice, it produced the most false negative test results. In conclusion, some urine adulterants, such as vinegar, could pass urine specimen validity test and remain undetected by laboratory testing. Our findings raise concern about this issue of preventing urine tampering and call for better control at sampling, privacy concerns notwithstanding, and better sample validity tests.

AB - Rhamnolipids are biodegradable low toxic biosurfactants which exert antimicrobial and anti-biofilm properties. They have attracted much attention recently due to potential applications in areas of bioremediation, therapeutics, cosmetics and agriculture, however, the full potential of these versatile molecules is yet to be explored. Based on the facts that many naturally occurring lipopeptides are potent antimicrobials, our study aimed to explore the potential of replacing rhamnose in rhamnolipids with amino acids thus creating lipopeptides that would mimic or enhance properties of the parent molecule. This would allow not only for more economical and greener production but also, due to the availability of structurally different amino acids, facile manipulation of physico-chemical and biological properties. Our synthetic efforts produced a library of 43 lipopeptides revealing biologically more potent molecules. The structural changes significantly increased, in particular, anti-biofilm properties against Candida albicans, although surface activity of the parent molecule was almost completely abolished. Our findings show that the most active compounds are leucine derivatives of 3-hydroxy acids containing benzylic ester functionality. The SAR study demonstrated a further increase in activity with aliphatic chain elongation. The most promising lipopeptides 15, 23 and 36 at 12.5 mu g/mL concentration allowed only 14.3%, 5.1% and 11.2% of biofilm formation, respectively after 24 h. These compounds inhibit biofilm formation by preventing adhesion of C. albicans to abiotic and biotic surfaces.

AB - The aim of our work was to optimize and apply simple high-performance liquid chromatography method with ultraviolet detection (HPLC-UV) for simultaneous determination of reduced (GSH) and oxidized (GSSG) glutathione in biological matrix (specifically, the rat liver tissue was used herein), since the ratio between oxidized and reduced glutathione forms (GSSG-GSH) has been recognized as an important biological marker of oxidatively depleted GSH in oxidative stress (OS)-associated diseases and poisonings. An isocratic chromatographic separation of GSH and GSSG (2.8 min and 6.3 min, respectively) was performed with the mobile phase consisted of sodium perchlorate solution (pH adjusted to 2.8) at flow rate of 1 mL min(-1), detection set at 215 nm, and column temperature of 40 degrees C. The method offers short run time, linearity in the range of 0.01-200 mu M concentration for both compounds (R-2 = 1), low limits of detection and quantification (GSH: 0.18 mu M and 0.56 mu M, GSSG: 0.52 mu M and 1.58 mu M, respectively), precision, accuracy (bias lt 2%), and high reproducibility. Through suitable sample handling, an overestimation of GSSG was prevented. High recovery (>99%) was achieved. The method was successfully applied for the analysis of GSH and GSSG in liver homogenates of Wistar rats intraperitoneally exposed to cadmium (Cd) (1 mg kg(-1) CdCl2/21 days). Regardless of other Cd-mediated hepatotoxicity mechanisms, herein, we have exclusively interpreted/emphasized oxidative GSH depletion. The presented method is acceptable for a routine analysis of GSH and GSSG in biological matrix, while the calculated ratio GSSG-GSH is considered as a valuable OS marker.

AB - The prevalence of alcohol in blood samples from medicolegal autopsies performed in the Institute of Forensic Medicine in Belgrade, in 2011, was studied. In total, 293 blood samples were analyzed for alcohol by headspace gas chromatography. The blood alcohol concentrations were evaluated according to sex, age, and cause of death. The blood alcohol concentrations were . 0.5 g/L in 23.9% of the cases; in suicides 22.4%, accidents 34.4% and homicides 25.0%. The largest proportion of high BACs were found in the sample from subjects killed in traffic accidents. These findings confirm that alcohol use is an important factor in many fatal accidents, suicides and cases of violent death.
