AB  - Background: Biomarkers of oxidative stress are relevant in the evaluation of the disease status and prooxidant-antioxidant balance, advanced oxidation protein products and lipid peroxidation products (malondialdehyde and 4-hydroxynonenal) are being extensively evaluated regarding their relationship with clinical presentation and disease severity.Aim of the study: The aim of this study was to evaluate the levels of the above-mentioned parameters in plasma of 39 men and 17 women with Parkinson's disease, originated from the Republic of Serbia and their relation to clinicopathological characteristics (gender, age at examination, duration of the disease, and Hoehn and Yahr score) and oxidative status.Results: The incidence of disease was 2:1 towards males. The investigated oxidative parameters were gender and Hoehn and Yahr related. Significant association of higher Hoehn and Yahr scores was observed for malondialdehyde (p = 0.01) and prooxidant-antioxidant balance (p = 0.02). Relation between oxidant-antioxidant status was further supported by observed positive correlation between 4-hydroxynonenal (p = 0.04) and prooxidant-antioxidant balance (p = 0.03). Finally, the multivariate analysis indicated that prooxidant-antioxidant balance and malondialdehyde were partially determined by gender (10.6% and 7.6%) and Hoehn and Yahr scores (13.6% and 18.8%), while Hoehn and Yahr scores contributed to the variance of advanced oxidation protein products with 13.2%.Conclusion: Our results indicate the higher level of oxidative stress (oxidant-antioxidant imbalance) and possible relation of several markers with gender and disease stage in patients with Parkinson's disease. The analyzed markers could be used to specify the severity of oxidative stress; however, their potential value should be analyzed in further studies.

AB  - Background: The aim of this study was to determine the reference values for thyrotropin (TSH), thyroid hormones (total and free thyroxine, T4 and fT4; total and free triiodothyronine, T3 and fT3), thyroglobulin (Tg) and thyroid antibodies (thyroid peroxidase, TPOAb and thyroglobulin antibody, TgAb) in the population of the Republic of Srpska. Methods: A total of 250 euthyroid subjects were enrolled in this study. A direct method for choosing reference subjects was used to establish reference intervals. The hormones and thyroid antibodies were measured by an electrochemiluminescence immunoassay method (ECLIA, Roche Diagnostics, Mannheim, Germany). We calculated the re f erence intervals by MedCalc, version 12.1.4.0 (MedCalc software, Belgium) as recommended by the IFCC (CLSI C28-A3). Results: Using guidelines recommended by the National Academy of Clinical Biochemistry (NACB) and based on standard statistical approaches, the reference intervals derived for TSH, fT4, T4, fT3, T3 were 0.75-5.32 mIU/L, 12.29-20.03 pmol/L, 73.49-126,30 nmol/L, 4.11-6.32 pmol/L, 1.15-2.32 nmol/L and for Tg, TPOAb, TgAb were 3.63-26.00 mu g/L,  lt  18.02 mIU/L,  lt  98.00 mIU/L, respe ctively. We found a significant difference (p lt  0.05) in TSH and fT3 values between different age groups as well as in T4, fT4 and fT3 values between ge nder groups. Conclusions: The established reference values for the population of the Republic of Srpska were significantly different from the values recommended by the manufacturer of reagents (Roche Diagnostics). Our results showed that a laboratory needs to establish its own reference values in order to set up a proper diagnosis, as well as to treat patients successfully.

Prooxidant-antioxidant balance, hsTnI and hsCRP: mortality prediction in haemodialysis patients, two-year follow-up

AB  - Oxidative stress and inflammation are highly intertwined pathophysiological processes. We analyzed the markers of these processes and high-sensitive troponin I (hsTnI) for mortality prediction in patients on haemodialysis. This study enrolled a total of 62 patients on regular haemodialysis. The patients were monitored for two years, and the observed outcomes were all-cause and cardiovascular mortality. Blood samples were taken before one dialysis session for analysis of the baseline concentrations of prooxidant-antioxidant balance (PAB), total antioxidant status (TAS), total oxidative status (TOS), hsTnI, hsCRP and resistin. The overall all-cause mortality was 37.1% and CVD mortality 16.1%. By univariate and multivariate logistic regression, our findings suggest that good predictors of all-cause mortality include hsCRP and PAB (p lt .05) and of CVD mortality hsCRP (p lt .05) and hsTnI (p lt .001). To evaluate the relationship between the combined parameter measurements and all-cause/CVD mortality risk, patients were divided into three groups according to their PAB, hsCRP and hsTnI concentrations. The cutoffs for hsCRP and hsTnI and the median for PAB were used. Kaplan-Meier survival curves pointed out that the highest mortality risk of all-cause mortality was in the group with hsCRP levels above the cutoff and PAB levels above the median (p lt .001). The highest risk of CVD mortality was found in the group with hsCRP and hsTnI levels above the cutoff levels (p =.001). Our data suggest that hsCRP and PAB are very good predictors of all-cause mortality. For CVD complications and mortality prediction in HD patients, the most sensitive parameters appear to be hsTnI and hsCRP.

AB  - We investigated the role of serum uric acid (sUA) and superoxide dismutase (SOD) as predictive factors for mortality in hemodialysis (HD) patients. SOD, butyrylcholinesterase, and malondialdehyde were estimated spectrophotometrically and the other parameters by standard procedures. High-sensitive C-reactive protein was assayed by a sandwich ELISA method. sUA among survivors (112.1 +/- A 13.82 mu mol/L) was significantly lower than in deceased (160.8 +/- A 16.81 mu mol/L, p  lt  0.001), while SOD was higher in survivors (31.8 +/- A 6.61 kU/L) than among deceased (20.2 +/- A 3.03, p  lt  0.05). Kaplan-Meier survival curves showed the greatest mortality risk in the highest tertile of basal sUA concentration (a parts per thousand yen127.11 mu mol/L, p  lt  0.001), and for SOD in the lowest tertile (a parts per thousand currency sign23.83 kU/L, p  lt  0.05). Our results suggest that high sUA and low SOD may predict all-cause and cardiovascular mortality in HD patients.

AB  - Objective: The aim of this study was to investigate whether high bilirubin concentration is a protective factor in cardiovascular disease (CAD) and how it correlates with parameters of oxidative stress in young males and females. Methods: The study comprised 628 healthy subjects of both genders, 18-22 years of age. In fasting sera the concentration of total bilirubin (Tbil), parameters of cardiovascular risk and oxidative stress were determined. The results were analyzed by appropriate statistical methods. Results: We found no gender differences in body mass index (BMI), blood pressure and lipid profile between subjects with low and high Tbil level. Men with high Tbil had higher concentrations of albumin and uric acid (p  lt  0.001) and lower of oxLDL ( lt 0.05), while women had higher albumin (p  lt  0.05) and lower TBARS (p  lt 0.05). Significant positive correlation in men was found between Tbil, uric acid and albumin, while for glucose and TSARS this association was negative. In female significant positive correlation was between Tbil, HDL-C, fibrinogen, albumin and uric acid and negative between Tbil and TBARS. The high concentration of Tbil in men was independently associated with uric acid (p  lt  0.05) and oxLDL (p  lt  0.001), while in women it was independently associated with TBARS (p  lt 0.05). After adjustment for traditional lipid parameters the predictive power of high bilirubin in men remained for uric acid (p  lt  0.001) and TBARS in women (p  lt  0.05). Conclusion: These findings jointly support the concept that bilirubin via its antioxidant potential has a protective effect against cardiovascular disease in young male and female.

A pilot study of thyroid testing in Montenegro

AB  - Context. There is a lack of evidence On thyroid hormonal status and the prevalence of thyroid autoimmune diseases in Montenegro. Objective. In order to get an idea about that we performed thyroid function tests on ambulatory patients from different areas of Montenegro. Design. The study took place since November 2005 till March 2007 at the Clinical Center of Montenegro. Subjects and methods. The study included 277 subjects of both sexes, referred for serum thyroid-stimulating hormone (TSH), thyroxin (T4) and free thyroxin (FT4), thyroperoxidase antibody (TPOAb) and thyroglobulin antibody (TgAb). Also a multi-step statistical analysis of the biochemical data was performed. Results. An age-dependent increment in serum TSH was detected in both males (p = 0.001) and females (p = 0.034). In females significant changes of T4 with age were detected (p = 0.017), but not in males (p = 0.427). Neither in males nor in females changes in FT4 were found (p = 0.342 and p = 0.831, respectively). A univariate logistic model demonstrated an association between thyroid antibodies and TSH, indicating that TSH >= 7.15 mU/L. was a better predictor of TPOAb (p = 0.001) than of TgAb (p = 0.01) presence. A multivariate model adjusted for both age and gender gave similar results. The highest TSH increment (86%) was found in sera containing both antibodies, while 33% of antibodies negative persons had TSH above 7.16 mU/L. Conclusion. The results demonstrate high percentage of subclinical thyroiditis among the investigated subjects.

Negative but not significant correlation between BuChE and high-density lipoprotein cholesterol (HDL-C) was detected for both genders. Conclusion: The tendency towards overweight in young male could explain our results on BMI as an independent risk factor for higher BuChE in young male. Glucose as an independent risk factor for higher BuChE activity in females indicates that BuChE may be a predictor of diabetes.

AB  - The population of Serbia is characterized by very low fish intake and a similarly inadequate intake of other natural dietary sources of n-3 fatty acids (FAs). On the other hand, the use of fish oil dietary supplements has increased as well as the number of different commercially available products with fish oil. The effect of fish oil supplement use under real-life conditions on the erythrocyte total FA profile was investigated in this study. Fifteen middle-aged participants without acute health problems but with slightly increased body weight and mild hyperlipiderma were chosen as representatives of the average fish oil supplement consumers in Serbia. For 8, weeks participants were supplemented with fish oil in doses that are commonly used among supplement consumers: 2 fish oil capsules per day, each with a declared content of 180 mg eicosapentaenoic acid (20:50) and 120 mg docosahexacnoic acid (22:60). Erythrocyte lipids were extracted and then analyzed for FA composition by gas chromatography. A significant decrease was observed in oleic and 20:3n9 FAs (P  lt  .05), whereas an increase in eicosapentaenoic acid (0.8 +/- 0.17 to 1.1 +/- 0.59) and docosahexaenoic acid (2.7 +/- 0.87 to 3.5 +/- 1.09) was noted. The ratio of n-6 to n-3 FAs significantly decreased after fish oil supplementation (P  lt .05). Our findings support the practice of moderate fish oil supplement use in the middle-aged slightly overweight and hypercholesterolemic individuals based on its effect on erythrocyte total FA profile.

Diagnostic value of CSF proteins assay in differential diagnosis of CNS disease

AB  - The differential diagnosis between inflammatory and noninflammatory diseases of CNS includes CSF quantitative measurements of immunoglobulin and albumin concentrations and their comparison to the serum. For that purpose we estimated the concentrations of albumin and immunoglobulin G both in CSF and serum of patients with inflammatory and noninflammatory diseases of CNS. Parallely, these parameters were determined in the control group composed of patients having the values of investigated parameters within reference ranges. The obtained data were statistically evaluated by nonparametric Mann-Whitney test. The values of calculated CSF/albumin ratio have shown significant differences between the both patient groups and the control one (p  lt  0.001). It proves that in each examined group the blood brain barrier is damaged. For the assessment of intrathecal IgG synthesis we have calculated both IgG/albumin index and Schuller index. The comparison of the values obtained for patients with noninflammatory diseases to the control one, revealed no statistically significant differences (p > 0.05). On the contrary, significant difference have been observed between the group with inflammatory diseases and the control (p  lt  0.05), proving the existence of IgG intrathecal synthesis. Values of both indexes were much higher in patients with inflammatory than noninflamatory diseases, meaning that they can be used in differential diagnosis.
AB  - Diferencijalna dijagnoza inflamatornih i neinflamatornih oboljenja CNS-a, obuhvata kvantitativno određivanje koncentracija imunoglobulina i albumina, kao i njihovo poređenje sa serumom. Koncentracije albumina i imunoglobulina G određene su u cerebrospinalnoj tečnosti (CST) u serumu pacijenata sa inflamatornim i neinflamatornim oboljenjima CNS-a. Ovi parametri određeni su i u kontrolnoj grupi, sačinjenoj od pacijenata kod kojih su ispitivani parametri bili u granicama referentnih vrednosti. Dobijeni rezultati statistički su obrađeni neparametarskim Mann-Whitney testom. Izračunate vrednosti CSF/albumin pokazale su statistički značajne razlike između obe grupe pacijenata i kontrolne grupe (p lt  0,001). Ovo dokazuje da je kod obe grupe pacijenata krvno moždana barijera oštećena. Za procenu intratekalne sinteze IgG izračunati su Ig/albumin indeks i Schuller-ov index. Poređenjem vrednosti za pacijente sa nezapaljenskim oboljenjima i kontrolne grupe nisu nađene značajne razlike (p > 0,05). Suprotno, statistički značajne razlike uočene su između grupe sa inflamatornim oboljenjima i kontrolne grupe (p  lt  0,05), što ukazuje na postojanje intratekalne sinteze IgG-a. Vrednosti izračunatih indeksa za zapaljenske procese bile su značajno više u odnosu na nezapaljenska oboljenja (p  lt  0,05), što se može iskoristiti za diferencijalnu dijagnozu ovih oboljenja.

Oxidative stress in blast-induced acute lung injury is independent of enzymatic nitric oxide production

AB  - Lung trauma has been considered to be one of the vital injuries induced by explosion-generated blast overpressure. Conflicting evidence exists as to whether nitric oxide plays a crucial role in acute lung injury induced by blast. Data presented in this study demonstrate that local exposure of midthoracic region to moderate-level blast overpressure significantly enhanced lipid peroxidation product malondialdehyde and superoxide anion generation in rabbit's lungs 30 minutes after exposure, whereas the activities of antioxidant enzymes (superoxide dismutase, glutathione peroxidase) activity showed parallel increase. NG-nitro-L-arginine methyl ester, a non-specific inhibitor of nitric oxide synthase (NOS), had no effects on the measured parameters suggesting that oxidative stress induced by blast exposure might be independent from NOS.
AB  - Smatra se da je povreda pluća jedna od najvažnijih povreda do kojih dolazi delovanjem blast talasa generisanog na mestu eksplozije. Do danas ne postoji definitivan stav da li azot-monoksid ima bitnu ulogu u akutnoj blast povredi pluća. Rezultati ove studije pokazuju da lokalno delovanje blast nadpritiska srednje jačine na središnji deo toraksa dovodi do povećanog stvaranja malondialdehida, produkta lipidne peroksidacije kao i povećanog generisanja superoksidnog anjona, 30 minuta posle traume. Istovremeno, dolazi do smanjenja aktivnosti antioksidativnih enzima (superoksid-dizmutaze i glutation-peroksidaze) u plućnom tkivu kunića. NG-nitro-L-arginin-metil estar (L-NAME), nespecifični inhibitor enzima azot-monoksid sintaze (NOS) nije imao efekta na određivane parametere, što ukazuje daje oskidativni stres indukovan blast povredom verovatno nezavisan od aktivnosti enzima NOS.

Parameters of oxidative stress and antioxidative defence

AB  - At present, the mechanisms of many diseases (cardiovascular diseases, cancer diabetes, rheumatoid arthritis, hepatitis, atherosclerosis, haemorrhagic shock, AIDS etc) are explained through interaction of oxidative stress (OS) mediators and antioxidative defence (AOD) sistem factors. It is important to develop new and evaluate existing analytical procedures for determination of OS mediators and methods for evaluation of antioxidative status. More frequent use of these tests will be very useful for evaluating organism status in health and disease, for monitoring the effects of therapies. These tests will be beneficial for judgment if antioxidant supplementation is necessary or therapy with vitamin E or selenium. All cells normally generate superoxide anion radical (O2 -), hydrogen peroxide (H2O2), hydroxyl anion radical (OH -), nitric oxide (NO), peroxynitrite anion (ONOO -). In cellular injury, during hypoxia and following reperfusion, increased amounts of free radicals can generate. This event can aggravate tissue injury. AOD system (albumin, uric acid, glutathione, vitamin E, vitamin C, SOD, GPx catalase, selenium, caeruloplasmin, transferrin) is very important physiological regulator in health and disease.
AB  - Danas se mehanizmi velikog broja bolesti (kardiovaskularne bolesti, kancer dijabetes, reumatoidni artritis, hepatitis, ateroskleroza, hemoragijski šok AIDS i dr) objašnjavaju interakcijom činilaca oksidativnog stresa (OS) i antioksidativne (AO) zaštite. Zbog toga je značajno razvijati nove i evaluirati postojeće metode za merenje pokazatelja oksidativnog stresa i to slobodnih radikala, njihovih reaktivnih intermedijera i produkata peroksidacije lipida, kao i metode za procenu antioksidativnog statusa organizma. Šira primena ovih testova bi bila značajna za procenu stanja kako već obolelog, tako i zdravog organizma u cilju prevencije potencijalnih bolesti, za praćenje efikasnosti primenjene terapije kao i procene da li je potrebna suplementacija ishrane antioksidansima ili uvođenje terapije vitaminom E ili selenom. U toku normalnog metabolizma u ćeliji se stvaraju superoksidni anjon (O2.-), vodonik peroksid (H2O2), hidroksilni anjon radikal (OH -), azot monoksid (NO), peroksinitritni anjon (ONOO -). U slučaju oštećenja/povrede ćelije, usled hipoksije i posledične reperfuzije dolazi do povećanog stvaranja slobodnih radikala, koji sami ili preko svojih reaktivnih produkata, produbljuju oštećenje tkiva. Sistem AO zaštite organizma (albumin, mokraćna kiselina, glutation; vitamini E i C; enzimi SOD, GPx, katalaza, selen; ceruloplazmin i transferin) je zbog toga značajan fiziološki regulator kako u toku normalnih metaboličkih procesa tako i u patološkim stanjima.

These tests will be beneficial for judgment if antioxidant supplementation is necessary or therapy with vitamin E or selenium. All cells normally generate superoxide anion radical (O2 -), hydrogen peroxide (H2O2), hydroxyl anion radical (OH -), nitric oxide (NO), peroxynitrite anion (ONOO -). In cellular injury, during hypoxia and following reperfusion, increased amounts of free radicals can generate. This event can aggravate tissue injury. AOD system (albumin, uric acid, glutathione, vitamin E, vitamin C, SOD, GPx catalase, selenium, caeruloplasmin, transferrin) is very important physiological regulator in health and disease., Danas se mehanizmi velikog broja bolesti (kardiovaskularne bolesti, kancer dijabetes, reumatoidni artritis, hepatitis, ateroskleroza, hemoragijski šok AIDS i dr) objašnjavaju interakcijom činilaca oksidativnog stresa (OS) i antioksidativne (AO) zaštite. Zbog toga je značajno razvijati nove i evaluirati postojeće metode za merenje pokazatelja oksidativnog stresa i to slobodnih radikala, njihovih reaktivnih intermedijera i produkata peroksidacije lipida, kao i metode za procenu antioksidativnog statusa organizma. Šira primena ovih testova bi bila značajna za procenu stanja kako već obolelog, tako i zdravog organizma u cilju prevencije potencijalnih bolesti, za praćenje efikasnosti primenjene terapije kao i procene da li je potrebna suplementacija ishrane antioksidansima ili uvođenje terapije vitaminom E ili selenom. U toku normalnog metabolizma u ćeliji se stvaraju superoksidni anjon (O2.-), vodonik peroksid (H2O2), hidroksilni anjon radikal (OH -), azot monoksid (NO), peroksinitritni anjon (ONOO -). U slučaju oštećenja/povrede ćelije, usled hipoksije i posledične reperfuzije dolazi do povećanog stvaranja slobodnih radikala, koji sami ili preko svojih reaktivnih produkata, produbljuju oštećenje tkiva. Sistem AO zaštite organizma (albumin, mokraćna kiselina, glutation; vitamini E i C; enzimi SOD, GPx, katalaza, selen; ceruloplazmin i transferin) je zbog toga značajan fiziološki regulator kako u toku normalnih metaboličkih procesa tako i u patološkim stanjima.",

AB  - We propose a simple and precise densitometric method for measuring vanillylmandelic acid (VMA). The method comprises direct urine application, thin-layer chromatographic (TLC) separation, postchromatographic derivatization, and in situ reading of the formed derivative at 560 nm. Quantitation was done by measuring peak area with a computer-controlled TLC Scanner and applying a five-point calibration function. The calibration function was 7.57-49.23 μmol/L. This method was used to determine urinary concentrations of excreted VMA; concentrations ranged from 4.84 to 56.06 μmol/day.
