Rise of Pregabalin Poisoning and Abuse Cases in Serbia: A Ten-Year Retrospective Study

AB  - Purpose: The aim of this study was to analyze data on gabapentinoid-related attendances to the National Poison Control Center of Serbia (NPCC), particularly abuse cases; to estimate its changes and to compare it with trends in national consumption rates of these drugs. We also aimed to analyze the main characteristics of the study population and to investigate the major clinical effects in poisoned patients. Patients and Methods: This is a retrospective study of patients admitted to the NPCC for acute poisoning involving gabapentinoids from 1 May 2012 to 1 October 2022. Results: There were 357 (95.5%) pregabalin-related and 17 (4.5%) gabapentin-related poisoning cases in 302 patients. Abuse of pregabalin was detected in 27.8% (84/302), while gabapentin abuse occurred in 0.7% (2/302) of all patients. A steady increase in rates of pregabalin poisoning and abuse cases strongly correlated with the increase in overall consumption of this drug, while there were no significant changes in rates of gabapentin consumption, poisoning and abuse rate during the study period. Most patients who abused pregabalin pregabalin were males (84.5%) and the median age was 26 years (range: 15–45 years). Almost 60% of patients who abused pregabalin (48/84) belonged to the migrant population. Co-ingestions occurred in 89.4% of pregabalin-related cases (319/357), resulting in more severe poisoning. The most often co-ingested drugs were benzodiazepines and among them clonazepam was detected in the largest number of cases. Conclusion: The poisoning and abuse cases involving pregabalin are on the rise in Serbia, which coincided with an increase in its overall consumption during the study period. Isolated pregabalin ingestions resulted in mild poisoning, although severe symptoms such as coma and bradycardia were recorded. When prescribing pregabalin to patients at risk of abuse caution is needed. Strengthening the measures for dispensing of pregabalin may reduce the risks associated with its abuse.

Treatment of vascular non-iatrogenic injuries of upper and lower extremities in tertiary vascular center

AB  - BACKGROUND: The purpose of this study was to examine demographic and baseline characteristics of patients with vascular injuries of extremities and to define the most relevant factors which influenced an early outcome, as well as limb salvage after the management of vascular trauma. METHODS: This study used the database that included 395 patients with peripheral arterial injuries, who were treated in the tertiary vascular university center in the period between 2005-2020. Exclusion criteria were isolated thoracic, abdominal and neck injuries as well as iatrogenic injuries and injuries of intravenous addicts. Univariate binary logistic regression analysis and multiple logistic regression were used to determine risk factors for lomb loss (after vascular reconstruction) or mortality. Decision to perform primary amputation (without vascular reconstruction) was based on surgeons' preference and experience. RESULTS: Out of 395 vascular injuries treated in the period 2005-2020, 210 (53.2%) presented with non-iatrogenic vascular injuries of upper and lower extremities were analyzed. According to the univariate regression analysis, hemorrhage as the main clinical manifestation on admission (P=0.035) and early reintervention (P=0.048) increased, while an early patency of repaired artery (0.010) significantly decreased a 30-day amputation rate. Multivariate logistic regression analysis of these three variables showed that only early patency of repaired artery significantly decreased the early amputation rate (P=0.009). CONCLUSIONS: Based on presented experience, the patency of vascular reconstruction plays a crucial role in limb salvage in patients with non-iatrogenic peripheral vascular injuries. All factors that might influence the patency should be in focus of improvement.

AB  - In the present article we investigate the nonlinear dynamics of microtubules, the basic components of the eukaryotic cytoskeleton, relying on the known general model. We introduce the W-potential energy, describing a crucial interaction among constitutive particles within the microtubule. Three kinds of this potential are studied, one symmetrical and two non-symmetrical. We demonstrate an advantage of the latter ones. Solutions of crucial differential equations are solitary waves. The stability of the solutions having physical sense is studied. We show that only subsonic solitary waves are stable, while supersonic ones are not.

AB  - Mushrooms have been evaluated for their nutritional and medicinal properties for
centuries. We evaluated the biological activities of commercial products of Cordyceps
Sinensis (CS), Agaricus blazei (AB), and Immune Assist (IA) (all produced by Aloha
Medicinals). Immune Assist (IA) is made from extract of six species of medical
mushrooms: Agaricus blazei - Cordycepssinensis - Grifola frondosa - Ganoderma
lucidum - Coriolus versicolor - Lentinula edodes.
The antioxidant evaluation showed that CS had strong OH scavenging properties and
moderate reducing power, while its DPPH scavenging ability was weak. AB displayed
remarkable ·OH scavenging properties, moderate reducing power, and modest DPPH
scavenging activity, similar to IA. Also, CS, AB and IA displayed significant antigenotoxic
effects in human peripheral blood cells against H2O2-induced DNA damage.
Further, the cytotoxicity of CS, AB, and IA to HS-5 (bone marrow stroma cells), MCF-
7 (human breast epithelial cell line), and MDA-MB-231 (human breast carcinoma cell
line) were assessed using MTT assay. AB and IA could inhibit the proliferative action
of mentioned cancer cell lines after 72h in a dose-dependent manner, while there were
no effects on HS-5 cell lines proliferation, while CS did not show anti-proliferative/
cytotoxic activity on cancer cell lines.
Current findings remain a significant challenge for the usage of medicinal mushrooms
in the field of cancer prevention/treatment, and thus an active area of future research.

AB  - Combretastatins are a class of natural phenols found in the bark of Combretum
caffrum, commonly known as South African Bush Willow. Despite having a similar
name, combretastatins are unrelated to statins, a family of cholesterol-lowering drugs.
Combretastatin A4 have been shown to be one of the most potent tubulin-depolymerizing
agent. Microtubules control chromosomal segregation and cytokinesis during mitosis
in both cancer and stromal cells and contribute to overall tumor growth. Consequently,
microtubule inhibitors interfere with cell cycle progression and induce apoptosis in
cancer cells in vitro.
The aim of this study was to investigate the potential gentoxic effect of Comretastatin
A4 (CA4) in isolated peripheral blood mononuclear cells (PBMC) in Comet assay in
order to establish is there any DNA damage in healty non-dividing cells. The aim also
was to explore potential cytotoxic activity of CA4 against human cervical carcinoma
(HeLa) cell line.
Genotoxicity of CA4 was evaluated on PBMC in a range of 9 concentrations
(from 1 nM to 200μM). Non of the tested concentrations showed genotoxiceffect.
The same range of different concentrations of CA4 (from 1 nM to 200μM) were
applied to evaluate potential cytotoxicity in a monolayer culture of HeLa cells using
the 3-[4,5-dimethylthiazole-2-yl]-2,5-diphenyltetrazolium bromide (MTT) assay.
After 24h incubation with CA4, there was a significant reduction in cell viability in all
concentrations above 250 nm, while IC50 (half maximal inhibitory concentration) was
123 ± 0.06396 μM.
We concluded that CA4 does not have gentoxic effect on PBMC, and that it reduce
cell viability of cancer HeLa cell lines. These results are especialy importanat because
they showed that CA4 does not damage the DNA molecule in healthy human cells, but
achieves its cytotoxic effect on malignant cells in the same range of concentrations.

Pancreatic cancer is difficult to treat and the cancer progresses rapidly. Unfortunately, current treatments are ineffective. In this study, ten new diarylthiophene-2-carbohydrazide derivatives were synthesized and evaluated for their pharmacological activity. The 2D and 3D anticancer activity studies suggested the compounds 7a, 7d, and 7f were promising. Among these, 7f (4.86 µM) showed the best 2D inhibitory activity against PaCa-2 cells. Compounds 7a, 7d and 7f were also tested for their cytotoxic effects on healthy cell line but only compound 7d showed selectivity. Compounds 7a, 7d, and 7f showed the best 3D cell line inhibitory effect according to spheroid diameters. The compounds were screened for their COX-2 and 5-LOX inhibitory activity. For COX-2, the best IC50 value was observed for 7c (10.13 µM) and all compounds showed significantly lower inhibition compared to standard. In the 5-LOX inhibition study, compounds 7a (3.78 µM), 7c (2.60 µM), 7e (3.3 µM), and 7f (2.94 µM) demonstrated influential activity compared to standard. Regarding molecular docking studies, binding mode of compounds 7c, 7e, and 7f to the 5-LOX enzyme were non-redox or redox types, but not the iron-binding type. As dual inhibitors of 5-LOX and pancreatic cancer cell line, 7a and 7f were identified as the most promising compounds.

AB  - Various dual inhibitors of COX-2 and 5-LOX enzymes have been developed so far in order to obtain more effective and safer anti-inflammatory drugs. The aim of this study was to design and synthesize new dual COX-2 and 5-LOX inhibitors, and to evaluate their enzyme inhibition potential and redox properties. Thirteen compounds (1–13) were designed taking into account structural requirements for dual COX-2 and 5-LOX inhibition and antioxidant activity, synthesized, and structurally characterized. These compounds can be classified as N-hydroxyurea derivatives (1, 2 and 3), 3,5-di-tert-butylphenol derivatives (4, 5, 6, 7 and 13), urea derivatives (8, 9 and 10) and “type B hydroxamic acids” (11 and 12). COX-1, COX-2 and 5-LOX inhibitory activities were evaluated using fluorometric inhibitor screening kits. The evaluation of the redox activity of newly synthesized compounds was performed in vitro in the human serum pool using redox status tests.
The prooxidative score, the antioxidative score and the oxy-score were calculated. Seven out of thirteen synthesized compounds (1, 2, 3, 5, 6, 11 and 12) proved to be dual COX-2 and 5-LOX inhibitors. These compounds expressed good COX-2/COX-1 selectivity. Moreover, dual inhibitors 1, 3, 5, 11 and 12 showed good antioxidant properties.

AB  - The aim of the study was a synthesis and investigation of the dose-dependent anti-inflammatory effect of new thiourea derivatives of naproxen with selected aromatic amines and esters of aromatic amino acids. The results of the in vivo study indicate that derivatives of m-anisidine (4) and N-methyl tryptophan methyl ester (7) showed the most potent anti-inflammatory activity four hours after injection of carrageenan, with the percentage of inhibition of 54.01% and 54.12%, respectively. In vitro assays of COX-2 inhibition demonstrated that none of the tested compounds achieved 50% inhibition at concentrations lower than 100 µM. On the other hand, the aromatic amine derivatives (1–5) accomplished significant inhibition of 5-LOX, and the lowest IC50 value was observed for compound 4 (0.30 μM). High anti-edematous activity of compound 4 in the rat paw edema model, together with potent inhibition of 5-LOX, highlight this compound as a promising anti-inflammatory agent

AB  - Introduction: Telomeres are protective chromosomal ends. Short telomeres are a proven biomarker of biological aging. We aimed to find an association of telomere length and telomerase activity in circulating leukocytes and thromboaspirates of patients with acute myocardial infarction. Furthermore, association of the telomere-telomerase system with oxidative stress markers (as common risk factors for coronary artery disease (CAD)) was tested. Material and methods: Patients were selected from the patients admitted to the intensive care unit with acute myocardial infarction with ST-segment elevation (STEMI), with the following inclusion criteria – STEMI patients between 18 and 80 years old of both genders and candidates for primary percutaneous coronary intervention, with infarction pain present for a maximum of 12 h. In all the patients leukocyte telomere length, telomerase activity and scores related to oxidative-stress status (Protective, Damage and OXY) were evaluated. Results: Patients were divided into different groups: with stable angina pectoris (AP) (n = 22), acute myocardial infarction with: STEMI (n = 93), non-obstructive coronary arteries (MINOCA) (n = 7), blood vessel rupture (n = 6) at three time points, and compared to the group of 84 healthy subjects. Telomerase activity was significantly higher in all CAD sub-groups compared to the control group (AP = 0.373 (0.355–0.386), STEMI = 0.375 (0.349–0.395), MINOCA = 0.391 (0.366–0.401), blood vessel rupture = 0.360 (0.352–0.385) vs. CG = 0.069 (0.061–0.081), p < 0.001), while telomeres were significantly shorter in STEMI, MINOCA and blood vessel rupture groups compared to the control group (STEMI = 1.179 (0.931–1.376), MINOCA = 1.026 (0.951–1.070), blood vessel rupture = 1.089 (0.842–1.173) vs. CG = 1.329 (1.096–1.624), p = 0.030]. Values of OXY score were significantly higher in STEMI and MINOCA patients compared to the control group and AP patients (5.83 (4.55–7.54) and 10.28 (9.19–10.72) vs. 4.94 (3.29–6.18) and 4.18 (2.58–4.86), p < 0.001). Longer telomeres and higher telomerase activity were found in thromboaspirates, compared to the peripheral blood leukocytes in the same patients (1.25 (1.01–1.84) vs. 1.18 (0.909–1.516), p = 0.036; and 0.366 (0.367–0.379) vs. 0.366 (0.367–0.379), p < 0.001, respectively). In addition, telomere length and telomerase activity had good diagnostic ability to separate STEMI patients from healthy persons. Conclusions: Leukocyte telomere length and telomerase activity can differentiate CAD patients from healthy persons, and relate CAD to oxidative stress.

Cadmium and lead implication in testis cancer; is there a connection?

AB  - Testis cancer (TC) is the most common malignancy of young men. Current evidence from studies, alongside genetics and hormonal status, suggests a significant role of toxic metals, cadmium (Cd) and lead (Pb), in the origin and development of TC. Besides oxidative stress and endocrine disruption, interaction with bioelements is one of the critical mechanisms of Cd and Pb toxicity and malign transformation. This study aimed to investigate metal levels in blood, healthy, and tumor testis tissue and to reveal hormone, oxidative status, and bioelements levels in patients with TC. The study enrolled 52 patients with TC and 61 healthy volunteers. Toxic metals and bioelements levels were analyzed by atomic absorption spectrophotometry (AAS) while electrochemiluminescence immunoassay (ECLIA) and spectrophotometry methods were used for hormone and oxidative parameters evaluation. Significantly higher blood Cd levels were depicted in TC cohort. Furthermore, blood Cd elevation was associated with a 1.98 higher probability of TC developing. However, a metal concentration between healthy and tumor testis tissue did not differ significantly. Lower levels of estradiol and testosterone, established in a cohort of TC patients, followed the significant role of hormones in TC development. At the same time, ischemia-modified albumin (IMA) has been recognized as a parameter with very good accuracy as a potential diagnostic marker for TC. The study revealed different distribution patterns of copper (Cu) and zinc (Zn) in the three compartments of the patients, as well significant correlation between essential metals Cu/Zn and toxic metals Cd/Pb indicating metal-metal interactions as pivotal mechanisms of metals toxicity.

Protolytic equilibria of ACE inhibitors in micellar solution of nonionic surfactant Brij 35

AB  - The acid–base equilibria of six ACE inhibitors (ACEIs), captopril, cilazapril, enalapril, lisinopril, quinapril, and rami-
pril, were investigated in the presence of micelles of nonionic surfactant Brij 35. The pKa values were potentiometrically
determined at 25 °C and at a constant ionic strength (0.1 M NaCl). The obtained potentiometric data were evaluated in the
computer program Hyperquad. On the basis of the shift in the pKa values (ΔpKa ) determined in micellar media in relation
to the pKa values previously determined in “pure” water, the effect of Brij 35 micelles on ACEIs ionization was estimated.
The presence of nonionic Brij 35 micelles caused a shift in the pKa values of all ionizable groups of the investigated ACEIs
(ΔpKa from − 3.44 to + 1.9) while shifting the protolytic equilibria of both acidic and basic groups toward the molecular
form. The Brij 35 micelles expressed the most pronounced effect on the ionization of captopril among the investigated ACEIs
and stronger effect on the ionization of amino than on the ionization of carboxyl groups. The obtained results suggest that
ionizable functional groups of ACEIs are involved in interactions with palisade layer of nonionic Brij 35 micelles, which
potentially can be considered in physiological conditions. Distribution diagrams of the investigated ACEIs equilibrium
forms as a function of pH indicate that the change in distribution is most strongly expressed in pH range 4–8, which includes
biopharmaceutically important pH values.

AB  - Background/Aim. One of the complications that can oc-
cur during pregnancy is the development of preeclampsia
(PE). The main characteristics of this condition are high
blood pressure and very often signs of kidney damage or
other organ damage. The condition affects 5–7% of all
pregnant women and is one of the main factors of maternal
and perinatal morbidity and mortality worldwide. The aim
of this study was to investigate the structural and functional
modifications of high-density lipoprotein (HDL) particles
during high-risk pregnancies (HRP) for PE development.
Methods. The longitudinal prospective study included a to-
tal of 91 pregnant women with a HRP for developing PE.
Out of this total number, 71 women did not develop PE
until delivery, and this group was designated as the group
without PE (WPE). The rest of the 20 HRP women devel-
oped PE before delivery and were designated as the PE
group. The blood was sampled toward the end of each tri-
mester and before the delivery. The distribution of HDL
particles was determined by the vertical 3–31% polyacryla-
mide gradient gel electrophoresis method. The antioxidative capacity of HDL particles was measured by the activity of
the HDL-associated enzyme – paraoxonase 1 (PON1).
PON1 activity was determined by the method of kinetic
spectrophotometry from serum samples. Results. The re-
sults have shown that the proportions of HDL 2b particles
significantly increased in the 2nd trimester (p ˂ 0.05) and
remained increased until the end of pregnancy in the WPE
group. PON1 activity was significantly higher in the 3 rd tri-
mester (p < 0.05) of the WPE group. In the PE group, we
found that the proportions of HDL 3a particles significantly
decreased in the 2 nd trimester (p < 0.05) and remained de-
creased until the end of pregnancy. PON1 activity has not
changed in the PE group during pregnancy. Conclusion.
Dyslipidemia in pregnancy could be associated with differ-
ent modifications of HDL particles. The adaptive pregnancy
mechanisms expressed as a functional modification of HDL
particles in pregnant women who develop PE seem inade-
quate and, therefore, lose their atheroprotective role.
AB  - Uvod/Cilj. Jedna od komplikacija koja se može javiti
tokom trudnoće je razvoj preeklampsije (PE). Glavne
karakteristike ovog stanja su visok krvni pritisak i vrlo često
znaci oštećenja bubrega ili drugih organa. Ovo stanje
pogađa 5–7% svih trudnica i jedan je od glavnih faktora
morbiditeta i mortaliteta trudnica i fetusa ili novorođenčadi.
Cilj ovog rada bio je ispitivanje strukturnih i funkcionalnih
modifikacija lipoproteinskih čestica visoke gustine (high-
density lipoprotein – HDL) u trudnoćama sa visokim rizikom
(TVR) za razvoj PE. Metode. U longitudinalnoj
prospektivnoj studiji učestvovala je ukupno 91 trudnica sa
TVR od razvoja PE. Od ukupnog broja trudnica, kod njih 71, PE se nije razvila do kraja trudnoće i ova grupa je
označena kao grupa bez PE (BPE). Kod preostalih 20 žena
sa TVR se razvila PE do porođaja, i one su svrstane u grupu
PE. Krv je uzimana za analizu na kraju svakog trimestra i
pred porođaj. Raspodela HDL čestica je određivana
metodom vertikalne elektroforeze u 3–31% gradijentu
poliakrilamidnog gela. Antioksidativni kapacitet HDL
čestica je određivan na osnovu aktivnosti enzima
paraoksonaze 1 (PON1) vezanog za HDL. Aktivnost
PON1 u serumu je određivana metodom kinetičke
spektrofotometrije. Rezultati. Rezultati su pokazali da se
udeo HDL 2b čestica značajno povećao u drugom trimestru
(p < 0,05) i ostao je povećan do kraja trudnoće u grupi BPE.
Aktivnost PON1 bila je značajno veća u trećem trimestru (p < 0,05) kod ove grupe trudnica. Udeo HDL 3a
čestica se značajno smanjio u drugom trimestru u PE grupi
trudnica (p < 0,05) i ostao je snižen do kraja trudno će.
Aktivnost PON1 enzima u PE gripi se nije menjala u toku
trudnoće. Zaključak. Dislipidemija u trudnoći može biti
posledica različitih modifikacija HDL čestica. Strukturne i
funkcionalne modifikacije HDL čestica, kao jedan od adaptivnih mehanizama, kod trudnica kod kojih se razvila
PE, nisu adekvatne i kao takve gube svoju ateroprotektivnu
ulogu.

Instability Induced by Random Background Noise in a Delay Model of Landslide Dynamics

AB  - In the present paper, we propose a new model for landslide dynamics, in the form of the spring-block mechanical model, with included delayed interaction and the effect of the background seismic noise. The introduction of the random noise in the model of landslide dynamics is confirmed by the surrogate data testing of the recorded ambient noise within the existing landslide in Serbia. The performed research classified the analyzed recordings as linear stationary stochastic processes with Gaussian inputs. The proposed mechanical model is described in the form of a nonlinear dynamical system: a set of stochastic delay-differential equations. The solution of such a system is enabled by the introduction of mean-field approximation, which resulted in a mean-field approximated model whose dynamics are qualitatively the same as the dynamics of the starting stochastic system. The dynamics of the approximated model are analyzed numerically, with rather unexpected results, implying the positive effect of background noise on landslide dynamics. Particularly, the increase of the noise intensity requires higher values of spring stiffness and displacement delay for the occurrence of bifurcation. This confirms the positive stabilizing effect of the increase in noise intensity on the dynamics of the analyzed landslide model. Present research confirms the significant role of noise in landslides near the bifurcation point (e.g., creeping landslides).

AB  - The number of “omics” approaches is continuously growing. Among others, epigenetics has appeared as an attractive area of investigation by the cardiovascular research community, notably considering its association with disease development. Complex diseases such as cardiovascular diseases have to be tackled using methods integrating different omics levels, so called “multi-omics” approaches. These approaches combine and co-analyze different levels of disease regulation. In this review, we present and discuss the role of epigenetic mechanisms in regulating gene expression and provide an integrated view of how these mechanisms are interlinked and regulate the development of cardiac disease, with a particular attention to heart failure. We focus on DNA, histone, and RNA modifications, and discuss the current methods and tools used for data integration and analysis. Enhancing the knowledge of these regulatory mechanisms may lead to novel therapeutic approaches and biomarkers for precision healthcare and improved clinical outcomes.

abstract = "The number of “omics” approaches is continuously growing. Among others, epigenetics has appeared as an attractive area of investigation by the cardiovascular research community, notably considering its association with disease development. Complex diseases such as cardiovascular diseases have to be tackled using methods integrating different omics levels, so called “multi-omics” approaches. These approaches combine and co-analyze different levels of disease regulation. In this review, we present and discuss the role of epigenetic mechanisms in regulating gene expression and provide an integrated view of how these mechanisms are interlinked and regulate the development of cardiac disease, with a particular attention to heart failure. We focus on DNA, histone, and RNA modifications, and discuss the current methods and tools used for data integration and analysis. Enhancing the knowledge of these regulatory mechanisms may lead to novel therapeutic approaches and biomarkers for precision healthcare and improved clinical outcomes.",

AB  - Aims: CYP2C19 transgenic mouse expresses the human CYP2C19 gene in the liver and developing brain, and it exhibits altered neurodevelopment associated with impairments in emotionality and locomotion. Because the validation of new animal models is essential for the understanding of the aetiology and pathophysiology of movement disorders, the objective was to characterise motoric phenotype in CYP2C19 transgenic mice and to investigate its validity as a new animal model of ataxia. Methods: The rotarod, paw-print and beam-walking tests were utilised to characterise the motoric phenotype. The volumes of 20 brain regions in CYP2C19 transgenic and wild-type mice were quantified by 9.4T gadolinium-enhanced post-mortem structural neuroimaging. Antioxidative enzymatic activity was quantified biochemically. Dopaminergic alterations were characterised by chromatographic quantification of concentrations of dopamine and its metabolites and by subsequent immunohistochemical analyses. The beam-walking test was repeated after the treatment with dopamine receptor antagonists ecopipam and raclopride. Results: CYP2C19 transgenic mice exhibit abnormal, unilateral ataxia-like gait, clasping reflex and 5.6-fold more paw-slips in the beam-walking test; the motoric phenotype was more pronounced in youth. Transgenic mice exhibited a profound reduction of 12% in cerebellar volume and a moderate reduction of 4% in hippocampal volume; both regions exhibited an increased antioxidative enzyme activity. CYP2C19 mice were hyperdopaminergic; however, the motoric impairment was not ameliorated by dopamine receptor antagonists, and there was no alteration in the number of midbrain dopaminergic neurons in CYP2C19 mice. Conclusions: Humanised CYP2C19 transgenic mice exhibit altered gait and functional motoric impairments; this phenotype is likely caused by an aberrant cerebellar development.

Lidocaine Clearance as Pharmacokinetic Parameter of Metabolic Hepatic Activity in Patients with Impaired Liver

AB  - Background: The study aimed to estimate lidocaine (LID)
pharmacokinetic parameter values in patients with
impaired liver function, level of correlation between the
pharmacokinetic parameters and Child-Pugh class and
change in pharmacokinetic parameters after liver tumor
resection compared to the preoperative value.
Methods: Patients with impaired liver function were subject
to the LID test 1 day prior to, 3 and 7 days after the inter-
vention. LID was administered in single i.v. dose of 1 mg/kg.
Blood samples were collected at 15, 30 and 90 minutes
after drug administration. Non-compartmental analysis was
applied for calculating the pharmacokinetic parameters.
Results: The study included 17 patients with the diagnosis of
cirrhosis and 41 patients with liver tumor. In both groups of
patients, the values of the coefficients of correlation show
the best correlation between clearance (CL) and Child-Pugh
score (-0.693, p<0.005) over other pharmacokinetic
parameters. The results indicate worsening hepatic function
on 3rd day after operation in comparison to the values of
LID CL prior to operation (mean LID CL for patients with
Child-Pugh class A are 25.91 L/h, 41.59 L/h, respectively;
while for B class are 16.89 L/h, 22.65 L/h, respectively). On
day 7th, the values of LID CL (mean value for patients with
Child-Pugh class A and B are 40.98 L/h and 21.46 L/h,
respectively) are increased in comparison to 3rd day after.
Conclusions: LID pharmacokinetic parameters consequent-
ly changed according to the severity of liver impairment,
assessed by Child-Pugh score. Values of LID CL and vol-
ume of distribution (Vd) coupled with standard biochemical
parameters may be used for preoperative assessment of
liver function and monitoring of its postoperative recovery.

kinetičkih parametara lidokaina (LID) kod pacijenata sa
oštećenom funkcijom jetre, stepena korelacije izme|u
farmakokinetičkih parametara i Child-Pugh klase i promene farmakokinetičkih parametara posle resekcije tumora
jetre u odnosu na preoperativnu vrednost.

Design of experiments and Derringer's desirability function in optimisation and validation of RP-HPLC method for the analysis of enrofloxacin and its impurities

AB  - Using the Design of Experiments methodology (Response-Surface Methodology and Derringer's Desirability Function), a simple, fast and robust RP-HPLC method was developed for the analysis of enrofloxacin (EFC), its impurity A (fluoroquinolonic acid, FQ) and impurity B (ciprofloxacin, CPX). Gradient elution of samples was performed on a Zorbax Eclipse XDB C18 column (150 × 4.6 mm, 3.5 μm) with a mobile phase consisting of 32 mM phosphate buffer pH 3.5-methanol (0 min-19.6% methanol; 15.5 min-19.6% methanol; 29.5 min-80% methanol; 30 min-19.6% methanol; 35 min-19.6% methanol), delivered at a flow rate of 1.5 mL min-1, wavelength of detection 278 nm (for EFX and CFX) and 265 nm for FQ. A good linear response was achieved in the range 15-35 μg mL-1 (EFX) and LOQ-150% for impurities (CFX and FQ). Other validation parameters were also tested: precision, accuracy, sensitivity and robustness. The developed method was shown to be simple, practical and suitable for the analysis of EFC and its impurities (CPX, FQ) in veterinary drugs.
