Antioxidative and anti-inflammatory activities of Erica spiculifolia extracts and fractions

AB  - There is little data on the phytochemical/pharmacological properties of Erica spiculifolia Salisb. (syn. Bruckentalia spiculifolia (Salisb.) Rchb.). This study examines the antioxidative and anti-inflammatory activities of different extracts and fractions of E. spiculifolia in vitro on isolated rat peritoneal macrophages, in the carrageenan-induced rat paw oedema test, BSA test, and two complementary antioxidant assays. Ethanolic extracts of leaves, flowers, and aboveground parts, and petroleum ether, ether, ethyl acetate, and water fractionations of the ethanol extract of E. spiculifolia applied at doses of 50–200 mg/kg p.o. exhibited dose-dependent anti-inflammatory activity comparable with indomethacin. All tested samples, except for the petroleum ether fraction, exerted excellent in vitro antioxidant activity, and all of them exhibited significant and similar inhibition of BSA denaturation comparable with diclofenac. Ethanolic extract of the aboveground parts obtained by percolation, ethyl acetate and water fractions had the highest efficiency, attenuating inflammation by more than 50% in the lowest applied concentration alongside exceptional radical scavenging activity.

The impact of treatment choices on potential drug–drug interactions in hypertensive patients

AB  - Aims: The aim of this study was to analyse potential drug–drug interactions (pDDIs) and their potential adverse drug reactions (ADRs) among hypertensive patients. Moreover, we investigated the possibility of reducing pDDIs with different treatment choices. Methods: This was a cross-sectional study including all outpatients with hypertension and two or more medications, treated in a university hospital in Serbia. Lexicomp Interact (Lexi-Comp, Inc., Hudson, OH) was used for identification of pDDIs and potential ADRs. Treatment choices were explored according to patient characteristics, treatment guidelines and the interacting potential of drugs. Data were analysed using descriptive analysis and multiple logistic regression. Results: A total of 350 patients were included in this study, with average age (77 [36–98] years and 6.1 [2.5]) medications. The majority of patients (86.0%) had at least one clinically significant pDDI, and the average was 3.78 (3.90) (range 1–25). Suggestions for treatment change aimed mainly at eliminating drug duplications, reducing the use of thiazide diuretics, sulfonylureas, alpha-lipoic acid and pentoxifylline and increasing the use of calcium-channel blockers, when appropriate. pDDIs would have decreased to 2.10 (2.52), P <.001, yet male gender, ≥6 medications, cardiovascular diseases, diabetes, benign prostatic hyperplasia, would be predictive of two or more pDDIs. The main potential adverse outcomes of pDDIs were hypotension, renal failure, hypoglycaemia, bradycardia and lactic acidosis. Conclusion: Careful choice of drugs can reduce but not eliminate pDDIs and their potential ADRs in hypertensive patients. Close monitoring for hypotension, renal failure, hypoglycaemia, bradycardia and lactic acidosis is necessary.

Pharmacological characterization of Cirsium ligulare Boiss. (Asteraceae) herb decoction

AB  - Background/Aim. Data on phytochemical and pharmacological investigations of genus Cirsium Mill. (Asteraceae) are scarce. Some data suggest that decoctions or infusions prepared from these plants are used in folk medicine as tonics, particularly in inflammatory, liver and stomach diseases. So far there have been no pharmacological investigations related to Cirsium ligulare (C. ligulare) Boiss. Accordingly, the aim of this study was to estimate antioxidative, anti-inflammatory and gastroprotective activities of aqueous extracts of C. ligulare herb prepared as 5% and 10% decoctions. Methods. Antioxidative activity was determined using the method of 2,2-diphenyl-1- picrylhydrazyl (DPPH) radical scavenging. Investigations of anti-inflammatory (a model of systemic inflammatory response induced by endotoxin of Escherichia coli and carrageenaninduced rat paw oedema model for local inflammatory response), as well as gastroprotective effects (a model of stress-ulcer induced by absolute ethanol), were conducted in adult female Wistar rats that were given the aqueous extracts of C. ligulare herb per os. Indomethacin and ranitidine were used as reference drugs for evaluation of local anti-inflammatory and gastroprotective effects, respectively. Results. The results demonstrated that aqueous extracts of C. ligulare herb produced strong antioxidative activity, diminished body weight loss induced by endotoxin, significantly reduced carrageenan-induced paw oedema, and prevented the ulcerogenic action of absolute ethanol. Both anti-inflammatory and gastroprotective activities of the extract tested were comparable to those of the reference drugs. Conclusion. Presented results justify the traditional use of C. ligulare herb decoctions and further phytochemical and pharmacological investigations are warranted.
AB  - Uvod/Cilj. Biljke roda Cirsium su hemijski i farmakološki nedovoljno ispitane. Tradicionalno se dekokti ili infuzi ovih biljaka koriste kao tonici, naročito kod zapaljenjskih procesa, kao i kod bolesti jetre i želuca. Vrsta Cirsium ligulare (C. ligulare) Boiss. do sada nije farmakološki ispitivana. Stoga je cilj ovog istraživanja bio da se proceni antioksidantna, antiinflamatorna i gastroprotektivna aktivnost vodenog ekstrakta herbe C. ligulare izrađenog kao 5% i 10% dekokt. Metode. Ispitivanje antioksidantne aktivnosti vršeno je testom neutralizacije 2,2-diphenyl-1-picrylhydrazyl (DPPH) radikala. Ispitivanje antiinflamatornog (model sistemskog inflamatornog odgovora indukovan endotoksinom bakterije Escherichia coli i karageninom-indukovani edem šape pacova kao model lokalne inflamcije), kao i gastroprotektivnog efekta (model ulkusa indukovanog apsolutim etanolom), sprovedeno je na odraslim ženkama pacova Wistar soja, kojima su per os dati vodeni ekstrakti herbe C. ligulare. Kao referentni lekovi prilikom ispitivanja lokalnog antiinflamatornog i gastroprotektivnog delovanja, primenjeni su indometacin i ranitidin, redom. Rezultati. Rezultati pokazuju da su ispitivani vodeni ekstrakti herbe C. ligulare ispoljili jaku antioksidantnu aktivnost, smanjili gubitak telesne mase indukovan endotoksinom, značajno redukovali karageninom indukovani edem šape pacova i spečili ulcerogeni efekat apsolutnog etanola. Antiinflamatorna i gastroprotektivna aktivnost ispitivanih ekstrakata bila je uporediva sa aktivnošću referentnih lekova. Zaključak. Dobijeni rezultati u značajnoj meri opravdavaju tradicionalnu primenu dekokta herbe C. ligulare, a planiraju se i dodatna hemijska i farmakološka ispitivanja ove droge.

Procalcitonin (PCT) levels are eleveted in the circulation in patients with bacterial sepsis and PCT might be useful in guiding antibiotic treatment. The aim of this study was to estimate factors influencing patients survival and treatment cost in ICU with special emphasis on the impact of PCT serum levels use in guiding antimicrobial therapy. Methods. The study was conducted from August 2010 to May 2012 in the Intensive Therapy Unit, Clinic of Anesthesiology and Intensive Therapy, Military Medical Academy (MMA), Belgrade, Serbia. All adult critically ill patients with sepsis and/or trauma admitted in the ICU were included in the study. This study included only the cost of antimicrobial therapy in the ICU and the cost for PCT analysis. We used prices valid in the MMA for the year 2012. PCT in serum was measured by homogeneous immunoassay on a Brahms Kryptor analyzer. Results. A total of 102 patients were enrolled. The mean patients age was 55 ± 19 years and 61.8% of patients were male. The mean length of stay (LOS) in the ICU was 12 ± 21 days. There was a statistically significant difference (p  lt  0.001) between the sepsis and trauma group regarding outcome (higher mortality rate was in the sepsis group, particularly in the patients with peritonitis who were mostly women). The patients younger than 70 years had better chance of survival. LOS, the use of carbapenems and PCT-measurement influenced the cost of therapy in the ICU. Conclusions. The obtained results show that age, the diagnosis and gender were the main predictors of survival of critically ill patients in the ICU. The cost of ICU stay was dependent on LOS, use of carbapenems and PCT measurement although the influence of these three factors on the outcome in the patients did not reach a statistical significance.
AB  - Uvod/Cilj. Kritično oboleli pacijenti imaju veliki rizik od razvoja teških infekcija u jedinicama intenzivne terapije (JIT). Nivo prokalcitonina (PCT) u cirkulaciji je povišen kod bolesnika sa bakterijskom sepsom, tako da PCT može biti koristan u praćenju antibiotske terapije. Cilj ove studije bio je da se ustanove faktori koji utiču na ishod i troškove lečenja u JIT u našoj ustanovi sa posebnim naglaskom na uticaj korišćenja serumskog nivoa PCT u vođenju antimikrobne terapije. Metode. Studija je sprovedena od avgusta 2010. godine do maja 2012. godine u Jedinici intenzivne terapije Klinike za anesteziologiju i intenzivnu terapiju Vojnomedicinske akademije (VMA) u Beogradu, Srbija. Svi kritično oboleli sa sepsom i/ili traumom koji su primljeni u JIT bili su uključeni u studiju. Studijom su obuhvaćeni samo troškovi antimikrobne terapije u JIT i troškovi PCT analize. Postojala je statistički značajna razlika (p  lt  0.001) između ishoda lečenja u grupi sa sepsom u odnosu na grupu sa traumom. Bolesnici mlađi od 70 godina imali su bolju šansu da prežive. Dužina boravka, upotreba karbapenema i merenje PCT uticali su na cenu terapije u JIT. Zaključak. Dobijeni rezultati pokazuju da su godine života, dijagnoza i pol bili glavni prediktori preživljavanja kritično obolelih u JIT. Cena terapije zavisila je od dužine boravka u JIT, upotrebe karbapenema i merenja PCT, ali uticaj ovih faktora na ishod lečenja nije dostigao statističku značajnost.

Bioactivity assays on Carlina acaulis and C. acanthifolia root and herb extracts

AB  - The present study describes investigation on chemical profile and pharmacological properties of hydromethanolic extracts of Carlina acaulis and C. acanthifolia roots and herbs. In order to investigate the antioxidant properties several methods were applied: FRAP (Ferric Reducing Antioxidant Power) assay, TBA assay (Fe2+/ascorbic induced lipid peroxidation) and scavenging of 2,2-diphenyl-1-picrylhydrazyl (DPPH) and OH radicals. In assessing anti-inflammatory activity the carrageenan-induced rat paw oedema test was used, with indomethacin as a reference drug. Anti-ulcer activity of the extracts was investigated using ethanol-induced stress ulcer in rats and ranitidine as a reference drug. Antimicrobial activity was tested on five bacterial strains and one strain of fungi using the agar diffusion method. Investigated extracts exhibited considerable dose-dependent antioxidant, anti-inflammatory and anti-ulcer activities. Extracts of herbs were more active than the corresponding root extracts. All extracts exhibited moderate activity against all tested microorganisms. Herb extracts of C. acaulis and C. acanthifolia, as well as their root extracts exhibited similar bioactivity, which could be explained with their similar chemical composition revealed by HPLC analysis. The pharmacological activities of investigated Carlina extracts demonstrated here will serve as the basis for their further investigations in order to estimate their impact on human health and nutrition.

AB  - The influence of methanol extracts of four Stachys species from Balkan on carbon tetrachloride induced hepatotoxicity in rats was investigated. The extracts were obtained from aerial parts of S. beckeana Dorfler & Hayek, S. anisochila Vis. et Pancic, S. plumosa Griseb. and S. alpina L. subsp. dinarica Murb. The liver damage was induced by s.c. injections of carbon tetrachloride (2.5 ml/kg b.w.) and the extracts were then consecutively applied for five days. Their effects were evaluated through alteration of biochemical parameters (liver enzymes in the serum), as well as through histopathological changes in the liver (liver damage score, LDS). Treatment of CCl(4)-intoxicated rats with methanol extracts of investigated Stachys taxa (200 and 100 mg/kg b.w. p.o.) significantly reduced increased level of marker enzymes aspartate transaminase (AST), alanine transaminase (ALT) and alkaline phosphatase (ALP) in the serum (with the exception of AST and ALT after 100 mg/kg of S. anisochila extract was administered). Vast degenerative and vascular changes in CCl(4)-treated rats were also notably reduced after the treatment with investigated extracts, corroborating the biochemical observations and confirming their hepatoprotective effects. The best dose-dependant activity was achieved by the methanol extract of S. alpina subsp. dinarica.

Critical analysis of clinical studies of the efficacy and safety of quetiapine in the treatment of patients with major depressive disorder

AB  - In this review is shown the analysis of clinical studies of the efficacy and safety of atypical antipsychotic extended release quetiapine, in the treatment of adult patients with major depressive disorder (MDD), as short-term monotherapy, short-term adjunctive treatment to on-going antidepressant and maintenance treatment or relapse prevention in major depressive episodes. Recommended dosing regimen was 50 to 300 mg daily. Six of seven studies were positive on the primary efficacy variable (MADRS, Montgomery Asberg Depression Rating Scale score) on at least one dose under investigation, whilst one study was not significant. None of the studies was positive on the secondary endpoint: improvement in the quality of life, by assessing the change in Q-LES-Q total score (Quality of Life Enjoyment and Satisfaction Questionnaire). Overall, the safety findings from the extended release quetiapine MDD clinical studies are consistent with known adverse event profile for quetiapine in other known indications: schizophrenia and bipolar disorder. However, during the short-term exposure in the extended release quetiapine MDD clinical studies, the long-term serious adverse events, in particular risk related to metabolic changes, and possibility of tardive dyskinesia and increased risk of sudden cardiac death, has not been adequately studied, therefore the risk benefit ratio, for now, can not be assessed as favorable, with high reliability. Despite that, the regulatory authorities in the U.S. (FDA - Food and Drug Administration) and EU (EMA - European Medicines Agency) have approved, however, reduced indication for extended release quetiapine in the treatment of major depressive disorder: add-on treatment of major depressive episodes in patients with MDD who had suboptimal response to antidepressant monotherapy, noting that, prior to initiating treatment, clinicians should consider the safety profile of extended release quetiapine.
AB  - U radu je prikazana analiza kliničkih studija za procenu efikasnosti i bezbednosti atipičnog antipsihotika kvetiapina, u obliku tableta sa produženom brzinom oslobađanja kod odraslih pacijenata sa velikom depresijom, primenjenog kao: kratkotrajna monoterapija, kratkotrajna dodatna terapija na primenjivani antidepresiv i monoterapija održavanja u prevenciji relapsa epizode velike depresije. Preporučen opseg doza kvetiapina u ovim studijama bio je 50 do 300 mg/dan. Rezultati šest od sedam sprovedenih kliničkih studija pokazali su da je kvetiapin u obliku tableta sa produženom brzinom oslobađanja statistički značajno efikasniji u odnosu na placebo, s obzirom na primarni parametar efikasnosti, promenu ukupnog MADRS skora (Montgomery Asberg Depression Rating Scale Score) za bar jednu od doza koje su ispitivane u studijama, dok u jednoj od studija nije pokazana statistički značajna razlika između kvetiapina i placeba, u odnosu na ovaj parametar efikasnosti. Niti u jednoj od studija kvetiapin u obliku tableta sa produženim oslobađanjem nije bio statistički značajno efikasniji od placeba u odnosu na sekundarni parametar efikasnosti: poboljšanje kvaliteta života procenjeno na osnovu promene ukupnog skora Q-LES-Q upitnika (Quality of Life Enjoyment and Satisfaction Questionnaire total score). Profil neželjenih efekata kvetiapina u obliku tableta sa produženim oslobađanjem u sprovedenim kliničkim studijama na pacijentima sa velikom depresijom nije se razlikovao od poznatog bezbednosnog profila kvetiapina ustanovljenog u ranije odobrenim indikacijama za ovaj lek (shizofrenija i bipolarni poremećaj). Međutim, kratkotrajna izloženost pacijenata sa velikom depresijom kvetiapinu u sprovedenim kliničkim studijama, nije dovoljna za procenu rizika od njegovih dugoročnih ozbiljnih neželjenih efekata kao što su: metabolički poremećaji, potencijalni rizik za razvoj tardivne diskinezije i mogućnost povećanja rizika od iznenadne srčane smrti, tako da se odnos korist/rizik primene ovog leka kod pacijenata sa velikom depresijom, još uvek ne može, sa velikom pouzdanošću, proceniti kao povoljan. Uprkos tome, regulatorna tela u SAD (FDA - Food and Drug Administration) i EU (EMA - European Medicines Agency) odobrila su, doduše, suženu indikaciju za kvetiapin u obliku tableta sa produženim oslobađanjem u terapiji velike depresije: dodatna terapija (add-on) kod pacijenata sa velikom depresijom, koji su imali suboptimalan odgovor na monoterapiju antidepresivom, uz napomenu da, pre započinjanja terapije, lekari treba da razmotre profil bezbednosti ovog leka.

abstract = "In this review is shown the analysis of clinical studies of the efficacy and safety of atypical antipsychotic extended release quetiapine, in the treatment of adult patients with major depressive disorder (MDD), as short-term monotherapy, short-term adjunctive treatment to on-going antidepressant and maintenance treatment or relapse prevention in major depressive episodes. Recommended dosing regimen was 50 to 300 mg daily. Six of seven studies were positive on the primary efficacy variable (MADRS, Montgomery Asberg Depression Rating Scale score) on at least one dose under investigation, whilst one study was not significant. None of the studies was positive on the secondary endpoint: improvement in the quality of life, by assessing the change in Q-LES-Q total score (Quality of Life Enjoyment and Satisfaction Questionnaire). Overall, the safety findings from the extended release quetiapine MDD clinical studies are consistent with known adverse event profile for quetiapine in other known indications: schizophrenia and bipolar disorder. However, during the short-term exposure in the extended release quetiapine MDD clinical studies, the long-term serious adverse events, in particular risk related to metabolic changes, and possibility of tardive dyskinesia and increased risk of sudden cardiac death, has not been adequately studied, therefore the risk benefit ratio, for now, can not be assessed as favorable, with high reliability. Despite that, the regulatory authorities in the U.S. (FDA - Food and Drug Administration) and EU (EMA - European Medicines Agency) have approved, however, reduced indication for extended release quetiapine in the treatment of major depressive disorder: add-on treatment of major depressive episodes in patients with MDD who had suboptimal response to antidepressant monotherapy, noting that, prior to initiating treatment, clinicians should consider the safety profile of extended release quetiapine., U radu je prikazana analiza kliničkih studija za procenu efikasnosti i bezbednosti atipičnog antipsihotika kvetiapina, u obliku tableta sa produženom brzinom oslobađanja kod odraslih pacijenata sa velikom depresijom, primenjenog kao: kratkotrajna monoterapija, kratkotrajna dodatna terapija na primenjivani antidepresiv i monoterapija održavanja u prevenciji relapsa epizode velike depresije. Preporučen opseg doza kvetiapina u ovim studijama bio je 50 do 300 mg/dan. Rezultati šest od sedam sprovedenih kliničkih studija pokazali su da je kvetiapin u obliku tableta sa produženom brzinom oslobađanja statistički značajno efikasniji u odnosu na placebo, s obzirom na primarni parametar efikasnosti, promenu ukupnog MADRS skora (Montgomery Asberg Depression Rating Scale Score) za bar jednu od doza koje su ispitivane u studijama, dok u jednoj od studija nije pokazana statistički značajna razlika između kvetiapina i placeba, u odnosu na ovaj parametar efikasnosti. Niti u jednoj od studija kvetiapin u obliku tableta sa produženim oslobađanjem nije bio statistički značajno efikasniji od placeba u odnosu na sekundarni parametar efikasnosti: poboljšanje kvaliteta života procenjeno na osnovu promene ukupnog skora Q-LES-Q upitnika (Quality of Life Enjoyment and Satisfaction Questionnaire total score). Profil neželjenih efekata kvetiapina u obliku tableta sa produženim oslobađanjem u sprovedenim kliničkim studijama na pacijentima sa velikom depresijom nije se razlikovao od poznatog bezbednosnog profila kvetiapina ustanovljenog u ranije odobrenim indikacijama za ovaj lek (shizofrenija i bipolarni poremećaj). AB  - Rational use of drugs in veterinary medicine has manifold significance. By using each drug, only when it is really necessary (indicated), in right dose and route of administration, the potential damage of their use is reduced, efficiency increased, and the risk of microorganisms resistance development (in case of antimicrobials) significantly decreased. All of this becomes more important when these drugs are used in food producing animals. Simultaneously with the intensifying of cattle breeding and exceptional increase of animal productivity, the number of used drugs is unavoidably increased. There are almost no animals today in intensive as well as in extensive production/rearing system, which haven't received at least one drug during their life. In poultry, cattle and pig production, which are main sectors for production of food stuffs of animal origin, the use of drugs has increased and reached the limits which are considered alarming for health of people. On the first place are antibiotics, or antimicrobial drugs, which are used very often in therapy or prevention of bacteria diseases, and also, very often, as growth stimulator. In addition to antibiotics, many other drugs are used, with proven numerous harmful effects, even with carcinogenic and teratogenic features, whose residues in food intended for human consumption can very seriously endanger the health of people, as potential consumers of this kind of food. Therefore, the control of use of antimicrobial, and other drugs in livestock production is exceptionally significant.
AB  - Racionalna primena lekova u veterinarskoj medicini ima višestruk značaj. Korišćenjem lekova samo kada su stvarno neophodni (indikovani), u pravoj dozi i na pravi način, smanjuje se potencijalna šteta od njihovog korišćenja, a ujedno se povećava delotvornost, i smanjuje rizik od nastanka rezistentnosti mikroorganizama (odnosi se na antimikrobne lekove). Sve navedeno postaje još važnije kada se veterinarski lekovi koriste u lečenju farmskih životinja, odnosno životinja čiji se proizvodi koriste za ishranu ljudi. Istovremeno sa intenziviranjem stočarske proizvodnje i povećanjem produktivnosti životinja, povećava se i broj lekova koji se koriste. Danas ne postoji skoro nijedna životinja, u intenzivnom, kao i ekstenzivnom uzgoju, koja nije primila barem jedan lek tokom svog života. U živinarstvu, govedarstvu i svinjarstvu, kao glavnim sektorima gde se proizvode prehrambeni proizvodi životinjskog porekla, upotreba lekova je dostigla granice koje se mogu smatrati alarmantnim za zdravlje ljudi. Na prvom mestu su antibiotici, ili antimikrobni lekovi, koji se veoma često koriste u terapiji ili prevenciji bakterijskih bolesti, a ne tako retko i kao promotori rasta. Osim antibiotika, koriste se i mnogi drugi lekovi, koji imaju dokazano štetno dejstvo, pa čak i kancerogene ili teratogene osobine/svojstva. Njihove rezidue u proizvodima životinjskog porekla koji se koriste u ljudskoj ishrani, mogu veoma ozbiljno da ugroze zdravlje ljudi, koji su potencijalni konzumenti ove vrste hrane. Zbog toga je kontrola primene antimikrobnih lekova, kao i ostalih lekova u stočarstvu, od izuzetne važnosti.

AB  - Rationale In developed countries, cyclooxygenase 2 (COX-2) inhibitors were shown to be less costly than the combination of non-steroidal anti-inflammatory drugs (NSAIDs) and proton pump inhibitors (PPIs) in treatment of patients with high risk of serious gastrointestinal (GI) adverse effects. It is questionable if such results apply to developing countries where health service costs are lower and there is high discrepancy between generic and patent protected drug prices. We analysed the direct cost of treatment with generic NSAIDs in combination with PPIs versus branded COX-2 inhibitors in patients with high risk of serious GI adverse effects from the perspective of the public health service in Serbia. Methods Total cost of treatment of serious GI complications and the use of NSAID+PPI versus COX-2 inhibitors were calculated. A model for estimation of cost of treatment of NSAID+PPI versus COX-2 inhibitors which included the probability of developing serious GI adverse effects was developed. Results Total cost of treatment of serious GI adverse effects resulted in an average of $814/patient. Considering the relative risk of such adverse effects for patients with four or more risk factors, the least costly treatment over 6 months was the use of celecoxib ($487). Compared with diclofenac+omeprazole, cost savings were estimated at $59 and $22 per patient with celecoxib and etoricoxib, respectively. Conclusion Cost savings may be achieved by using COX-2 inhibitors in patients at high risk of GI adverse effects even in countries with moderate health care service expenditures. Such possibility requires further investigation.

AB  - The present study investigated the antiinflammatory, gastroprotective and antioxidant activities of a CH(2)Cl(2) extract of western Balkan endemic Hieracium gymnocephalum Griseb. ex Pant. (Compositae). The carrageenan-induced rat paw oedema test was used as an experimental model for screening the antiinflammatory activity. The extract was administrated p.o. in doses of 25, 50, 100 and 200 mg/kg to rats and its effects compared with indomethacin, used p.o. as a reference drug. The results showed that the investigated extract reduced the oedema in a concentration-dependent manner. The obtained antiinflammatory effect was 5.9%, 11.7%, 31.2% and 44.1% at doses of 25, 50, 100 and 200 mg/kg, respectively, being statistically significant at a dose of 100 mg/kg. Indomethacin had a strong antiinflammatory effect of 73.4% at a dose of 8 mg/kg, but caused large gastric lesions. When the plant extract in the highest tested dose (200 mg/kg) was concomitantly given with indomethacin, the and inflammatory effect was slightly enhanced, but the gastric lesions were significantly reduced. The antioxidant activity of the H. gymnocephalum extract, investigated using DPPH radical assay, OH-radical assay and TBA-test, was not substantial. Copyright

AB  - The article describes a two-step synthesis of diastereomeric 3-hydroxy-2methyl-3-( 4-biphenylyl) butanoic acids. In the first step an intermediate alpha-bromo propanoic acid 1-ethoxyethyl ester was synthesized. The second step is a new modified Reformatsky reaction in presence of Zn in tetrahydrofuran (THF) at -5 to 10 C between the previously synthesized intermediate and 4-acetylbiphenyl. Synthesis of the other studied beta-hydroxy- beta-arylpropanoic acids has already been reported. These beta-hydroxy-beta-arylpropanoic acids belong to the arylpropanoic acid class of compounds, structurally similar to the NSAIDs such as ibuprofen. The anti-inflammatory activity and gastric tolerability of the synthesized compounds were evaluated. Molecular docking experiments were carried out to identify potential COX-2 inhibitors among the beta-hydroxy- beta-aryl-alkanoic acids class. The results indicate that all compounds possess significant anti-inflammatory activity after oral administration and that the compounds 2-(9-( 9-hydroxy-fluorenyl))-2-methylpropanoic acid ( 5) and 3-hydroxy-3,3-diphenyl- propanoic acid (3) possess the strongest anti-inflammatory activity, comparable to that of ibuprofen, a standard NSAID, and that none of tested substances or ibuprofen produced any significant gastric lesions.

AB  - The root of Carlina acanthifolia All. (Asteraceae) has been traditionally used in the treatment of various disorders including stomach and skin diseases. We studied antimicrobial, anti-inflammatory, anti-ulcer and antioxidant activities of Carlina acanthifolia root essential oil, in order to validate some of the ethnopharmacological claims. Antimicrobial activity was tested on 15 bacteria and three strains of fungi using the agar diffusion and broth microdilution methods. In assessing anti-inflammatory activity the carrageenan-induced rat paw oedema test was used, while ethanol-induced stress gastric ulcer test in rats was used in testing anti-ulcer activity. Antioxidant properties were evaluated trough the effect of the essential oil on lipid peroxidation (TBA assay) and its capability of quenching 2,2-diphenyl-1-picrylhydrazyl (DPPH) and OH radicals. The oil expressed significant antimicrobial activity, being the most active against Gram (+) bacteria: Streptococcus pyogenes, Enterococcus faecalis, Bacillus subtilis and against Candida albicans. In all applied concentrations, Carlina acanthifolia root essential oil reduced carrageenan-induced rat paw oedema in dose-dependent manner, achieving high degree of anti-inflammatory activity. The effect was comparable with that of indomethacin used as a reference drug. In the ethanol-induced stress gastric ulcer test in rats, it was shown that the tested essential oil produced significant dose-dependent gastroprotective activity. The results also pointed out substantial and dose-dependent antioxidant activity of the investigated essential oil, with carlina oxide as the main antioxidant component.

stomach diseases. Data on phytochemical and pharmaco-toxicological investigation of
genus Cirsiunt are inadequate,-while there are almost no investigation data related to C
ligulare.
The aim of this study was to estimate antioxidative, anti-inflammatory and
gastroprotective activities of aqueous extract crf C. ligulare herb prepared as 50lo and
l}%decoction. An antioxidative activiry was determined by using the method of DPPH
radical neutralizatiol. Investigations o1'antiinflammatory (model of systemic
inflammatory reaction induced by endotoxin of E. coli and canageenan-induced rat paw
oedema model), as well as gastioprotective (stress-ulcer produced by absolute ethanol)
activities were conducted on adult female Wistar rats that r.vere given the extract per os'
Indomethacin and ranitidine were used as reference drugs for evaluation of an antiinflammatory
and an anti-ulcer action, respectively.
The results dernonstrated that the aqueous extract of C. ligulare herb produced
strong untio*iaative activity, diminished body weight loss induced by endotoxin,
significantly reduced .ur.ug..nan-induced paw oedema, and prevented ulcerogenic
action of absolute ethanol. Eoth anti-inflammatory and gastroprotective actions of the
extract tested were comparable to those of the reference drugs'
These resultslustitr, the traditional use of C. ligularehetb and imply the need for
its detailed phytochemicar and pharmacological investigations.

Safe use of herbal drugs during pregnancy and lactation

AB  - Pregnancy is very specific period in which special warnings in drug use, including herbal drugs are recommended. This is related on period of lactation, too, since all drugs distributed and excreted into human milk potentially producing effects on nursing babies. There are no limits for use during pregnancy and lactation only for few herbal drugs. Some herbal drugs are contraindicated for use in these conditions due to their known adverse effects on fetus and newborns. However, for most herbal drugs there are no data, or they are very scarce, on the use during pregnancy and lactation. According to general recommendations in these cases, such herbal drugs should only be used after consulting a physician or under his supervision. However, for safety reasons, their use should be avoided every time if it is possible, especially in the first trimester when a fetus is the most vulnerable. Some herbal drugs can be used for therapy of pregnancy-related conditions (e.g. ginger for nausea and vomiting, and Lini semen, Plantaginis ovatae semen/testa and Psyilii semen for constipation). The use of herbal drugs for stimulation of lactation is traditional, and for none of them a clinical confirmation on efficacy and safety exist.
AB  - Trudnoća je veoma specifičan period u kome treba biti posebno oprezan pri primeni lekova, uključujući i biljne. Ovo se odnosi i na period dojenja, budući da se svi lekovi, u većem ili manjem stepenu, raspodeljuju i izlučuju mlekom s mogućim ispoljavanjem dejstava na odojče. Za manji broj biljnih lekova ne postoji ograničenje za primenu u ovim stanjima, neki su kontraindikovani zbog poznatih neželjenih efekata na plod i odojče, dok za najveći broj biljnih lekova podaci o primeni u trudnoći i tokom perioda dojenja ne postoje ili su nepotpuni. U skladu sa opštom medicinskom praksom, oni se ne smeju koristiti bez savetovanja i nadzora lekara. Međutim, iz bezbednosnih razloga, najbolje je izbegavati ih tokom trudnoće i dojenja, kada god je to moguće, naročito u prvom trimestru kada je fetus najosetljiviji. Izvesne biljne droge mogu da se koriste za terapiju propratnih tegoba u trudnoći (npr. rizom đumbira, Zingiberis rhizoma kod mučnine i povraćanja, seme lana, Lini semen, seme i semenjača ispagule, Plantaginis ovatae semen/testa i seme indijske bokvice, Psyllii semen kod opstipacije). Primena biljnih droga za stimulaciju laktacije isključivo je tradicionalna, tj. ni za jednu ne postoji klinička potvrda efikasnosti i bezbednosti ovakve primene.

AB  - The root extracts of Onosma leptanhtha were evaluated for their anti-iflammatory and cytotoxic activities. The cyclohexane extract, which appeared as the most active in both assays, has been further subjected to bioassay-directed fractionation to afford the naphthazarine derivatives: beta,beta-dimethylacrylshikonin (1), isovalerylshikonin (2) and acetylshikonin (3). The evaluation of the anti-inflammatory activity was performed on carrageenan-induced rat paw edema test. All the tested compounds proved to be active, while compound 3 showed the best anti-inflammatory effect. In addition, the cytotoxic activity of the extracts and isolated compounds, was also assayed against L1210 murine lymphoblastic leukemia cell line, and human fibrosarcoma HT-1080 cells. Compound I exhibited remarkable cytotoxic activity (390nM for L1210 cells), which is superior to that of shikonin, which was used as control.

Diuretic activity of Maydis stigma extract in rats

AB  - Maydis stigma (corn silk) is a herbal drug reputed for the treatment of urinary ailments in various traditional medicine systems. To determine its influence on urinary volume and the excretion of sodium, potassium and chloride, 5% and 10% decoctions were administered daily to adult male Wistar rats for eight days. The concentration of electrolytes and urea in plasma, the influence of treatment on urinary pH value as well as creatinine clearance were also investigated. Daily oral administration of 5% decoction at the dose of 10 ml/kg led to a significant and acute diuresis in rats, reaching the peak value in the first 24 h of treatment. Over a similar period, application of 10% decoction did not affect urinary excretion of water, but significantly increased the pH value of excreted urine. A significant decrease in sodium and chloride plasma levels was observed in both treated groups. The creatinine clearance was markedly increased after the treatment with both extracts. Our findings indicate that the diuretic effect of 5% aqueous Maydis stigma extract is in accordance with the increase in glomerular filtration rate and inhibition of sodium and chloride tubular reabsorption, caused a by still unidentified intrinsic factor, but not the salt-loading effect.

Evaluation of Tanacetum larvatum for an anti-inflammatory activity and for the protection against indomethacin-induced ulcerogenesis in rats

AB  - Oral administration of the chloroform extract from Tanacetum larvatum (Griseb. ex Pant.) Kanitz caused a dose-dependent anti-inflammatory effect in the carrageenan-induced rat paw oedema test. The obtained anti-inflammatory effect was 8.6, 32.8, 37.0 and 49.5% for the extract doses of 25, 50, 100 and 200 mg/kg, respectively, being statistically significant at a dose of 50 mg/kg. Indomethacin had a strong anti-inflammatory effect of 73.4% at a dose of 8 mg/kg, but large gastric lesions were detected. When the plant extract in the highest tested dose (200 mg/kg) was concomitantly given with indomethacin, the anti-inflammatory effect was slightly enhanced, but the gastric lesions were significantly reduced. The anti-inflammatory and anti-ulcer activity may be mainly due to the inhibition of DNA binding of the transcription factor NF-kappaB by components of the plant extract. This was proven in an electrophoretic mobility shift assay at a concentration of 50 mug/ml. Due to its anti-inflammatory as well as anti-ulcer effects, Tanacetum larvatum should especially be used combined with those drugs that are known both for their strong anti-inflammatory activities and the ulcerogenic side effects such as NSAIDs.
