How Supplementation with SOD-Rich Plant Extract, Combined with Gliadin, Can Affect Oxidative Stress Markers and Zonulin Levels in Exercise-Induced Oxidative Stress

AB  - A randomized, double-blind, placebo-controlled study was conducted to investigate the influence of supplementation with a superoxide dismutase (SOD)-rich plant extract on markers of oxidative stress, zonulin levels and the performance of elite athletes. Participants were 30 international-level rowers, divided into an experimental group (n = 15) and a control group (n = 15). The rowers performed a maximal effort incremental test on a rowing ergometer at the beginning and at the end of the study. Markers of oxidative stress (total antioxidant status (TAS), total oxidant status (TOS), oxidative stress index (OSI), superoxide dismutase (SOD), glutathione peroxidase (GPx), advanced oxidation protein products (AOPPs), malondialdehyde (MDA), sulfhydryl (SH) groups, bilirubin, uric acid, albumin and zonulin) were determined in serum. A lower TOS (p = 0.010) and OSI (p = 0.004), a lower MDA (p = 0.001) and a higher level of SH groups (p = 0.031) were observed in the experimental group after supplementation. Physical performance was evaluated through metabolic efficiency, taking lactate levels and power output on the ergometer into account. After 6 weeks of supplementation, the relative increase in metabolic efficiency at a 4 mmol/L lactate concentration and maximal effort was significantly higher in the experimental group (p = 0.004 and p = 0.015, respectively). These results suggest that supplementation with a SOD-rich extract promotes lower oxidative stress, better antioxidant protection and, consequently, the better work performance of athletes.

AB  - Background: The unpredictable course of Coronavirus
Disease 19 (COVID-19) is making good severity assessment tools crucial. This study aimed to assess the usefulness of serum amyloid A (SAA) and other acute-phase
reactants (APRs) in ambulatory care COVID-19 patients
and identified relationships between these markers and disease outcomes.
Methods: From August to November 2020, patients seen
in the outpatient department of the Clinic for Infectious and
Tropical Diseases (Belgrade, Serbia) with confirmed
COVID-19 were included. Patients were classified into
mild, moderate, and severe disease groups based on World
Health Organization criteria. SAA, C-reactive protein
(CRP), interleukin-6 (IL-6), procalcitonin (PCT), ferritin, fibrinogen, D-dimer, albumin, and transferrin were measured.
The median values of all APRs were compared between
COVID-19 severity groups, hospitalized and non-hospitalized patients, and survivors and non-survivors. The
Receiver operator characteristic (ROC) curve analysis was
used for the classification characteristics assessment of
individual APRs for the severity of illness, hospitalization,
and survival.
Results: Higher levels of SAA, CRP, IL-6, PCT, and lower
levels of transferrin and albumin were observed in severe
cases, hospitalized patients, and non-survivors. Based on
ROC curve analysis AUC for SAA has fair classification performance for disease severity (0.794) and death (0.732)
and good performance for hospitalization (0.853).
Conclusion: SAA is a valuable marker in everyday practice
for assessing COVID-19 severity and prognosis in ambulatory patients.
AB  - Uvod: Tok koronavirusne bolesti 2019 (COVID-19) je nepredvidiv, zbog čega su neophodni dobri alati za procenu težine ove infekcije. Svrha ovog istraživanja je procena značaja određivanja serumskog amiloida A (SAA) i drugih reaktanata akutne faze (RAF) kod ambulantnih pacijenata sa COVID-19, kao i identifikacija veze između ovih markera i ishoda bolesti. Metode: Od avgusta do novembra 2020. godine, u studju su uključeni pacijenti koji su pregledani u Prijemnoj ambulanti Klinike za infektivne i tropske bolesti (Beograd, Srbija), sa potvrđenim COVID-19. Pacijenti su bili klasifikovani u grupe blage, umerene i teške bolesti na osnovu kriterijuma Svetske zdravstvene organizacije. Određivane su vrednosti SAA, C-reaktivnog proteina (CRP), interleukina-6 (IL-6), prokalcitonina (PCT), feritina, fibrinogena, D-dimera, albumina i transferina. Medijane vrednosti RAF su poređene između grupa težine bolesti, bolesnika koji su bili hospitalizovani i onih koji nisu, kao i preživelih i preminulih. Korišćena je analiza Receiver operator characteristic (ROC) krive i Area-under-curve (AUC) za analizu klasifikacionih karakteristika pojedinačnih RAF za težinu bolesti, hospitalizaciju i preživljavanje. Rezultati: Viši nivoi SAA, CRP, IL-6, PCT i niži nivoi transferrina i albumina su uočeni kod osoba sa teškim oblikom bolesti, hospitalizovanih pacijenata i preminulih. Na osnovu analize ROC krive, AUC za SAA je pokazao zadovoljavajuće klasifikacione performanse za težinu bolesti (0,794) i smrtni ishod (0,732) i dobre performanse za hospitalizaciju (0,853). Zaključak: SAA je vredan marker u svakodnevnoj praksi za procenu težine i prognoze COVID-19 kod ambulantnih pacijenata.

The phenotype of Graves’ orbitopathy is associated with thyrotropin receptor antibody levels

AB  - Purpose: Graves’ orbitopathy (GO) is a specific inflammatory disorder of the orbit characterized by a highly heterogeneous clinical phenotype. The role of thyrotropin receptor antibodies (TSH-R-Ab) has been widely researched, however there is still no evidence that these antibodies have a direct pathogenic role in this pathology. The aim of this study was to examine their relation to the individual clinical features of GO. Methods: Ninety-one consecutive patients with GO were recruited. Total antibody concentration (TSH-R binding inhibitory immunoglobulins, TBII) and their functional activity (stimulating TSH-R-Ab, TSAb) were measured using binding immunoassay and cell-based bioassay, respectively. Results: Both TSAb and TBII levels were significantly associated to the clinical parameters of GO activity. TSAb was a more sensitive serological marker compared to TBII pertaining to eyelid retraction and edema, proptosis, extra-orbital muscle disorders, diplopia, irritable eye symptoms, and photophobia. TSAb, but not TBII, was a significant predictive marker of conjunctival redness, chemosis, caruncle/plica inflammation, eye irritation, and orbital pain, (odds ratio: 3.096, p = 0.016; 5.833, p = 0.009; 6.443, p = 0.020; 3.167, p = 0.045; 2.893, p = 0.032; versus 2.187, p = 0.093; 2.775, p = 0.081; 3.824, p = 0.055; 0.952, p = 0.930; 2.226, p = 0.099, respectively). Neither TSAb nor TBII correlated with the level of proptosis (ρ = 0.259, p = 0.090, and ρ = 0.254, p = 0.104, respectively), however rising TSAb levels were strongly associated to the level of proptosis. Conclusions: TSH-R-Ab were significantly associated with GO’s phenotype. Especially TSAb, as a sensitive and predictive serological biomarker, can improve diagnosis and management of GO.

Asymmetric Dimethylarginine Is a Marker of Endothelial Dysfunction in Thrombotic Antiphospholipid Syndrome Patients

AB  - Objective: The potential contribution of asymmetric dimethylarginine (ADMA) and high-sensitivity C reactive protein (hsCRP) to endothelial dysfunction in APS patients has not been studied in detail, until now. The study involved 105 APS patients (59 diagnosed with primary APS (PAPS) and 46 APS associated with systemic lupus erythematosus (SAPS)) who were compared to 40 controls. Endothelial dysfunction was assessed by measurement of flow-mediated dilatation (FMD) and glyceryl trinitrate dilatation (NMD) of the brachial artery. ADMA (micromol/L) was analyzed by ELISA. Results: FMD in patients with APS was significantly lower than that of the controls (p < 0.001), with no difference between the PAPS and the SAPS groups. ADMA and hsCRP concentrations were significantly higher in the patient cohort than in the control group (p < 0.001, p = 0.006, respectively), as was the case with the SAPS group as compared to the PAPS group (p < 0.001, p = 0.022, respectively). FMD impairment correlated to ADMA (ρ 0.472, p < 0.001) and to hsCRP (ρ 0.181, p = 0.033). In the regression model, the ADMA concentration confirmed the strength of its association (B 0.518, SE 0.183, Wald 8.041, p = 0.005, Exp(B) 1.679, 95% CI 1.174–2.402) to FMD impairment. The synergistic probability model of ADMA and hsCRP caused FMD impairment when the positivity of β2GPIIgG was added. ADMA may be used as a simple and low-cost tool for verifying the presence of endothelial dysfunction in APS patients. According to the results of the study, we could presume that hsCRP, together with aPL, has a preparatory effect on the endothelium in causing endothelial dysfunction.

The study involved 105 APS patients (59 diagnosed with primary APS (PAPS) and 46 APS associated with systemic lupus erythematosus (SAPS)) who were compared to 40 controls. Endothelial dysfunction was assessed by measurement of flow-mediated dilatation (FMD) and glyceryl trinitrate dilatation (NMD) of the brachial artery. ADMA (micromol/L) was analyzed by ELISA. Results: FMD in patients with APS was significantly lower than that of the controls (p < 0.001), with no difference between the PAPS and the SAPS groups. ADMA and hsCRP concentrations were significantly higher in the patient cohort than in the control group (p < 0.001, p = 0.006, respectively), as was the case with the SAPS group as compared to the PAPS group (p < 0.001, p = 0.022, respectively). FMD impairment correlated to ADMA (ρ 0.472, p < 0.001) and to hsCRP (ρ 0.181, p = 0.033). In the regression model, the ADMA concentration confirmed the strength of its association (B 0.518, SE 0.183, Wald 8.041, p = 0.005, Exp(B) 1.679, 95% CI 1.174–2.402) to FMD impairment. The synergistic probability model of ADMA and hsCRP caused FMD impairment when the positivity of β2GPIIgG was added. ADMA may be used as a simple and low-cost tool for verifying the presence of endothelial dysfunction in APS patients. According to the results of the study, we could presume that hsCRP, together with aPL, has a preparatory effect on the endothelium in causing endothelial dysfunction.",

AB  - This study aimed to investigate the effect of supplementation with plant origin superoxide dismutase (SOD), GliSODin, on parameters of muscle damage, metabolic, and work performance at international level rowers. Twenty-eight rowers were included in a randomized, double-blind study. The study was conducted during a 6-week preparation period. At the beginning of the study and after 6 weeks of the supplementation period, all rowers were tested on a rowing ergometer. Blood samples were taken from the antecubital vein before and after every ergometer testing. Muscle damage markers creatine kinase (CK) and lactate dehydrogenase (LDH), total antioxidant capacity (TAC), inflammation parameters interleukin-6 (IL-6), and C-reactive protein (CRP) were measured. Rowing performance was assessed by lactate level in capillary blood and power output on the rowing ergometer. After supplementation, experimental group had significantly lower CK (p = 0.049) and IL-6 (p = 0.035) before and IL-6 (p = 0.050) after exhausting exercise on ergometer. Relative change of power output at 4 mmol/L concentration of lactate in blood, considering the initial and final test, was significantly higher (p = 0.020) in the supplemented group. It was concluded that GliSODin could be considered a good supplement in preventing some deleterious effects of intensive physical activity, including inflammation and muscle damage, and consequently, to enable a better rowing performance of elite rowers.

AB  - Background: Thyrotropin receptor autoantibodies (TSH-R-Ab) are indispensable biomarkers in the laboratory assessment of thyroid-associated orbitopathy (TAO). Clinical sensitivity of three different assays for TSH-R-Ab determination was evaluated in patients with TAO.

Methods: 87 consecutive TAO patients were enrolled and their serum samples analyzed in parallel with three assays. An ECLIA competitive binding and a chemiluminescent bridge immunoassay were used to measure total and bind-ing TSH-R-Ab concentration, while their functional activity was determined using a stimulatory TSH-R-Ab (TSAb) cell-based bioassay.

Results: Compared to the two binding assays (ECLIA p<0.001, bridge p=0.003), the TSAb bioassay was more sensitive pertaining to the positive detection of TSH-R-Ab in TAO patients. No difference (p=0.057) was noted between the ECLIA and bridge assays regarding sensitivity rate. All patients with active and/or moderate-to-severe TAO tested positive in the TSAb bioassay (100% and 100%, respectively), while the positivity rates for bridge and ECLIA binding assays were 89.7% and 82.1% for active TAO, and 90.2% and 86.3% for severe TAO, respectively. Negative predictive values of the bioassay, bridge, and ECLIA assays were 100%, 75%, and 71%, respectively for active TAO, and 100%, 86%, and 71%, respectively for moderate-to-severe TAO. The superiority of the bioassay was most prominent in euthyroid (ET) TAO. Positivity rates of the TSAb bioassay, bridge and ECLIA binding assays were 89.6%, 75%, and 64.6%, respectively for inactive TAO; 86.1%, 69.4%, and 52.8%, respectively for mild TAO; 87.5%, 62.5%, and 12.5%, respectively for euthyroid TAO. The bridge assay correlated better with the ECLIA binding assay (r=0.893, p<0.001), compared to the bioassay (r=0.669, p<0.001).

Conclusions: In patients with TAO of various activity and severity, the TSAb bioassay demonstrates a superior clinical performance compared to both ECLIA and bridge binding assays.

U radu je procenjena klinička osetljivost tri različita testa za određivanje TSH-R-Ab kod pacijenata sa TAO. Metode: U studiju je ključeno 87 uzastopnih pacijenata sa TAO i njihovi uzorci seruma su analizirani paralelno sa tri testa. Za merenje ukupne i vezujuće koncentracije TSH-RAb korišćeni su ECLIA imunohemijski test kompetitivnog vezivanja i hemiluminiscentni imunohemijski "sendvič" test, dok je njihova funkcionalna aktivnost određena pomoću ćelijskog biološkog testa (bioeseja) za određivanje stimulatornih TSH-R-Ab (TSAb). Rezultati: U poređenju sa dva imunohemijska testa vezivanja (ECLIA P<0,001, "sendvič test" P=0,003), TSAb bioesej se pokazao najosetljivijim u pogledu pozitivne detekcije TSH-RAb kod TAO pacijenata. Nikakva razlika nije detektovana (P=0,057) između ECLIA i "sendvič" testa u pogledu stope osetljivosti. Moving average procedures as an additional tool for real-time analytical quality control: challenges and opportunities of implementation in small-volume medical laboratories

The aims of this study were to: (1) implement previously optimized MA procedures for 10 clinical chemistry analytes into the laboratory information system (LIS); (2) monitor their performance as a real-time quality control tool, and (3) define an algorithm for MA alarm management in a small-volume laboratory to suit the specific laboratory. Materials and methods: Moving average alarms were monitored and analysed over a period of 6 months on all patient results (total of 73,059) obtained for 10 clinical chemistry parameters. The optimal MA procedures were selected previously using an already described technique called the bias detection simulation method, considering the ability of bias detection the size of total allowable error as the key parameter for optimization. Results: During 6 months, 17 MA alarms were registered, which is 0.023% of the total number of generated MA values. In 65% of cases, their cause was of pre-analytical origin, in 12% of analytical origin, and in 23% the cause was not found. The highest alarm rate was determined on sodium (0.10%), and the lowest on calcium and chloride. Conclusions: This paper showed that even in a small-volume laboratory, previously optimized MA procedures could be successfully implemented in the LIS and used for continuous quality control. Review of patient results, re-analysis of samples from the stable period, analysis of internal quality control samples and assessment of the analyser malfunctions and maintenance log have been proposed for the algorithm for managing MA alarms.

Antiphospholipid antibodies in healthy serbian middle-aged subjects: preliminary data

AB  - Background: The investigation of the prevalence of the IgG and the IgM isotypes of anticardiolipin (aCL) and antib2glycoprotein I (ab2gpI) Abs in healthy Serbian middle-aged subjects was the main goal of our study. In addition, we analyzed the potential associations of above-mentioned Abs with serum proteins and lipids/lipoproteins. Methods: Forty healthy subjects were included in our study. Obesity (BMI 30 kg/m2) was present in 8/40 (20%) subjects. Titers of analyzed Abs were measured by ELISA. Results: The prevalence of IgG and IgM ab2gpI Abs was 5% and 12.5%, respectively, while the prevalence of IgM aCL was 10%. The IgG ab2gpI Abs were significantly different between subjects with normal triglycerides levels and those with hypertriglyceridemia (Mann-Whitney, P = 0.014). The significant difference in hsCRP concentrations was observed between subjects with the increased levels of the IgM isotype of aCL Abs and those with normal IgM aCL values (Mann-Whitney, P = 0.028). Conclusions: Dyslipidemia and BMI ≥30 were associated with aPL Abs and therefore, the correction of BMI and lipid status might be beneficial in reduction or elimination of predisposing factors that might trigger thrombotic events in otherwise healthy middle-aged subjects. Larger national study is necessary to confirm our findings.

AB  - Depresija pogađa 322 miliona ljudi u svetu i jedan od neurobioloških mehanizama koji doprinosi manifestaciji je hiperaktivnost ose hipotalamus-hipofiza-nadbubreg (HPA). Rizik za nastanak kardiovaskularnih oboljenja je dvostruko povišen kod depresivnih osoba. Poznato je da deficit magnezijuma (Mg) može indukovati hiperaktivnost HPA ose. Cilj istraživanja bio je da se u modelu depresije rezistentne na triciklične antidepresive, indukovane ACTH tretmanom (10 μg/dan, 21 dan) ispitaju bihejvioralni efekti primene Mg (300 mg magnezijuma/L vode za piće, 28 dana) i uticaj na parametre HPA osovine i neurogeneze kod pacova. Drugi cilj bio je da se ispitaju kardioprotektivni efekti Mg i uticaj na proliferaciju kardiomiocita. Treći cilj bio je da se odrede in vitro efekti ACTH u trodimenzionalnom modelu sferoida humanih kardiomiocita. Rezultati istraživanja su pokazali da je u modelu depresije rezistentne na triciklične antidepresive Mg ostvario anksiolitički i antidepresivni efekat kod mužjaka Wistar pacova. Mg je suprimirao hiperaktivnost HPA ose, što se manifestovalo smanjenjem nivoa kortikosterona i IL- 6 u plazmi. Pored toga, Mg je delovao inhibitorno na proliferaciju fibroblasta i endotelnih ćelija, ali i deponovanje endomizijalnog kolagena u srcu, indukovanih ACTH tretmanom. Takođe, ACTH i Mg su delovali stimulatorno na proliferaciju kardiomiocita. U in vitro modelu, visoka koncentracija ACTH nije uticala na apoptozu, ali je indukovala porast ATP u kardiomiocitima. Rezultati ukazuju da antidepresivni efekat Mg nastupa zahvaljujući supresiji hiperaktivnosti HPA ose i stimulaciji neurogeneze. Magnezijum deluje i kardioprotektivno i inhibira razvoj fibroze miokarda, indukovane primenom ACTH. U modelu sferoida, visoka koncentracija ACTH izaziva hipertrofiju kardiomiocita.
AB  - Depression affects 322 million people worldwide and one of the underlying neurobiological mechanisms involves hyperactivity of the hypothalamic-pituitary-adrenal (HPA) axis. People who suffer from depression show a two-fold increased risk of cardiovascular disease. Studies have shown that magnesium deficiency can provoke HPA axis hyperactivity. The aim of our research was to examine the effects of Mg treatment (300 mg/L, 28 days) on behavioral changes and parameters of HPA axis and neurogenesis in rats, in a model of depressive-like behavior induced by chronic administration of adrenocorticotropic hormone (10 μg/day, 21 days) and resistant to tricyclic antidepressants. Our second aim was to explore cardioprotective effects of Mg, as well as changes in cardiomyocyte proliferation. The third aim was to investigate in vitro effects of ACTH exposure on human cardiomyocytes in a 3D model of spheroids. Results have shown that Mg exerts anxiolytic and antidepressant effects in male Wistar rats, in a model of depressive-like behavior resistant to tricyclic antidepressants. Furthermore, Mg supressed proliferation of fibroblasts and endothelial cells and endomysial collagen deposition evoked by ACTH treatment. ACTH and Mg treatment promoted cardiomyocyte proliferation. In our in vitro model, exposure to high ACTH concentration did not alter level of cardiomyocyte apoptosis, however, an increase in ATP was observed. Mg exerts anxiolytic- and antidepressant-like effects and possible underpinning mechanisms involve attenuated HPA axis hyperactivity and increase in neurogenesis. Moreover, Mg attenuates ACTH-evoked cardiac fibrosis in rats, whereas in vitro exposure to high concentrations of ACTH induces changes that potentially reflect cardiomyocyte hypertrophy.

Synergy of oxytocin and citalopram in modulating Itgb3/Chl1 interplay: Relevance to sensitivity to SSRI therapy

AB  - Intranasal treatment with oxytocin showed beneficial effects in post-traumatic stress disorder and autism spectrum disorders; however, it was not investigated as much in depression. Keeping in mind the favorable effects of oxytocin on animal models of anxiety and depression, we postulated that synergy between prescribed first choice drugs, selective serotonin reuptake inhibitors (SSRIs) and oxytocin could improve the treatment outcome compared with SSRI monotherapy. Our previous in vitro genome-wide transcriptomic study on human lymphoblastoid cell lines exposed to paroxetine resulted in increase of integrin β3 (ITGB3) gene expression, and further, ITGB3/CHL1 expression ratio was hypothesized to influence the sensitivity to SSRIs. The aim of this report was to explore molecular mechanisms behind the antidepressant-like oxytocin effect, alone and in synergy with citalopram, on behavioral and molecular level in corticosterone treated rats, a paradigm used to model anxiety and depression in animals. Oxytocin treatment (1) ameliorated corticosterone-induced reduction of neurogenesis and number of parvalbumin-positive interneurons in the hippocampal CA1 region, (2) enhanced anxiolytic- and antidepressant-like effects of citalopram in the open field test, and (3) the SSRI/oxytocin synergy persisted in reversing the reduction of the Itgb3 gene expression and increased Itgb3/Chl1 ratio in the prefrontal cortices. These results support the existence of synergy between citalopram and oxytocin in reversing the molecular and behavioral changes induced by corticosterone treatment and point to possible molecular mechanisms behind antidepressant-like effect of oxytocin.

AB  - Purpose: Alpha-melanocyte-stimulating hormone (alpha-MSH) has proven cardiovascular effects and plays a significant role as an endogenous countermeasure to ischemia-reperfusion injury. The aim of the current study was to examine the response of alpha-MSH during exercise in patients diagnosed with coronary artery disease (CAD) and evaluate its value in the assessment of severity and prognosis. Methods: Forty subjects with documented CAD (i.e., lesions on coronary angiography ≥ 50%) were included. Cardiopulmonary exercise testing (CPET) on a treadmill (TM) and recumbent ergometer (RE) were performed on two visits, 2–4 days apart, during 2 months of coronary angiography; subsequently, the subjects were followed up for 32 ± 10 months. At rest, at peak CPET, and after 3 min of recovery, plasma levels of alpha-MSH were measured by enzyme-linked immunosorbent assay technique. Results: Mean ejection fraction was 56.7 ± 9.6%. Alpha-MSH similarly increased from rest to peak CPET on both modalities. There were no significant differences in alpha-MSH values during testing in patients with 1,2- and 3-vesel CAD, nor in patients with a SYNTAX score </≥ 23 (p > 0.05). Among CPET and hormonal parameters, ∆alpha-MSH recovery/peak during RE CPET was the best predictor of cardiac event occurrence (chi-square 6.67, HR = 0.51, CI = 0.25–1.02, p = 0.010). Conclusion: ∆alpha-MSH recovery/peak during RE CPET has predictive value for CAD prognosis, demonstrating involvement of alpha-MSH in CAD and a link between stress hormones and cardiac events.

AB  - In medical laboratories, bias presents quantitative expression of trueness, as
analytical performance characteristic that describes systematic errors. Despite bias evaluation
is an integral part of basic lessons of laboratory quality management, it is still difficult to achieve
adequate bias estimation in routine laboratory practice. There are several principles that could
be used to estimate bias, but fundamental for evaluating bias in routine medical laboratories is
the availability of a suitable reference materials. Besides, it is important that the measuring
method and measuring analytical system are in a stable state, and that they have completed
an appropriate validation or verification process. This could be checked routinely by the results
of internal and/or external quality control. In addition, information about the uncertainty of the
certified reference material is a prerequisite for testing the bias. Significant efforts were made
to minimized bias in routine laboratory practice, using reference measurement methods and
commutable reference materials. Still, there are some challenges that remain for the laboratory
professionals as the end users, due to remaining bias between measurement methods and
systems from different and/or the same manufacturers.

Comparison between bone alkaline phosphatase immunoassay and electrophoresis technique in hemodialysis patients

AB  - Background: Problem  of  the  variability  between  the different  methods  using  for  bone  alkaline  phosphatase(bALP)  determination  greately  influences  the  clinical significance of bALP as direct marker of bone metabolism.The  aim  of  this  study  was  to  compare  immunoassay  with electrophoresis technique for bALP determination.  Methods: We  measured  bALP  in  71  patients  on  hemodialysis with agar gel electrophoresis (ISO-PAL, SEBIA) and immunoassay (OSTASE, Beckman Coulter).Results: The   analyzed   methods   showed   significant correlation  (Spearman’s  rho:  0.776,  P  <  0.01),  but  we found statistically significant (P < 0.01) positive bias (27%)for  the  results  measured  by  immunoassay.  In  support  of this,  using  electrophoresis  technique  we  have  detected presence  of  the  intestinal  isoenzymes  of  alkaline  phosphatase in 55% of patients with median value of 30% of the total  alkaline  phoshatase  and  presence  of  liver-2  alkaline phosphatase isoform in 42% of patients with median value of 16.6%. The Kendall’s W of 0.787 (P<0.0001) revealed significant  concordance  between  two  analysed  methods.Cusum  test  showed  no  significant  deviation  from  linearity(P=0.850).Conclusions:Despite  good  agreement  between  immunoassay  methods  and  electrophoresis  technique  for  bALP determination,  interchangeability  between  these  two methods  is  questionable.  Although  immunoassays  are increasingly  used,  as  fully  automated  methods,  in  a  large number  of  laboratories  and  become  routine  methods  for bALP determination, it should be beared in mind, besidesvarious  interferences,  also  the  heterogeneity  of  the  bALP itself, especially in patients on hemodialysis.
AB  - Uvod: Problem varijabilnosti između različitih metoda pomoću kojih se određuje koštani izoezim alkalne fosfataze (bALP) u velikoj meri utiče na klinički značaj bALP kao direktnog markera metabolizma kostiju. Cilj ove studije bio je poređenje imunohemijskog testa sa elektroforetskom tehnikom za određivanje bALP. Metode: bALP je određen kod 71 pacijenta na hemodijalizi, agar gel elektroforetskom tehnikom (ISO-PAL, SEBIA) i imunohemijskom metodom (OSTASE, Beckman Coulter). In vivo toxicity evaluation of two polyoxotungstates with potential antidiabetic activity using Wistar rats as a model system

AB  - In this study, thein  vivohypoglycemic effect of a donut-shaped polyanion salt (NH4)14[Na@P5W30O110]$31H2O{NaP5W30} and its Ag-containing derivative K14[Ag@P5W30O110]$22H2O$6KCl {AgP5W30}, as wellas their hepatotoxicity and nephrotoxicity, was evaluated. In the screening hypoglycemic study,Wistaralbinorats with streptozotocin induced diabetes were treated intraperitoneally with three single doses (5,10, and 20 mg per kg per b.w.) of both investigated polyoxotungstates. The blood glucose levels,measured before and after 2, 4 and 6 h polyoxotungstate application, showed that both studiedcompounds induced the most pronounced and time dependent glucose lowering effects at the doses of20 mg kg1. Thus, daily doses of 20 mg kg1were administered toWistar albinorats orally for 14 days infurther toxicity examinations. The serum glucose concentration and biochemical parameters of kidneyand liver function, as well as a histopathological analysis of kidney and liver tissues were evaluated 14days after the polyoxotungstate administration. Both investigated compounds did not induce statisticallysignificant alterations of the serum glucose and uric acid concentrations, as well as some of the liverfunction markers (serum alanine and aspartate aminotransferases, and alkaline phosphatase activities).However, the significant decrease in serum total protein and albumin concentrations and the increase inbiochemical parameters of renal function–serum urea (up to 63.1%) and creatinine concentrations (upto 23.3%) were observed for both polyoxotungstates. In addition, the detected biochemical changeswere in accordance with kidney and liver histhopathological analysis. Accordingly, the hepatotoxic andnephrotoxic effects of these potential antidiabetic polyoxotungstates could be considered as mild.

methods in laboratory medicine, with which analytical and clinical evaluation of laboratory
test quality is assessed. Literature data indicate that the calculation of MU is not a simple process,
but that its assessment in daily laboratory practice should be reduced to routine and simple
presentation, understandable to both laboratory professionals and physicians. In order to achieve
this, it is necessary to understand the purpose of the test for which MU is to be determined.
Various suggestions have been given for presentation of MU as a quantitative indicator of the
quality of the final measurement result in the medical laboratory. Although MU refers to the final
measurement result, this metrological concept reflects the entire laboratory measurement process.
The data on estimated MU is used to interpret the measured numerical result, and represents
quantitatively the quality of the measurement itself, i.e. how different are the results of
multiple measurements of the analyte of interest in the same sample, as well as whether the
method of determination itself is subjected to significant random and systematic deviation.
Initially, in the metrological concept, the MU is viewed in relation to the true value of the analyte
of interest. However, the true value of the analyte measured in the biological fluid matrix of the
study population cannot be known. It is therefore considered the closest value obtained by the
perfect method, for which the bias and inaccuracy, as measures of systematic and random error,
are equal to zero, which is practically impossible to achieve in routine laboratory practice.
Although current standards require accredited medical laboratories to estimate MU, none of
these guidelines provide clear guidance on how this can be achieved in daily laboratory work.
This review examines literary data and documents dealing with MU issues, but also highlights
what additional terms and data should be considered when interpreting MU. This paper ultimately
draws attention, and once again points out, that a simpler solution is needed for this universal
concept to be formally and universally applicable in routine laboratory medicine practice.

AB  - According to theWorld Health Organization, in 2015, the Serbian population ranked among the highest ones in Europe in terms of smoking habit: 44.3%males and 36.2%females aged 18–64 smoked tobacco. In the last 7 years, 25%of total mortality inmen and 9%in women from Serbia were associated with smoking. Tobacco smoking is one of the most important sources of exposure to many toxic substances in general population. Our study confirmed higher blood levels of two toxic metals, cadmium and lead, in the blood of smokers (3.5 and 1.5 times higher than in non-smokers, respectively). Furthermore, smoking habits, such as number of smoked cigarettes per day, smoking period and cigarette type, along with age, were shown to influence these metals’ blood concentration. Higher blood levels of Cd and Pb were found in smokers consuming more than 10 cigarettes per day for more than 10 years. The present study also highlighted the importance of the controlled tobacco production, since it was shown that consumption of illicit tobacco could manifold the exposure to toxic metals that can subsequently increase the frequency of related diseases as well.

The relationship between uric acid concentration and cardiovascular risk: Retrospective analysis of patients on hemodialysis

AB  - Literature data indicate that increased uric acid (UA) levels are an important risk factor for cardiovascular disease. The aim of this study was to examine the association of  UA with cardiovascular risk factors in patients with hemodialysis using retrospective analysis. In 110 patients,  we  studied  the  values  of  basic  renal  function  parameters,  bone  metabolism  and cardiovascular risk factors in the period from 2010 to 2017. The results indicate a significant increase in urea (P=0.004), creatinine (P=0.028) and inorganic phosphate (P=0.001), as well as significant differences in gender (P=0.013), in risk categories defined on the basis of the UA decision  limit  (cut  off  <350  μmol/L).  After  controlling  the  effects  of  most  parameters, statistically  significant  correlation  coefficients  were  obtained  for  UA  and  urea  (r=0.361; P=0.0013),  creatinine  (r=0.388;  P=0.0005)  and  inorganic  phosphate  (r=0.366;  P=0.0011). Significant regression coefficients were obtained for UA and male gender (β=-0.227, P=0.004), age (β=-0.298, P˂0.001), urea (β=0.271, P=0.005) and inorganic phosphate (β=0.232, P=0.009). The  predictive  value  of  independent  parameters  in  relation  to  UA  was  confirmed  for  male gender  (OR=3.595;  95%  CI:  1.421-9.094;  P=0.007)  and  inorganic  phosphate  (OR=14.842; 2.518-87.472, P=0.003). By Cox regression analysis of proportional hazard ratio, we obtained the most significant combined effect of the body mass index, dialysis and diastolic pressure on UA concentration in relation to the duration of hemodialysis (P <0.0001). The results of this long-term  study  suggest  that  UA  can  not  be  considered  an  independent  cardiovascular  risk factor, but that HD patients need to strategically control the level of MK in order to reduce the resulting complications, morbidity and mortality.

AB  - The antiphospholipid syndrome (APS) is an autoimmune disease characterized by the presence of the IgG and/or IgM isotypes of the antiphospholipid antibodies, thrombosis and/or recurrent pregnancy losses. Various markers of inflammation are associated with clinical and/or laboratory features of APS. Adiponectin (Ad) is a member of the adipocytokines that exert its roles by binding to its receptors (AdR). Peroxisome proliferator-activated receptor gamma (PPAR-gamma) agonists induced Ad production. The aged Pparg null-mice represented the first animal model that spontaneously develops APS and this model emphasized the importance of PPAR-gamma signaling in the development of APS. Recombinant Ad (rAd) application was beneficial for the improvement of glucose, insulin and lipid levels in mice. Orally active AdR agonist exerted similar effects to Ad in mice. Due to the re-occurrence of thrombotic episodes in APS patients (despite life-long anticoagulation), administration of PPAR-gamma agonists, rAd, or AdR agonists should be further tested in experimental models of APS, which eventually, will provide more data for novel therapeutic strategies that will ameliorate clinical manifestations of the APS.

AB  - Intrahepatic cholestasis of pregnancy (ICP) is a gestation-specific liver disorder, defined most often as the onset of pruritus, usually from the third trimester of pregnancy, associated with abnormal liver test results and/or increased total serum bile acids and spontaneous relief after delivery. The 21-year-old patient was admitted to our ward in the 11th week of pregnancy due to raised liver enzymes. The first onset of pruritus and jaundice appeared a month before hospitalization. Immunology tests and Toxoplasma gondii were negative. We excluded viral etiology, while alpha-1-antitrypsin, serum and urine copper levels, and thyroid hormones were within the reference values. The patient denied she had taken any medicines and herbal preparations before and during pregnancy. Total bile acids in the serum were significantly elevated (242 μmol/L). The abdominal ultrasound revealed a regular finding. Liver biopsy suggested a cholestatic liver disorder. After a presentation of all risks, the patient decided to stop the pregnancy. After a month, the hepatogram was within the reference values. Very rarely an ICP can occur in early pregnancy (first trimester), which calls for close monitoring. The risk of serious adverse fetal outcomes and spontaneous preterm delivery is proportional with increased levels of maternal serum bile acid.

AB  - Although a magnesium-mediated attenuation of memory deficits was reported in animal models of ageing and traumatic brain injury, a possible memory enhancement in healthy subjects has not been investigated yet. We used novel object recognition test (NORT) to examine the effects of acute (30 mg/kg) and chronic (50 mg/kg, 28 days) Mg-sulfate treatment on the long-term memory (LTM) in healthy adult male rats, and to test the sustainability of magnesium effects in the models of acute and chronic (21 days) ACTH administration (10 mu g/animal), mimicking the stress- and depression-like conditions. A single dose of Mg-sulfate enhanced the LTM retrieval in the 24 h inter-trial NORT protocol, in healthy, as well as in rats acutely treated with ACTH. Memory enhancement was also detected after 4-week long Mg-sulfate intake, in both healthy and rats chronically treated with ACTH. While the present findings on procognitive effects of chronic Mg-sulfate treatment corroborate with those from studies on the therapeutic potential of Mg-threonate, the current study is the first to report on memory enhancement induced by a single dose of magnesium.

AB  - Magnesium (Mg), is not only a modulator of the glutamatergic NMDA receptors' affinity, it also prevents HPA axis hyperactivity, thus possibly being implicated in neurobiological features of mood disorders. Further uncovering of molecular mechanisms underlying magnesium's proposed effects is needed due to the recent shift in research of treatment resistant depression (TRD) towards glutamatergic pathways. Here, we applied Mg via drinking water for 28 days (50 mg/kg/day), in ACTH-treated rats, an established animal model of depression resistant to tricyclic antidepressants. Using this model in male rats we measured (1) changes in hippocampal neurogenesis and behavioral alterations, (2) adrenal hormones response to acute stress challenge and (3) levels of biometals involved in regulation of monoamines turnover in rat prefrontal cortex. Our results support beneficial behavioral impact of Mg in TRD model together with increased hippocampal neurogenesis and BDNF expression. Furthermore, Mg prevented ACTH-induced disruption in HPA axis function, by normalizing the levels of plasma ACTH, corticosterone and interleukin-6, and by increasing the peripheral release of adrenaline, noradrenaline and serotonin after the acute stress challenge. Finally, the influence on copper/zinc ratio suggested probable magnesium's involvement in monoamine turnover in PFC. Our findings provide further insights into the possible pathways implicated in the behavioral modulation effects of Mg, as well as its central and peripheral effects in ACTH-induced TRD model. Thus, further investigation of molecular signaling related to the glutamatergic transmission and role of Mg, could reveal prospects to novel treatment strategies that could be of particular importance for patients suffering from TRD.

AB  - Background: Data suggest cystatin C (CysC) levels and hepatic artery resistive index (HARI) correspond to the progression of chronic liver disease. We aimed to evaluate the clinical significance of these parameters in assessment of fibrosis in patients with liver cirrhosis. Methods: The cross-sectional study included 63 patients with liver cirrhosis. A control group consisted of 30 age- and gender-matched healthy persons. Results: We confirmed significantly higher values of CysC in patients with cirrhosis compared to control group (p = 0.036). Average value of HARI in the examined group was increased (0.72 +/- 0.06) and there was the statistically significant difference compared to controls (0.66 +/- 0.03) (p  lt  0.001). We found statistically significant correlation between HARI and CysC in the study group. Analyzing the possibility of distinguishing healthy subjects from patients with fibrosis, we have found that the area under the curve is far greater in the HARI index than CysC. Comparison of CysC among Child-Pugh stages and correlation with a model for end-stage liver disease (MELD) score showed statistically significant results. Conclusion: We confirmed HARI is a more accurate parameter than CysC in discriminating healthy subjects from patients with fibrosis, while CysC could be a better indicator of the stage of liver cirrhosis.

AB  - Introduction Vitamin D has a role in cellular differentiation, proliferation, apoptosis, and angiogenesis and therefore is studied as a prognostic factor in cancer. The aim of our study was to assess the prevalence and significance of 25(OH)D deficiency in patients with lymphoid malignancies. Methodology Between January 2014 and June 2016 at the Clinic for Hematology, Clinical Center of Serbia, Belgrade, the pretreatment serum level of 25(OH)D was determined in 133 (62 women/71 men, median age 58 (18-84) years) previously untreated patients with lymphoid malignancy using a chemiluminescent immunoassay. From their medical records, we noted the age, clinical stage, Eastern Cooperative Oncology Group Performance Scale (ECOG PS), nutritional status using the Nutritional Risk Score 2002 (NRS2002), the time of year, comorbidity index, progression, and progression-free survival (PFS) for a median of 20 (1-32) months. The optimal cutoff point for prediction of outcome was determined using the Maximally Selected Rank Statistics. Results There were 37 (27.8%) patients with the severe 25(OH)D deficiency  lt = 25 nmol/l, 80 (60.2%) with 25(OH)D deficiency 25-50 nmol/l, and 16 (12%) with 25(OH)D insufficiency 50-75 nmol/l. None of the patients had the desired normal level. There were significant differences between groups in regard to ECOG PS, NRS2002, type of lymphoma, and progression. The severely 25(OH)D-deficient patients had a shorter mean time until progression (P = 0.018). Cox regression analysis showed that 25(OH)D  lt  19.6 nmol/l remained the only significant parameter for PFS (HR = 2.921; 95% CI 1.307-6.529). Conclusion The prevalence of 25(OH)D deficiency in the analyzed group of patients with lymphoid malignancies is high and greater in malnourished individuals. Patients with pretreatment serum 25(OH)D  lt  19.6 nmol/l had a significantly shorter PFS.

Undiagnosed Hyperglycaemia and Hypertension as Indicators of the Various Risk Factors of Future Cardiovascular Disease among Population of Serbian Students

AB  - Background: A number of risk behaviours, such as smoking, overweight, excessive alcohol intake, insufficient physical activity, excessive and frequent intake of salt, reduced fruit and vegetable intake, increased fat intake, which constitute living habits of an individual can influence the occurrence of hypertension and hyperglycaemia. The changing of these lifestyles can reduce the risk of developing prehypertension and prediabetes. Methods: The survey was conducted at student's campuses. The respondents were subjected to the height, weight, blood glucose and blood pressure. Respondents filled in previously created questionnaire that was approved by the Ethics Committee for Biomedical Research Faculty of Pharmacy, University of Belgrade. Results: The percentage of respondents with a glucose value above the reference value was 14.6% (n=19), 2.4% (n=3) had values greater than 7 mmol/L without being diagnosed with diabetes, and accordingly, 2.4% (n=3) had elevated HbA1c values (above 42 mmol/mol or 6.0%). The percentage of respondents with elevated systolic and diastolic blood pressure was 14.9% and 7.4% respectively. Regarding calculated risk scores, they showed parallel increase with increasing of BMI (HPS), systolic and diastolic pressure (OHS), and glucose concentration (OPS). Conclusions: When analysing all the factors that could cause the later development of diabetes, which is associated with hypertension as well, it is observed that the student population is very much exposed to those factors. The results of this study cannot be representative for the general population of students, but they can provide recommendations for further research.
